MatrixMANDIBLE preformed reconstruction plates are pres-haped to the anatomy of the mandible. Their shape was obtained by statistical analysis of data derived from more than 2000 CT scans, originating from various adult popula-tions*. Plates are available in three sizes: small, medium and large.

The combination of anatomic design and manufacturing pro-cesses allows reduction of stress in the plate compared to classical intraoperative bending of a flat plate. The minimal intraoperative bending that may be required preserves the optimal threaded hole shape, with no deformation of holes in the preformed sections. These features result in a plate which has an increased fatigue life compared to flat reconst-ruction plates, reducing the risk of premature plate failure.

Moreover, the preformed shape of the plates allows for tran-soral application, in combination with the transbuccal instru-mentation and/or 90° screwdriver.

In addition, the preshaped form of the MatrixMANDIBLE Preformed Reconstruction Plates can reduce time in the OR by reducing the number of bends required to completely contour the plate when compared to a flat reconstruction plate.

AO Principles

In 1958, the AO formulated four basic principles, which have become the guidelines for internal fixation1. They are:

Anatomic reductionFracture reduction and fixation to restore anatomical rela-tionships. A comprehensive implant and instrument selection offers the ability to address most simple and complex fixa-tion needs.

Stable fixationStability by fixation or splintage, as the modality of the frac-ture and the injury requires. The MatrixMANDIBLE Plating System conical locking plate and locking screw technology are designed to achieve stable bone fixation.

Preservation of blood supplyPreservation of the blood supply to soft tissue and bone by careful handling. Instrumentation and smooth profiles and edges on implants minimize disruption of soft tissue and pre-serve vascular blood flow for bone healing.

Early, active mobilizationEarly, active mobilization of the part and patient. The MatrixMANDIBLE Plating System implants, combined with AO technique, provide stable fixation to allow a functional aftercare.

The DePuy Synthes MatrixMANDIBLE Preformed Reconstruc-tion Plates are specifically preformed plates intended for mandibular reconstruction with bone graft (vascularized or not), temporary bridging until secondary reconstruction, the treatment of the comminuted fractures of the mandible and the treatment of fractures in edentulous and atrophic mandi-bles, and unstable and/or infected mandibular fractures.

– These devices can break during use (when subjected to excessive forces or outside the recommended surgical technique). While the surgeon must make the final deci-sion on removal of the broken part based on associated risk in doing so, we recommend that whenever possible and practical for the patient, the broken part should be removed.

– Instruments, screws, and cut plates may have sharp edges or moving joints that may pinch or tear the user’s glove or skin.

General Adverse Events

As with all major surgical procedures, risks, side effects and adverse events can occur. While many possible reactions may occur, some of the most common include:Problems resulting from anesthesia and patient positioning (e.g. nausea, vomiting, neurological impairments, etc.), thrombosis, embolism, infection or injury of other critical structures including blood vessels, excessive bleeding, dam- age to soft tissues incl. swelling, abnormal scar formation, functional impairment of the musculoskeletal system, pain, discomfort or abnormal sensation due to the presence of the device, allergy or hyperreactions, side effects associated with hardware prominence, loosening, bending, or breakage of the device, mal-union, non-union or delayed union whichmay lead to breakage of the implant, reoperation.

Device specific adverse events include but are not limited to: – Loosening, bending, or breakage of the device – Non-union, mal-union or delayed union which may lead

to breakage of the implant – Pain, discomfort or abnormal sensation due to the pres-

ence of the device – – Infection, nerve and/or tooth root damage and pain – Soft tissue irritation, laceration or migration of the device

through the skin – Allergic reactions from material incompatibility – Glove tear or user puncture – Graft failure – Restricted or impaired bone growth – Possible transmission of bloodborne pathogens to the

user – Injury of patient – Soft tissue thermal damage – Bone necrosis – Parasthesia – Loss of tooth

MRI Information

Magnetic Resonance Environment Torque, Displacement and Image Artifacts according to ASTM F2213-06, ASTM F2052-06e1 and ASTM F2119-07Non-clinical testing of worst case scenario in a 3 T MRI system did not reveal any relevant torque or displacement of the construct for an experimentally measured local spatial gradient of the magnetic field of 5.4 T/m. The largest image artifact extended approximately 31 mm from the construct when scanned using the Gradient Echo (GE). Testing was conducted on a 3 T MRI system.

Radio-Frequency-(RF-)induced heating according to ASTM F2182-11aNon-clinical electromagnetic and thermal simulations of worst case scenario lead to temperature rises of 13.7 °C (1.5 T) and 6.5 °C (3 T) under MRI Conditions using RF Coils (whole body averaged specific absorption rate (SAR) of 2 W/kg for 15 minutes).

Precautions: The above mentioned test relies on non-clini-cal testing. The actual temperature rise in the patient will depend on a variety of factors beyond the SAR and time of RF application. Thus, it is recommended to pay particular attention to the following points: – It is recommended to thoroughly monitor patients under-

going MR scanning for perceived temperature and/or pain sensations.

– Patients with impaired thermo regulation or temperature sensation should be excluded from MR scanning proce-dures.

– Generally it is recommended to use an MRI system with low field strength in the presence of conductive implants. The employed specific absorption rate (SAR) should be reduced as far as possible.

– Using the ventilation system may further contribute to reduce temperature increase in the body.

After completing the preoperative plan, expose the fracture or osteotomy site. For trauma, reduce the fragments anatomically.

2Determine the size of the preformed plate

Place the sizer marked with “MEDIUM” on the mandible as though placing the plate, beginning at the chin region (men-tal foramen) and continuing through the body and the ramus. The sizer should be in contact with the mandible on both chin and body regions.

Note: Do not bend sizers.

One of three conditions may occur with respect to the relation between the ramus of the mandible and the cor-responding region of the sizer. Use the following guideline for proper implant selection.

If the posterior border of the vertical mandibular ramus and the corresponding region of the sizer are correctly aligned (Figure 1), use the medium plate and bending template.

If the vertical ramus portion of the sizer is posterior to the posterior border of the mandible (Figure 2), use the small plate and bending template. Use the “SMALL” sizer to con-firm the size.

If the posterior border of the ramus is posterior to the corresponding region of the sizer (Figure 3), use the LARGE plate and bending template. A precise fit can be reconfirmed with the LARGE sizer if deemed necessary.

The length of the plate can be slightly increased or de-creased by widening or narrowing the curvature in the chin region (Figure 4). The first 2–4 holes following the non-adaptable section of the plate will be most effective to adjust the length with minimal bending. See section 5 for bending instructions.

Note: If none of the plates fit and if adaption of the MatrixMANDIBLE Preformed Reconstruction Plate is not suit-able for the patient, then a MatrixMANDIBLE flat plate of 2.0 mm, 2.5 mm or 2.8 mm thickness, depending on the indication, should be used.

3Select, preform and form the bending template

Instruments

Select the bending template equivalent to the plate size determined in step 2.

Preform the bending template inside the preformed plate. Align the specific delimited regions (ramus, body) with the corresponding areas of the plate and press it against the in-ner surface of the plate. Press along the length of the plate, starting from the ramus.

Form the bending template by placing it on the mandible as when placing the plate, and press it against the bone surface.

Bending templates can be adjusted to the desired length if necessary, by cutting the unwanted simulated hole sections with a cutter.

Precautions: – Stable fixation requires a minimum of 3–4 screws in

both proximal (posterior) and distal (anterior) segments, depending on indication.

– When using the MatrixMANDIBLE Preformed Reconstruc-tion Plates as a temporary bridging device with 2.4 or 2.9 mm locking screws, allow for four screws per seg-ment. If limited bone length or poor bone quality exists, a minimum of three 2.9 mm locking screws should be used.

4Cut the plate

Plate holes can be cut using two short cut plate cutters.To avoid soft tissue damage, deburr the cut plate whenappropriate, using the manual deburring feature on thehandle of the cutter.

If necessary, cut the last holes of the posterior vertical por-tion of the plate to match the length of the ramus.

Reposition the plate on the mandible to determine the number of holes to be cut from the chin region, if any.

5Contour the plate

Use the bending pliers with nose to perform in-plane and out-of-plane bending. For torsional bends use the bending irons. The bending irons can also be used to perform in-plane and out-of-plane bending.

If in-plane bending is required, complete the bend using the “IN-PLANE BENDING” feature of the bending pliers with nose, or the bending irons.

To make out-of-plane bends in the ramus portion of the plate, starting with the hole adjacent to the non-bendable plate sec-tion. Bend each hole section in consecutive order, as necessary.

Bend the chin area, starting at the hole adjacent to the non-bendable plate section. Bend each hole in consecutive order, as necessary. Flip the plate if the desired region of the plate does not fit in the “OUT-OF-PLANE BEND” feature of the instrument.

Precautions: – If the Condylar Head Add-On System will be used, the

last three holes in the region of the ramus should not be bent or restricted.

– Avoid reverse bends, repetitive bends, and sharp bends (sharp bends include a single out-of-plane bend between two adjacent holes of >30º). Reverse, repetitive, or sharp bends may weaken the plate and lead to premature plate failure.

Note: When using the MatrixMANDIBLE System Combina-tion Bender, follow the steps as shown on the instrument. The “LAST HOLE BEND” feature should only be used for bending the last hole of the plate.

Bend the chin area, starting at the hole adjacent to the non- bendable plate section. Bend each hole section in consecu-tive order, as necessary. Flip the plate if the desired region of the plate does not fit in the “OUT-OF-PLANE BEND” feature of the instrument.

Precaution: For extensive bending, bending screws may be used. Extensive bending includes bends that exceed 20 de-grees in torsion and “in-plane” bending, and 30 degrees for “out-of-plane” bending.

Notes: – Bending screws are made from biocompatible implant

grade material (Ti-6Al-7Nb). If bending insert becomes stuck in plate, the plate may be implanted with the insert.

– For out-of plane bends >30 ,̊ perform bend across multi-ple hole segments. This can be achieved by first using the “OUT-OF-PLANE BEND” features of the Combination Bender, then bending to the desired angle.

Precautions: – When bending out-of-plane in a single point (using the

“LAST HOLE BEND” feature of the bending pliers with nose or the bending irons) bend in a controlled manner. Do the bend in small increments. Do not excessively bend outwards in a single point, or plate breakage may occur. Distribute sharp bending over several holes whenever possible.

– Do not use threaded drill guides as benders.

6Position the plate

Place the plate over the planned resection or fracture zone.

7Select the screws size

Determine the appropriate locking screw diameter (2.4 mm or 2.9 mm).

Precautions: – 2.0 mm diameter screws should only be used with the

MatrixMANDIBLE Preformed Reconstruction Plates if inserted into a bone graft, or if bone volume does not permit the placement of a larger screw.

– In accordance with AO technique, it is important not to insert screws into infected bone.

8Drill the first hole

Three drill guide options are available to meet surgeon preference: short threaded drill sleeve, double drill guide or transbuccal instruments.

Regardless of the option chosen, drill hole next to the frac-ture or osteotomy site in the proximal (posterior) segment (1). Then drill the hole closest to the fracture or osteotomy site in the distal (anterior) segment (2).

Precautions: – Drill speed rate should never exceed 1,800 rpm, parti-

cularly in dense, hard bone. Higher drill speed rates can result in: – thermal necrosis of the bone, – soft tissue burns, – an oversized hole, which can lead to reduced pullout

force, increased ease of the screws stripping in bone, suboptimal fixation, and/or the need for emergency screws.

– Avoid damaging the plate threads with the drill. – Always irrigate during drilling to avoid thermal damage to

the bone. – Irrigate and apply suction for removal of debris potentially

generated during implantation or removal. – Take care while drilling as to not damage, entrap, or tear a

patient’s soft tissue or damage critical structures. Be sure to keep drill clear of loose surgical materials.

– Handle devices with care and dispose worn bone cutting instruments in a sharps container.

After creating a stab incision pass the cannula with obturator carefully through the soft tissue, then remove the obturator. Pass the drill sleeve through the cannula and snap it in place. Position the tip of the cannula on plate, at the hole intended for the screw. If the drill sleeve with thread is used, rotate the drill sleeve clockwise to engage the threads in the plate. With the correct diameter calibrated drill bit, drill directly through the drill sleeve. When using the threaded drill sleeve, the depth of drilling can be determined by observing where the colored rings on the drills match the fixed surface on the drill sleeve, and correlating these to the chart in the trans-buccal module.

Precaution: To achieve optimal angular stability with locking screws, the hole must be drilled coaxially with the plate hole, or at a right angle to the plate.

Three different cheek retractor options are available for use with the cannula and obturator.

Insert the proper length screw through the plate into the first drilled hole. Tighten the screw until secured.

Precaution: For maximum stability, locking screws are recommended. Use nonlocking screws if a bone fragment has to be repositioned by pulling it against the plate, or if additional screw angulation is needed.

Notes: – If using the cannula 2.0 (as described in step 8, option 3),

remove the drill sleeve, then insert the self-holding screwdriver with the screw engaged on the blade.

– To engage the screw on the blade, align the blade over the cruciform recess and slowly rotate it counterclock-wise until the blade drops into the recess. Firmly press the blade to fully seat it into the screw. A half counter-clockwise rotation of the engaged screwdriver facilitates the screw removal from the screw holding clip.

11Drill and place the remaining screws

Drill the hole closest to the fracture or osteotomy site in the distal (anterior) segment after the first screw was placed.

Insert the second screw following the previously described procedure.

Insert all remaining screws alternating to each side of the mandible. Securely tighten all screws unless resection is to follow. Apply additional fixation as desired.

2Replace the implants

Place the plate back onto the mandible in its original position. Reinsert each predetermined screw. Check all screws to ensure a secure fit in the plate.

3Apply bone graft2

A vascularized or non-vascularized bone graft may be applied for a primary reconstruction.

The MatrixMANDIBLE Preformed Reconstruction Plate may bridge continuity defects without bone grafting temporarily, prior to a secondary reconstruction.*

* Plate fracture is possible when any plate bears the entire functional load for extended periods. Therefore, the implantation of a bone graft immediately, or at a later date, is necessary to support the construct.
