Revised National TB Control Programme

Dear Colleagues,

We are pleased to inform you that during this quarter five state representatives have participated in National Strategic Planning workshop held at New Delhi, National level Common Review Mission team has invited to state for review of overall health programme in Gujarat, work of RNTCP was appreciated by the team, International workshop for BRICS countries was carried out by Gujarat team for India. TB-HIV patient now receiving daily FDC Anti-TB drugs with ICT monitoring mechanism (99 DOTS) from ART centres along with ART under single window services. BDQ-CAP, all 58 doses received at Ahmedabad DR-TB Site are provided to eligible DR-TB patients.

RNTCP performance during 4th quarter 2016: During the 4th quarter 1,86,987 presumptive TB patients were examined, 15,866 sputum positive TB patients were diagnosed, and a total of 21,931 all forms of TB patients were registered for treatment. The annualized total case notification rate is 134 cases per 100,000 population. With a total of 10,035 New Smear Positive patients being registered for treatment; the new smear positive TB case Notification Rate (annualized) is 61 per 100,000 population. In addition to this, 2,463 New Smear Negative patients, 2,817 New Extra Pulmonary patients registered for treatment in this quarter. The treatment success rate amongst the New Smear Positive pulmonary TB cases registered in the 4th Quarter 2015 is 89% and the Sputum Conversion rate of patients registered during 3rd Quarter 2016 is 92%. In TB-HIV intensified services 98% patients were tested for HIV out of 21,931 total patients registered during 4th quarter 2016. PMDT: During this quarter, 8393 MDR-TB suspects were tested for drug susceptibility. Total 525 patients were diagnosed as having drug resistant and eligible for second line treatment. Total 493 patients were put on Category-IV regimen in this quarter. Additionally, 34 patients were put on Category-V regimen. Training: State has completed ToG training for all key RNTCP staff including District Officials, 1 ToT for District TB Officer and 20 Batch for key RNTCP staff has been carried out by State. Zonal Task Force Workshop carried out in the state in which more than 70 participants from 5 state has been participated from different medical college and state office.

Glimpse of revised Technical & Operational Guideline (TOG ‘2016) 

Revised Technical and Operational guideline (TOG) is launched with vision of – Zero death, disease and suffering due 

to TB. To meet the goal of end TB strategy and SDG, reduction in TB death up to 95%, decrease TB incidence up to 

90% and sustain zero catastrophic cost by 2035 compared to 2015 status.   Unique feature of revised TOG is briefly 

narrated here.  

• RNTCP has already introduced daily regimen for all TB HIV co‐infected cases and by Jun’17 it will be provided 

to all TB cases. 

• Previous  algorithm  is  replaced  by more  flexible  and  highly  sensitive  algorithm  which  prevents  delay  in 

diagnosis by providing all possible means of investigation available  

• To reduce pill burden and all drugs delivery to the patient, formulation of daily regimen is now Fixed Doses 

Combination  (FDC), which will be provided  in 2 drugs  (HR), 3 drugs  (HRE) and 4 drugs  (HRZE) preparation 

with different weight band  in adult and paediatric  . Additionally, flavoured and dispersible formulation will 

be available under paediatric drug regimen.  

• Daily regimen includes three drugs (HRE) during Continuation Phase (CP) for both CAT I and CAT II patients 

so there is no need of Intensive Phase (IP) extension in first line treatment. 

• Certain Definition and Terminology is revised while few new terms introduce like  

 

• Currently available 24‐27 months  treatment  for DR TB cases will be cut short  to 9  to 12 months  regimen 

once the approval and drugs will be made available. 

• Currently available  standardized  regimen  for DR TB  cases  is going  to be  replaced by Drug Sensitivity Test 

(DST) guided treatment regimen where patient will be offered drug  regimen based on more  than six  first 

and second line drugs DST. Treatment for Mono/Poly resistant cases is introduced. 

• Additional  treatment monitoring  options  (Family  DOTs,  ICT  enabled)  are  introduces  to  strengthen  and 

supplement existing DOT mechanism.  

• To ensure relapse  free treatment and early detection of recurrent cases  long term follow up at six month 

interval till 2 year after completion of treatment is introduce. 

• Revised  surveillance system proposed notification  (registration with Nikshay) at  the  time of diagnosis at 

PHI  level  which  is  also  responsible  to  report  fate  of  these  cases  while  treatment  outcome  will  the 

responsibility of treating health facility.  

• All private health facilities need to be registered  in Nikshay and ensure TB notification of all cases. Extend 

diagnostic and treatment services to private sector.  

• Active case finding especially in key population is one of the essential cases finding strategy 

• Robust ADR management and reporting with PvPI is integrated part
