Mr. Dennis Barnette, BOH Chair, called the meeting to order at 12:13pm and welcomed members

member to the Board of Health as we continue to look for

someone to fill the Optometrist position.

Nursing by Ms. Brandi Poplin, Director of Nursing.

PROGRAM HIGHLIGHTS: COMMUNICABLE DISEASE AND CLINICAL SERVICES – MS. BRANDI POPLIN, DIRECTOR OF NURSING

Accreditation Activity 2.4. Ms. Brandi Poplin reports the Health and Nutrition Center Communicable

Disease program operates under CDC guidelines and utilizes State Consultants. Agreement Addendums

for TB control, HIV/STD, and general communicable disease control follow General Statute 130A,

Article 6. Revenue for the CD Program for FY Budget 2016-2017 totals $288,474.00 coming from

county, state, Medicaid, patient fees, and other sources. There are over 80 reportable diseases for the state

of North Carolina and more information can be found at

o Revenue for School Nurse Funding Initiative (SNFI) are used to employ five nationally

certified school nurses in the three school districts. They work closely with school nurses and

administrators to ensure immunization compliance.

Reviewed Health and Nutrition Center 2017 Strategic Plan and Agency Goals with discussion following.

o Board agreed to support the Health & Nutrition Center 2016 Strategic Plan overall, as a

whole, including the goals of:

Health clinical services

Fee Schedule Accreditation Activities 33.5, 37.2, 37.6, & 39.3.

o Overview of Fee Schedule was presented and circulated among Board Members for review with

discussion regarding little difference from last year’s Fee Schedule.

Debt Set-off Accreditation Activity 33.7

o Debt Set-off submitted for $32,321.41 with $7,399.17 having been collected for 2016.

o Write-off amount for the past three years 2016 - $20,278.01

Motion made by Ms. Brenda Miller to accept the Fee Schedule, Debt Set-of, and Debt Write-off as

Current Budget Accreditation Activity 33.6

o Expenditures targeted at 42% and resulted at a lower 37.1%.

Members voiced pleasure with fiscal totals.

Audits and Surveys

o WIC, STD, Child Health, Primary Care Clinic, Nutrition, Diabetes, Immunizations, Family

Planning, Environmental Health, Dental, Behavioral Health, and BCCCP all had good results

with small issues that were corrected immediately – nothing alarming.

2017 Meeting Schedule/Programs

Discussion regarding proposed meeting dates, times, and location. Agreed to keep with the meetings

on the first Wednesday of every other month with locations alternating between Mount Airy and

Dobson in a restaurant meeting room. Agree to change meeting time from 12:00pm to 1:00pm

beginning with next meeting of March 01, 2017.

WELCOME AND CALL TO ORDER– MR. DENNIS BARNETTE, BOH CHAIR Mr. Dennis Barnette, BOH Chair, welcomed everyone and called the meeting to order at 1:05pm. PROGRAM HIGHLIGHTS: 2016 STATE OF THE COUNTY HEALTH REPORT, COMMUNITY HEALTH RANKINGS, COMMUNITY HEALTH ASSESSMENT, AND CHILD FATALITY PREVENTION TEAM Ms. Maggie Simmons, Administrative Director began with a brief description of the 2016 State of the County Health Report (SOTCH), a document which outlines how Surry County Health and Nutrition Center is addressing the top 3 priorities that were selected from the 2014 Community Health Assessment. The SOTCH report includes basic demographic data, new and emerging issues affecting health, morbidity and mortality data, priority interventions, and new initiatives. Surry County’s priority initiatives are:

• Obesity o International Walk to School Day o Tai Chi for overall health and Arthritis

• Substance Abuse Prevention o Worksite Tobacco Cessation o Mission Possible o Youth Risk Behavior Survey (YRBS) o Candy versus Medication Boards

• Access to Affordable Healthcare o Tobacco Cessation

o Cancer Screenings The top 5 leading causes of death in Surry County to be Cancer (21.3%), Heart Disease (18.7%), Chronic Respiratory Disease (7.7%), Cerebrovascular Diseases (4.1%), and Alzheimer’s disease (3.8%). BOH question regarding if Cancer statistics were broken down by municipality within the county and why not. Surry County Health Rankings: Surry County Health Rankings showed improvement in 2016. Surry County moved up in rankings from 53rd to 46th in Health Outcomes and from 64th to 38th in Health Factors. Health Outcomes:

• Diabetes—11% • Infant Mortality—9 • Child Mortality—50.3

Health Behaviors:

• Food Insecurity—16% • Motor Vehicle Crash Deaths—17 • Drug Poisoning Deaths—19

Health Care:

• Uninsured Adults—20% • Uninsured Children—7% • Could not see a doctor due to cost—18%

Social/Economic Factors:

• Median Income—$37,900 • Children Eligible for Free Lunch—50%

CFPT The final topic was the Child Fatality Prevention Team (CFPT). Ms. Simmons did a brief breakdown of the ten deaths reviewed for 2016. They are as follows:

• Congenital Conditions/Prematurity—4 deaths • Medical Illness—4 deaths

o Pneumonia/respiratory failure – 1 o Brain damage/renal failure/respiratory arrest – 1 o Primary atelectasis of newborn – 1 o Protein-energy malnutrition/acute intestinal vascular – 1

• Motor Vehicle Accident—1 death • Smoke/Fume Inhalation—1 death

BOH discussion about the situations that preceded causation for the deaths. More discussion regarding who attends the CFPT meeting and how they are determined.

NEW BUSINESS

• New Environmental Health Fees—Johnny Easter, Environmental Health Director

o Environmental Health requests to implement two new fees. Well Sample Kit for $125, which would provide citizens a $90 savings. Engineered Option Fee (EOP), 30% of the permit fee. This fee would be

implemented as a result of local level changes allowed by NC G.S. 130A-336.1.

Motion made by Mr. Eddie Jordan to approve and implement the Well Sample Kit fee and Engineered Option Permit (EOP). Dr. Mark Hauser seconded the motion. Motion approved. • BOH Operating Procedure

o Operating Procedures were reviewed by Members with no changes noted.

Motion made by Ms. Emma Jean Tucker to approve and adopt the Operating Procedures as written for 2017. Mr. Eddie Jordon seconded the motion. Motion approved. • Community Outreach Survey--2016

o Limited return of feedback from our 2016 survey campaign with only 23 completed surveys. 57% report having not received services from our Health & Nutrition Center. 75% report they are somewhat or very likely to use our services in the next year. 74% report the hours of service for our Health & Nutrition Center as convenient. 75% report interest in most of Health & Nutrition Center services, especially Adult

Health, Dental, and Senior Services. 0% report any ideas or suggestions 57% report they would not come to our Animal Control facility when seeking a pet. 78% report not using Health & Nutrition Center website to review EH restaurant

inspections.

Board discussion regarding challenges with getting people to participate in surveys. • Annual Report - 2016

o Board received and reviewed copies of the 2016 Annual Report. Topics reviewed: 10 Essential Public Health Services Goals and Objectives Trend of services in relation to budget Services by the numbers

Board discussion on why cats are euthanized in much higher numbers than dogs.

• 2017-2018 Budget Approval

o SCHNC budget was presented by Ms. Samantha Ange: At approximately eight million dollars excluding personnel expansion Requested a 2.7% increase but not from County allocations Approximately 78% of the budget is employee salary

• Increase in for Animal Control Officers as incentive to deflect employee poaching offers from competing counties.

Lost funding revenue of approximately +/- $90,000.00 from several programs

o The projected total spending amount year to date was projected for 58%, and currently SCHNC is currently under budget at 53%.

o Is a symptom of a problem within the structure. o Equates mold and allergies similarly to pollen and allergies (it bothers some people and not

others.) o No regulations or standards governing mold. o Complaints previously managed through the State but now will be handled on a local level

through Environmental Health. o Environmental Health Staff have received extensive training and certification regarding mold.

Their role will primarily be to abate the situation through education and guidance. o Any more serious problems will be referred up to the State.

• Legal Updates o Pending Bills in the General Assembly:

Senate Bill 131-Regulatory Reform Act of 2016-2017 • Clarifies private drinking water wells permitting and inspection.

Environmental Health will continue to inspect each well for water quality but is not required to inspect the pump and lines leading to the well.

Mr. Dennis Barnette, BOH Chair, welcomed everyone and called the meeting to order at 1:04pm. Mr.

Barnette made note that this is his last meeting for serving as Chair and the last meeting where he and Mr.

Mitch Williams will serve as members on the Board of Health as their terms will end August 2017

completing nine years of service each.

BOH discussion about differences in partnerships between the hospitals (Northern Hospital of Surry and

Hugh Chatham Memorial.) Hugh Chatham conducts dialog and referrals with our Health Center while

Northern is mainly outreach. Discussion regarding school health and federal monies. Partnerships

address Senior Activities, Nutrition, Diabetes, Obesity, Drugs, Bullying, and all stages of the human life

span. Ms. Cave reported Project Lazarus had notable impact on granddaughter who participated in one

of the school outreaches.

2017-2018 Officer Transitions o Mr. Barnette spoke to Board Members about their responsibilities to service with emphasis

on community and financial stewardship.

o Nominations made for Ms. Cave as Chair and Mr. Jordan as Vicechair. Commissioner

Golding moved to close the nominations.

Health Director Update o Grants

Applied for a $6,800.00 Blue Cross Blue Shield Foundation Grant towards a

Bilirubin Meter. This meter will save parents making a separate trip with billing to

the local hospitals and the heal sticks on the babies.

Applied for a $8,000.00 NC Department of Health & Human Services Anne Wolf

Mini-grant to purchase a two-year supply of Sleep Saks as part of the Nutrition

Centers SIDS Reduction Campaign.

Board of Health has requested the Health Director obtain any and all grants that fit

within the mission and provide updates on application submissions at each scheduled

meeting.

Motion made by Ms. Cave to continue, until otherwise voted on, procedure of seeking out secondary

funding and grant sources for application, follow-up, and afterward reporting to the Board of Health on

outcomes. Commissioner Golding seconded the motion. Motion approved.

Annual Health Director Legal Conference – Ms. Maggie Simmons, Assistant Health Director Ms. Simmons passed around the notebook of materials and highlighted legalities presented during the

conference for members to review during her report.

PUBLIC COMMENT – MR. JACK MOORE, SURRY COUNTY HEALTH FOUNDATION

2nd Annual Mayberry Pickleball Tournament Fundraising Event

o Mr. Jack Moore reported their experience with the June 16 -18 Pickleball Fundraiser was

even better than last years’. They had 92 athletes from NC, SC, TN, VA, and GA to

participate with 18 volunteers. The tournament committee would have even more potential

players if other space options were available.

o Mr. Moore reviewed details of the event including accommodations and feedback.

o Raised $5,615.95 ($2,299.93 more than last year) through sponsorships, donations, and

athlete registrations.

o Samantha Ange expressed appreciation to the Health Foundation for working so diligently

and for their passion to service in providing the Health & Nutrition Center with financial

support through their mini-grant program.

Discussion about how smooth the fundraiser went and what kind of impact the event has on the local

economy and Health & Nutrition Center programs. Members also complimented the Health Foundation

on a job well done.

o Animal shelter has been running at a “no kill rate,” for dogs, for the past three years, meaning that the only dogs euthanized are ones who are diseased, maimed, or aggressive.

o The shelter partners with over 100 different animal rescue organizations, 40 of whom are regulars, to rehome all eligible pets for adoption.

BOH discussion about the many partnerships with animal rescue organizations and pride of maintaining the no-kill dog euthanasia rate at the animal shelter.

o Performance Evaluation – Mr. Barnette and Ms. Cave received positive feedback from Board Members regarding Health Director performance, stating “she is doing an excellent job.” Mr. Barnette, as Chair, recommended continued Board support of Ms. Ange as Health Director.

• Health & Nutrition Center Agency Partnerships list was reviewed with Board of Health. o North West Partnership for Public Health (NWPPH) – is a regional Health Director collaborative o School Superintendents – meets quarterly for collaboration between school systems and public health o Healthy Surry – “think tank” collaborative with various community stakeholders o Child Fatality Prevention Team (CFPT) – will be combining soon with the Community Child

Protection Team (CCPT) from Department of Social Services o Juvenile Crime Prevention Council (JCPC) – efforts to help juveniles in the community. o Partners Behavioral Health – grants to support behavioral health initiatives within our agency o Program Specific Partnerships – 60 plus partnerships with various programs within our Health &

Nutrition Center. o Surry County Health Foundation – supports various Health & Nutrition Center initiatives through their

mini-grant project. • Health Director Update

o Animal Control Program Ms. Ange has been conducting intense conversation with Board of County

Commissioners regarding possible transferring of the Animal Control unit from under our Health & Nutrition Center to under the County Sheriff Department. She reports that Health’s responsibilities with Animal Control (AC) is less than 10% and is in regard to General Statutes for rabies control. The Sheriff’s Department would be a better fit for remaining Animal Control responsibilities such as ordinance enforcement and community protection from stray, feral, and wildlife animals. Some minor in-house changes will need to be made within both agencies to make for a smooth transition. Ms. Ange and Mr. Doby had only kind words to say about Sheriff Combs and his interest in bringing the AC program to his department.

WELCOME AND CALL TO MEETING– MS. DEBBIE CAVE, BOH CHAIR Ms. Debbie Cave, BOH Chair, welcomed everyone to meeting at 6:05pm. Ms. Barbara Long was introduced as our new public member and made the oath of membership. OLD BUSINESS – MS. DEBBIE CAVE • July 05, 2017 Minutes • September 16, 2017 Minutes Minutes reviewed by members with no changes noted. Motion made by Commissioner Golding to approve both minutes as written. Dr. Scott Baird Seconded the Motion. Motion approved. NEW BUSINESS – MS. DEBBIE CAVE • Board Member Transitions

o Ms. Barbara Long begins her first three-year term as Public Representative on October 02, 2017.

o Dr. Scott Baird retiring from Board of Health December 31, 2017 after nine years of service to our Surry County community.

o Dr. Drew Nowlin will begin his first three-year term of service as the Pharmacist Representative on January 01, 2018

• Animal Control – Mr. Abraham Doby, Animal Control Director o Rule Rescinding

The Local County Board of Health Rule will be rescinded due to the Rule being adopted into Stale Law.

Discussion of Rule rescinding and standards being adopted by State.

• SCHNC Strategic Planning is scheduled for December 11, 2017 at the Round Peak Vineyards from

9:00am to 4:00pm. • NW Partnership for Public Health (NWPPH) Renewal

o Ms. Samantha Ange is a member and our representative.

Discussion of Partnership benefits with Public Health agencies. Motion made by Commissioner Golding to keep Ms. Ange as representative and renew our membership with the NWPPH. Motion Seconded by Dr. Scott Baird. Motion passed.

• SCHNC Christmas Party 2017

o December 12, 2017 at the First Baptist Church in Dobson o Request for an additional 90 minutes to lunch period to allow for all employees to participate

in team-building activities and celebrate the holiday season. Discussion of importance for employee’s participation in team building activities and socialization with peers. Motion made by Dr. Scott Baird to extend SCH&NC employee lunch period December 12, 2017 from 11:30am to 2:00pm. Motion Seconded by Ms. Emma Jean Tucker. Motion Passed. • BOH Annual/New Member Training

o Training materials were emailed out to all Board Members. o All Members are responsible for reviewing and completing the training materials. o Members are to advise of completion of training via email.

HEALTH DIRECTOR REPORTS – MS. MAGGIE SIMMONS • Current Budget FY 2017-2018

o The total spending amount year to date was projected for 25.0%, and SCHNC is currently under budget at 23.1%.

• 2018 Meeting Schedule o The new 2018 BOH meeting schedule was distributed and reviewed. No questions noted.

• Health Director Update o Animal Control Program

Animal Control will be transitioning to Sheriff’s Department. Effective January 1, 2018.

Euthanizing less animals. Boosters when animals come in, titer day 0 and day 5. Booster and a 45-day home

quarantine if previously vaccinated. This saves money on quarantine time. o Opioids Response Director

Dr. Scott Baird mentioned he received a letter from County Superintendent Chris Knopf requesting to be part of the Opioid Task Force. Maggie Simmons explained the BOCC established an Opioid Director position. When the Director position is filled then the Opioid Task Force will be filled as well. She stated it is important for Dr. Baird to give his input as a local pharmacist, as he sees prescription drug abuse every day.

Board discussion commenting on the volume of partnerships with the Health & Nutrition Center. Discussion of Animal Control departmental transition with agreement voiced. Pleasure also voiced with plans of new OR department and expected qualifications and responsibilities of the OR Director.

• Grants

o Preparedness Activity Exercises every 5 years. Contracted with Roland Hamrick as consultant $18,000 through Homeland Security Program (HSGP) received by Myron Waddell

with Surry County Emergency Services to cover exercises expenses for Health Department’s participation.

Motion by Commissioner Golding to approve Preparedness Activity and HSGP monies through Surry County Emergency Services as presented. Second by Dr. Scott Baird. Motion approved. • Media Distribution

o Previously received all as they came in. Now, only receive B Grade and below. o Links to all establishments are located on SCHNC website.

Discussion with comments of preference to new system making more sense.
