What is supported living? Supported living describes a range of services to help people live in their own home. It also describes services that help people who are homeless find suitable accommodation to meet their needs. These services are often funded by the Council from a number of different budgets. These budgets can include Supporting People Grant, Social Care budgets and NHS budgets.
What type of support is provided? The type of support often provided varies between services. The support offered often includes:
help to manage your tenancy e.g. explaining your rights and responsibilities as a tenant, filling out forms, talking to your landlord, reporting repairs
help to access suitable accommodation to meet your needs help to develop your independent living skills such as cooking, cleaning
as well as personal care help to manage additional support needs such as learning disabilities,
mental health problems, problems managing your alcohol intake. Someone to check on you regularly to make sure you are ok Signposting to other specialist services e.g. health services,
employment services These are just some examples of what supported living services help people to do. The supported offered is based on what people need help to do. The philosophy of services is to support people to be as independent as possible so if you do not need help in a particular area then you are empowered to manage this yourself.

How is support provided? There are different ways in which support is provided. The main ways are listed below:
Community alarms are a piece of technology linked your property. If you need help you can trigger the alarms and someone will call you to find out what help you need.
Sheltered accommodation is a collection of properties in a scheme with a scheme manager. The scheme manager checks on your wellbeing each day and helps you contact other services if you need more help. There is often a community room attached to the service with social activities on. These services also have community alarms in each property.
Extra care services are similar to sheltered accommodation as they are a collection of properties in a scheme with a scheme manager. Extra care services however provide more services as they provide care services if you need them such as help to cook, clean and wash yourself. These services also have a community alarms in each property.
Floating support is where someone comes to your home and supports you. When you no longer need support they stop supporting you and support someone else.
Accommodation based services are where the support is linked to your accommodation. You must need to receive support to live in that accommodation.
Supported Lodgings are where you live with a family, couple or individual as part of their family.
Do I have to pay for the support? There is a charge for most services. How much you pay depends on your financial circumstances. This is determined by a financial assessment completed by the Council. If you cannot afford to pay for your service then the Council may pay this for you. This is decided by the financial assessment. The provider will help to arrange a financial assessment if you need one. If you receive housing benefit then the Council may pay for services on your behalf. This does not apply to all services so make sure you speak to the service about costs when you talk to them.

Description:
Service users are usually visited in their own homes by pre arranged appointments although other venues can be used by mutual agreement. The support from the scheme is available 9 - 5 Monday to Friday, excluding bank holidays and weekends. Support is flexible and based upon a step up step down approach depending upon the service users needs and if therefore likely to vary from one person to another. On average the service provides 1 hour of support per household per week. The service also provides community alarms which linked to a call centre 24 hours a day, 7 days a week. When the alarm is triggered the call centre calls the service user to establish if a response is needed. The response used is dependent on the service user’s needs assessment and support plan. Every week day (Monday – Friday) each service receives a courtesy call from the centre to check that the system is working and they the service user does not require any support.

Description:

Eligibility Criteria: Young, single, homeless individuals aged 16 -17, who are deemed too vulnerable for independent living, and who require support in gaining life skills necessary for independent living.
Exclusions:
MAP cannot accept pregnant young women, individuals with chaotic drug / alcohol misuse problems, those with a history of violent behaviour or persistent offending behaviour due to potential risk factors; however every referral is assessed on an individual basis. MAP welcomes any agency risk assessment or risk management plans that would contribute to effective risk assessment to avoid excluding individuals.

Duration: Very short stay, typically a few weeks
Description:
The service provides a suitably trained mediator to mediate between the young person and the family. The mediators act as ‘facilitators’ to listen and assist young people and their family to engage in constructive discussion with a view to finding a mutually acceptable outcome. Staff will talk through the young person’s issues and alternatives to look at what can be done to resolve the situation with the parent/family. If returning to the home is not an option for the young person staff will discuss other housing alternatives with the young person. This will include looking at what they need to do in order to apply to supported accommodation. Staff also provides information on benefits and training organisations to young people at these times.
