SUMMER MIX...Do you get bored during the summer holidays? Summer Mix 2013 offers over 100 courses (many of which are new this year!) and has more than 3,000 places available for young people to sign up to.

Summer Mix 2013 is: � open to young people living in Plymouth (your home address must

receive services from Plymouth City Council) � completely free! � for young people aged 11 to 19 years - some courses are available for

those aged up to 25 years if disabled or have an additional need � allocated on a !rst come, !rst served basis � available to sign up to online at www.summermix.co.uk � organised by Plymouth City Council Youth Services and funded in

partnership with local organisations

Part two of enrolment process: registering onlineFrom 9am on Monday 1 July 2013, young people should register for Summer Mix online. This year ALL registrations must be completed online – you can access computers at libraries or ask at your local youth centre.

Go to www.summermix.co.uk and follow the instructions to register.

Please note: � Courses cannot be chosen until the online registration has been

completed � Places will not be con!rmed until a signed consent form has been

received by the Summer Mix team and matched to an online registration.

Terms and conditions of enrolmentPlease read carefully the following terms and conditions of enrolment. By enrolling on the Summer Mix activity you acknowledge and understand that:

� Some courses may involve groups going off-site with the tutor for speci!c activities. � In the event of an emergency medical treatment including anaesthetic may be authorised on

your behalf by the Summer Mix staff and their representatives. � Personal information of those applying for courses will be recorded on an electronic database

and will be accessible to the organisations hosting the activity and to Summer Mix. � Any form of abuse or threatening behaviour by you may result in you being excluded from an

activity or trip and being sent home, the cost of which will be billed to you. I agree with the terms and conditions above.
