Stipend Program Reminder..

• The stipend program will NOT be funded

• Health Dept. employees only (phone interview)

Renew by March 31st of every year it is NOT the calendar year (Please note: The phrase “I didn’t know my card expired” after being certified for 10 years will no longer be acceptable.)

(Trainers – PLEASE explain this to students who are training from January to March)

Renewing Your Renewing Your CertificationCertification

$25.00 late fee (per discipline) If you need a refresher you have a 6

month grace period (after March 31st) however you CANNOT perform any lead activities from March 31st until you complete the training and State exam.

Expired for more than a year (when you need a refresher) than you will need the initial training.

Last Minute or Incomplete Applications (Initial or

Renewing)Incomplete Applications – will be notified at the exam or by phone ONCE. You will not be certified until we receive everything required. (SSN are required to process application)

Last Minute Applications – Can’t guarantee there is or will be room in an exam. If the exam is full you WILL BE scheduled for the next AVAILABLE exam.

Lead Contractor Renewals

Completed application – including your Federal ID number

A check for the appropriate fee’s

Expiration – December 31

Online Contractor Renewals

Payment by credit cardStill need a CURRENT lead

certified supervisor (We do check)Mail a copy of your current

workers comp. insurance (unless you are a sole proprietor.)

No longer renew online once it is lapsed

Contractor CertificationYou MUST have one lead certified

supervisor working for the company AT ALL TIMES (You CANNOT sub the work out)

Workers comp insurance – unless you are a sole proprietor

Trainer Info…..Scheduling exams – Students

are not scheduled for an exam until we RECEIVE an application.

Please make sure students know what they need to send in with their applications. (Copies of certificate(s) & work history)

Remember…..I am also a compliance officer so keep in mind if you fail to renew your certification I will see you in the field and know that you are not current…...

Lapsed certification + working = citation (It’s cheaper to pay your renewal fee’s )

And………If you fail to pay your citation I am also a collections officer……..I will never go away!
