HbA1c in der Primärdiagnostik
• Ende 2010: neue Leitlinie der DDG
• Diagnostik des Diabetes mellitus bisher nur durch oGTT
1. Ein HbA1c unter 5,7% (39 mmol/mol Hb) schließt einen Diabetes mellitus aus.
2. Ein HbA1c-Wert über 6,5% (48 mmol/mol Hb) beweist ihn.
3. Patienten mit HbA1c 5,7% - 6,4%: Nüchternglukose oder oGTT
4. Patienten mit Zuständen, die zu einer Verfälschung des HbA1c führen könnten (Hämoglobinvarianten, Verkürzung der Erythrozytenlebensdauer…): Nüchternglukose oder oGTT

• Sauerstofftransportierendes Molekül: 95% des Erythrozyten
Bestehend aus 4 Untereinheiten:2 α und 2 ß Ketten = Hb A2 α und 2 δ Ketten = Hb A22 α und 2 γ Ketten = Hb F = Fetales

HbA1c und Hämoglobinopathien
Einflüsse auf die HbA1c-Bestimmung:
• HbA1c = Bindung der Glukose an die ß-Kette des Hämoglobins (HbA)
• HbA1c abhängig von der Erythrozytenlebensdauer(Normbereich)
• Hämoglobinvarianten: auch diese werden glykiert (z.B. HbS1c), sind aber vom HbA1c zu unterscheiden
• Hämoglobinvarianten: Änderung der Erythrozyten-lebensdauer (120 � 30 Tage)
• Homozygote Varianten (z.B. HbSS): HbA wird nicht gebildet � kein HbA1c vorhanden

The clinical requirements of a HbA1c test include:
1. Low individual variance
2. Precision that justifies clinicians acting on differences of0.35%-0.5% as being significant;
3. HbA1c assays with coefficient of variation (CV) <2% for combined, between- and within-run precision.

Schlussfolgerungen
• Nur standardisierte Methoden sind für die Bestimmung desHbA1c zulässig (CV < 2.0 %).
• Eine Bestimmung von HbA1c mittels HPLC(Referenzmethode) ist wann immer möglich anzustreben(CV <1, geringer Bias)
• Die Abweichung des HbA1c eines Labor im Ringversuchkann bis zu 10% betragen.
