Welcome to the Otsego Public Schools’ Summer B.A.S.E Program!

This booklet provides information needed to make a well informed decision regarding quality summer child care. Any further questions can be answered through the Program Director Lacey Stout and the Community Education Office at 692-6225.

PROGRAM PURPOSE1. Enhance children’s physical, intellectual, social and emotional development. 2. Promote the school as a safe, secure, and stimulating environment.3. Provide child care at a reasonable cost in a convenient location.

FINANCIAL POLICIES The Otsego Community Education Program is operated on the philosophy that programs offered must be financially self- sufficient. This means that fees generated by programs must offset costs such as instruction, material and supplies. Should less than 30 full time participants register for this program by April 30th, the program may be cancelled. Enrollment will be limited to the first 50 participants.

LOCATIONThe Summer B.A.S.E. Program will be held at Dix Street Elementary.

Summer B.A.S.E. students will be divided into age groups @ 8:30 am each morning. Lower elementary students will be located in the BASE room and upper elementary BASE students in the gym. This is to provide more age appropriate activities for the students.

Drop off and pick up will still be located at the Dix Street BASE Room. Any questions can be answered through Program Director Lacey Stout or the Community Ed. office @ 692-6225.

SCHEDULE OF OPERATIONThe program runs Monday - Friday, 6:30 a.m. - 6:00 p.m. Summer B.A.S.E will be closed on days of severe weather, in accordance with the current elementary handbook, and July 4. Parents will be notified if there is any deviation from the proposed schedule.

STAFFThe Summer B.A.S.E. Director, Lacey Stout, is an educated and experienced individual in the area of child care. The assistants are responsible, competent adults. They are well trained and demonstrate personal characteristics for successfully working with children.

ENROLLMENT POLICIESAny students going into grades K-5, are eligible to attend Summer B.A.S.E. At the time of registration, a parent is required to commit his/her child to a regular weekly schedule. A parent may enroll his/her child at any time during the summer provided there are openings in the program. Early registration will help to ensure a place in the Summer B.A.S.E. Program. The following forms must be completed:

Child Information Card Health and Immunization Statement Payment of Registration Fee Payment Options Card Registration Form

All necessary forms are available at each B.A.S.E. site and the Community Education Office. Parents must keep the Director informed of any changes to their enrollment information.

FEESThe Summer B.A.S.E. Program’s payment options are as follows: A) One payment of $900 B) Two payments of $460 C) Three payments of $315 D) Four payments of $240 E) Weekly rate of $125 F) Daily rate of $30.

There is a registration fee of $30 per child with a family maximum of $60.Your registration fee and forms and all signature sheets must be turned in before your child starts Summer Base. If scheduled payments are not paid in full within one week of date indicated by your payment option, you will be automatically defaulted to payment option F.

PAYMENT PROCEDUREStatements are distributed according to individual due dates for each pay-ment option. A $5.00 late fee will be added to the account for each week that the payment is late if payment is not made per payment plan.

Payments by check and/or money order can be left at the Summer B.A.S.E site or mailed to: OTSEGO COMMUNITY EDUCATION 501 S. Grant St. Otsego, MI 49078

Please make all checks and/or money orders payable to Otsego Public Schools. Payment in the form of cash can be made ONLY at the Com-munity Education Office.

LATE PICK-UP FEEA late fee will be charged for picking up your child after 6 p.m. The extra charge will be $5.00 for each 10 minute period. The staff will contact the emergency persons designated on the Child Information card after 10 minutes.

TAX CREDITThe Summer B.A.S.E. Program qualifies for the Child Care Tax Credit.

CLOTHING & PERSONAL POSSESSIONSChildren should dress for indoor and outdoor play. T-shirts will be provided for field trips. Toys may be brought to the site at the parents discretion. Summer B.A.S.E. is not responsible for lost or broken items. Toy weapons and skateboards are not allowed in BASE. Rollerblades and bikes are allowed on designated days only.

SIGN IN/SIGN OUT POLICY Children must be signed in and out by a parent or authorized adult upon arrival and departure.

FOODThe regular fee includes two snacks daily, breakfast and mid-afternoon. Children are welcome to bring in snacks for birthdays, or other special occasions. Lunch must be provided by parents.

HEALTH/ACCIDENT POLICIESEach parent is required to sign a health statement assuming the responsibility for his/her child’s state of health while participating in the B.A.S.E. Program. Staff needs to be informed of any allergies or medical problems.Hands of both the staff and the children will be washed prior to handling food and before eating.

Precautions are used when handling children’s bodily fluids. Latex gloves and sanitizing will be done when handling blood, including blood containing body fluids and tissue discharges. Any personal items will be placed in a plastic bag and returned to the parents. If a child becomes injured while at the B.A.S.E. site, the child will be given appropriate fiirst aid, an accident report will be completted, and a parent will be notified. Personal insurance must be provided to cover injuries.

All toys, equipment, and surfaces are cleaned once every three months. Tabletops are cleaned daily, and cots are cleaned after each use, unused cots are cleaned every three months.

Parents need to use discretion when your child isn’t feeling well and needs to be kept home due to: fever, diarrhea, vomiting, lice etc. You will be called to pick up your child if they are running a fever of 100 degrees or more, If they are vomiting, if they are in pain, or if they cannot rest comfortably. Accommodations will be made for the child to rest comfortable and be supervised while waiting for a parent or designated person to arrive. The staff may give medication only after the proper forms are filled out.

RULES AND REGULATIONSStudents enrolled in the Summer B.A.S.E Program are expected to adhere to the rules and regulations stated in the current elementary handbook. Failure to comply with these rules will lead to disciplinary action.

If a student breaks a rule, the following consequences will apply: 1st consequence: Verbal Warning2nd consequence: Time out from special activities3rd consequence: Loss of entire activity, disciplinary note is sent home, a meeting is setup with parent/guardian Three disciplinary notes may result in dismissal from the program.

Lunch Agreement

I agree to provide a lunch daily for my child. Lunches provided shall be wholesome in quality. Please no glass bottles or containers. Milk will be available for lunch upon request. The Otsego Summer B.A.S.E. Program will provide breakfast and mid-morning snack.
