You can even have your Annual Wellness Visit through telehealth.

All you need is a smartphone, tablet or computer with a camera, you can call our offices and we will get you set up to be seen for a wide variety of newly developed conditions, help managing your medications and follow up on previous visits to manage any chronic health challenges you may be facing.

Telehealth Steps are as Easy as 1-2-3

1 Call your doctor’s office first to schedule appointment and get set up. You will find a list of the locations and

office numbers on the back of this document.

2 Prior to your appointment, we'll send you a link to connect you with your doctor for your telehealth visit.

3 Click the link we send you at your appointed time,and you'll be ready to go.

Make Your Telehealth Visit a Success● Conduct the visit in a location with a high-speed

internet connection● Check your Settings to ensure you are on Wi-Fi

and not using your cellular network device● Select a room that is private and free of

distraction and noise● Gather all your medications and pharmacy

information to have them handy● Complete any online paperwork in advance

of your visit● Weigh yourself and write down your weight● If you have an at-home blood pressure monitor,

take your blood pressure and write it down● Write down any questions you may have

for the doctor● Set up your device so the camera is at eye level● Test your microphone and speaker● Check in by logging on 10 minutes before

your scheduled visit● Enjoy this easy and convenient visit

with your doctor!

FollowMyHealth is PriMED's secure patient portal. Using the FollowMyHealth app makes telehealth visits even easier. Plus E-mail Visits (E-Visits) enables you to send your physician a secure message and receive medical advice, without visiting the office. Ask your PriMED office about registering for FollowMyHealth.
