Documents Required for Enrollment

1.□ Original Birth Certificates of Every Child Identified in the Family

2.□ Child’s Immunization Record 3.□ Child’s Recent Physical Report must be signed & completed within the last 12 months

4.□ Family’s Proof of Income (Last four Pay stubs received; Wages, Cash Aid, Unemployment Benefits,

Annual Income tax returns for self-employment only)

5.□ Employment Verification (Form must be completed and signed by employer)

6.□ Proof of Address (Utility bills - must have parent’s name on the document)

7.□ Volunteer’s Negative Tuberculosis Test for every adult volunteering in the classroom

8.□ Volunteer’s Immunization Record- adult must be immunized for Pertussis, Measles & Influenza

9.□ Volunteer’s Fingerprint Clearance only if an adult other than a parent is volunteering in the classroom

Eligibility Waiting List: families applying for State Preschool services must be eligible as low-income. Admission priority is given based on family size and income level. When classes become full, California Department of Education (CDE) Child Development Division mandates the use of an Eligibility Waiting List to fill vacancies. The Eligibility Waiting List ranks families according to CDE income guidelines. Waiting List Applications are available at the Enrollment Center, 925 South A St.

Eligibility for State Preschool

Child must be 4 years old on or before December 1, 2017 Family must live within the Oxnard School District attendance boundaries Family must be eligible based on family size and low-income according to State Child Development Division income guidelines Parents volunteer in the classroom six hours each month, and must have a Negative Tuberculosis skin test & Vaccines Other adult participants in the classroom must have a Negative Tuberculosis skin test, Vaccines & Fingerprint Clearance Parents must provide their own transportation to and from school

El niño debe cumplir 4 años en ó antes del 1 de diciembre del 2017 La familia debe vivir dentro del área de asistencia del Distrito Escolar de Oxnard La familia debe ser elegible de acuerdo al tamaño de familia y bajos ingresos de acuerdo a las guías de División de Desarrollo

Infantil del Estado Padres son voluntarios en el salón de clase 6 horas al mes y deben presentar prueba Negativa de Tuberculosis y Vacunas Otros adultos voluntarios deben tener prueba Negativa de Tuberculosis, Vacunas y Huellas Digitales Libradas Padres proveen su propia transportación

Lista de Espera de Elegibilidad: familias que aplican para servicios Preescolares deben ser elegible de bajos ingresos. Prioridad de admisión es basada en el ingreso y tamaño de la familia. Cuando se llenen las clases, el Depto. de Educación de California División de Desarrollo Infantil requiere el uso de una Lista de Elegibilidad (lista de espera). La Lista de Elegibilidad enumera a las familias de acuerdo a los ingresos más bajos. Aplicaciones están disponibles en el Centro de Matriculación, 925 South A St.

Documentos Requeridos Para Matriculación

1.□ Acta de Nacimiento Original de Todos los Niños en la Familia

2.□ Cartilla de Vacunas del niño que va a registrar 3.□ Examen Físico Reciente/Reporte Médico debe ser firmado y completado dentro de los últimos 12 meses

4.□ Prueba de Ingresos Familiares (Últimos cuatro Talones de cheque recibidos; Salario, Asistencia

Monetaria, Beneficios de Desempleo, Declaración Anual de Impuestos-sólo para trabajo por cuenta propia) 5.□ Verificación de Empleo (Forma debe estar completa y firmada por empleador)

6.□ Prueba de Domicilio (Recibo de utilidades - debe tener nombre de los padres)

7.□ Prueba Negativa de Tuberculosis del Voluntario para cada adulto voluntario
