SpeedFlex... The Evolution of Human Performance

The SpeedFlex Training System is designed to enhance the speed, strength, power and endurance of human movement. Modern society emphasizes rest. Home and work environments have been adjusted to reduce physical activity, and adaptations to this lifestyle include muscle wasting, muscle imbalance, obesity and lack of endurance. Commercial Gyms are now common place. However most feature resistance training devices that create further imbalance through the isolationist characteristics and minimal joint interplay within the human musculoskeletal system. Free weights do not provide safety in multi-joint exercises.

SpeedFlex Training Systems provide a safe and effective mode of full kinetic chain activity, while also allowing for specificity of training for a variety of movement goals.

Whether your daily performance is powerful ground based athletic movement or simply navigating through life, SpeedFlex Training Systems are tools for the evolution of human performance.

INTENDED USE

SpeedFlex products are intended for use as an exercise device. The SpeedFlex products contained in this manual are not medical devices and are not intended to treat, diagnose or rehabilitate specific injuries of the human musculoskeletal or cardiovascular systems.

Do not use SpeedFlex products with any subject encountering back pain. All users should complete a “Physical Activity Readiness Questionnaire” (PAR-Q) (se appendices) and any use of the product by you, your staff or subject clients and patients is at your sole risk. SpeedFlex, Its officers and assigns are not liable for subject injury while using SpeedFlex products in any manner outside of the intended uses described in this manual.

ADVANCED EXERCISE

Many of the exercises described in this manual are closed chain, multi-joint, gross motor movement patterns working through a user selection of isometric, isotonic and isokinetic resistance profiles. To this end, such exercise is deemed advanced, and should be performed under instruction where possible. SpeedFlex users, even those free of injury, should have minimal musculoskeletal imbalances.

The advanced exercises unique to SpeedFlex provides significant adaptations for the human musculoskeletal system. The SpeedFlex Training System engages the musculoskeletal system, cardiovascular system, central nervous and neurological system. The intensity of training performed on SpeedFlex systems will dictate the resultant adaptation. SpeedFlex is designed to become a critical tool for the evolution of human performance.

SpeedFlex Resistance Training Options and Benefits

SpeedFlex offers three distinct types of resistance designed to engage the muscles and joints of the body differently. The resistance profile types and the proposed benefits are as follows:

ISOMETRIC

An Isometric (meaning same distance or not moving) contraction, is one in which the muscle is activated and is held at a constant length. An example of an isometric contraction would be carrying an object in front of you. The weight of the object would be pulling downward, but your hands and arms would be opposing the motion with equal force going upwards. Since your arms are neither raising or lowering, your biceps will be isometrically contracting. The force generated during an isometric contraction is wholly dependent on the length of the muscle while contracting. Maximal isometric tension is produced at the muscle's optimum length, where the length of the muscle's sarcomeres are on the plateau of the length-tension curve.

BENEFITS Isometrics are purely "muscle" movements that place the stress entirely on the muscle fibers, eliminating reactive contribution, allowing for increases in muscle motor unit recruitment above and beyond eccentric or concentric movement. Activation refers to the recruitment of the motor-units in a muscle. One can recruit nearly all the muscle fibers during a maximal isometric contraction - something that doesn't happen with regular eccentric and concentric (down and up) repetitions. Strength gains of 14-40% were found over a 10-week period using isometric training.

Isometrics build muscle mass. A recent experiment found an average size improvement of 12.4% for heavy isometric training and 5.3% with isometric training using weights equivalent to 60% of 1RM (Repetition Maximum) weight after a training period of 10 weeks. (1)

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

13

Page 14: speedflex

ISOTONIC Isotonic contractions are those which cause the muscle to change length as it contracts and causes movement of a body part. There are two types of Isotonic contraction: Concentric Concentric contractions are those which cause the muscle to shorten as it contracts. Following our example in Isometric training, bending the elbow from straight to full flexion, causing a concentric contraction of the Biceps Brachii muscle. Concentric contractions are the most common type of muscle contraction and occur frequently in daily and sporting activities.

Eccentric Eccentric contractions are the opposite of concentric and occur when the muscle lengthens as it contracts. This is less common and usually involves the control or deceleration of a movement being initiated by the eccentric muscles agonist.

For example, when kicking a football, the Quadriceps muscle contracts concentrically to straighten the knee and the Hamstrings contract eccentrically to decelerate the motion of the lower limb. This type on contraction puts a lot of strain through the muscle and is commonly involved in muscle injuries. (2)

BENEFITS There are many benefits of isotonic training. Human movement is constantly under its own load in a gravitational environment. It is commonly identified that the basic premise of human bio-locomotion, walking, is an isotonic resistance exercise. By manipulating the variables in isotonic exercise- concentrically and eccentrically- through various joint ranges of motion, isotonic exercise provides goal adaptive responses for many activities.

Page 15: speedflex

ISOKINETIC

In Isokinetic exercise, dynamic muscle activity is performed at a constant angular velocity. Both torque and tension remain constant while the muscles shorten or lengthen. In other words, a concentric or eccentric contraction that occurs at a set speed against a force of maximal resistance (produced at all points in the exercise range of motion).

BENEFITS One of the main benefits of isokinetic exercise is that it can be controlled to prevent injury. Because isokinetic exercise is a “relatively controlled” form of exercise, it's often used in physical therapy. Skilled physical therapy professionals may include isokinetic devices as part of a regimen for healthy recovery after an injury or surgical procedure. (3) Isokinetic training on SpeedFlex provides a unique opportunity to maximally engage muscle groups in a variety of movements, without the risk associated with free-floating resistance such as free-weights or cables. The fixed plane Arc of Motion available in the FPM allows the user to position the body for goal based muscle recruitment patterns, many of which can occur in a standing position- functional- for many athletic based activities.

Prime Movers on SpeedFlex Exercises The identification of prime mover musculature (the dominant muscle group performing a joint action) is a relatively simple task in free weight exercise. The Concentric- Concentric action the isokinetic movement pattern, and the overall force reduction in eccentric load makes prime mover identification more difficult, and dependent upon the individual use and movement during exercise.

The isokinetic Concentric- Concentric patterns of resistance allow for two critical elements in SpeedFlex training: (1) Muscle Balance - in many cases the muscles used in a press concentrically through the first phase of a movement are counter-balanced with a pull to return using opposing concentric action. This balance is fundamental to overall movement conditioning. (2) The reduction in eccentric load- allows for a safer exercise experience overall. Eccentric movements- (lengthening a muscle under load) are important in specific conditioning practices, (and should not be neglected), however the reduction of these forces allows high rates of safe concentric activation in SpeedFlex Training Systems.

Circuit Design

Depending upon the users goals, there are many ways to design circuits using the SpeedFlex Training

System. Circuits can emphasize time at each station, a certain amount of repetitions at each station,

variation on sets and rest between sets, etc. SpeedFlex has pre-designed circuits with difficulty assessment

assigned to each, and these are made available to SpeedFlex customers for a fee. Please contact your

SpeedFlex Training Systems distributor to find out more.

Exercises

Exercising on SpeedFlex presents a challenge to most users, and movements may be considered advanced

when compared to standard isolationist resistance training products. For this reason, ensure that users are

pre-screened for exercise readiness, muscular imbalance, and movement effectiveness at each joint prior to

embarking on the SpeedFlex training program.

The exercises presented on the following pages are random examples, and do not reflect an exhaustive list.

Getting StartedWelcome to the SpeedFlex user interface. This simple step by step guide will assist you in the set up of a user program.

Initialization Screen

Fig. 1

After powering on your SpeedFlex Training System, the touchscreen will illuminate and display the “SpeedFlex

Initialization” screen shown above (fig.1). You will be requested to move the exercise arm at a moderate rate toward

the horizontal position (parallel with the floor).

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

36

Page 37: speedflex

Once you have reached horizontal, move the arm back and forth a few degrees above and then below the parallel

position until the screen changes to the SpeedFlex Main Menu, displayed in figure 2. The purpose of this

procedure is that every time your SpeedFlex machine is turned on, it must reference where the horizontal position is

for the processor to effectively calibrate. This is accomplished by means of a “proximity switch”. If you encounter

difficulty getting the Main Menu screen to appear, ReBoot by turning off the power at the source, wait 30

seconds and repeat the procedure. Once the SpeedFlex System is calibrated, it will not be necessary to do so

again until the machine is switched off and powered up again.

Main Menu Fig. 2

The Main Menu (Fig. 2) provides access to all of the features available with the SpeedFlex Training System.

The Main Menu screen offers three selections. Two of them, Manual Training and Auto Training Routines are to

establish user training .

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

37

Page 38: speedflex

Technician Setup is password protected and designed to enable SpeedFlex Service Technicians to diagnose the

various functions of the machine's mechanical parts, electrical system and its 3 microprocessors.

Manual Training MenuIf your desire is to set up SpeedFlex for a few, quick sets, or target a specific muscle group through a SpeedFlex

exercise, you may prefer to use Manual Training for the quickest and easiest way to begin exercising. This mode

may also used by professional trainers to assess physical properties such as strength and endurance.

From the Main Menu select Manual Workout. You are now given the option to select one of the three exercise

modes that SpeedFlex offers for training, as seen in figure 3; Isometric, Isotonic and Variable.

Fig. 3

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

38

Page 39: speedflex

Isometric Training simply locks the arms in a stationary position. This allows for fixed bar isometric contractions, or

you may perform isotonic work with body weight in a fixed position, such as pull-ups, dips, etc. Isotonic Training

works similarly to the way conventional weight machines work with the exception that you are the one applying force

upon the machine. The machine never expends force upon you. Variable Training is the ability of SpeedFlex to

limit the velocity of your movement, with that movement being measured in degrees per second.

Fig. 4

“ID” WindowThe SpeedFlex Menu System uses color codes to feature the different functions of the touch buttons on the screen.

Red = enter a value

Blue = navigation

Gold or Green = change a condition

Gray = no value can be entered

Page 40: speedflex

In the upper right hand corner of figure 4 there is a red window labeled ID with a few numbers following. Each

Training Mode has a red ID window in the upper right hand corner that allows you to input your unique ID number. If

you enter the number “0”, you can begin exercising immediately however the SpeedFlex System will not save any of

the data from your exercise session. If you enter your ID number, SpeedFlex will save and record the data from

each exercise at the end of each routine. This protocol applies to each every exercise mode.

Selecting a User ID.

When you enter your ID, you will be prompted to insert your thumb drive into the USB port, located next to the On/

Off selector switch in the back of the machine. You will not be able to start your routine until you place your stick in

the USB slot or change the ID field to “0”. You may choose any number from 1 to 999,999 for your ID. Always

use the same number.

Graph Button Functions

The Graph (Fig. 5) Displays four items during exercise movement:

Degrees Up: How far up you are moving the input arms on Speed Flex

Degrees Down: How far down you are moving the input arms on Speed Flex

Speed: How fast you are moving the input Arms (measured in degrees per second)

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

41

Page 42: speedflex

The Graph function also allows you to isolate any of the four items being recorded that you would like to to observe,

by pressing its colored icon at the bottom of the screen. You can also enlarge the graph view by pressing the down

arrow in the lower left corner of the screen. The buttons at the top left operate in the same manner as those on a

video recorder for going forward or backward. The CLR button clears the current data off the screen and the blue

Back button returns you to the previous screen.

Isometric Training Mode

Figure 4 (Fig.4) shows the menu selections for Isometric Training Mode. When you select this mode, first place the

arms of the machine in the position necessary to perform your exercise. If you have entered the value “0” in the red

ID button, then you may press LOCK ARM on the touchscreen to lock the arms in place, then begin exercising. If

you entered your unique ID number, a Gold START TEST button will appear on the right side of the screen. When

you press START TEST, the LOCK ARM button will change from Gold to Green. When you press LOCK ARM, the

number 5 will replace START TEST on the screen and will count down to zero with a simultaneous audible count

down.

When the “test” begins, the screen displays the amount of Rotary Force being applied to the arms, measured in

foot pounds. Concurrently, the clock on the screen is counting up the seconds that a minimum of 10 pounds of

rotary force is being applied. As soon as the Rotary Force being applied drops below 10 pounds, the test is

complete. At this point the clock will stop and the touchscreen will display the Training Results screen (figure 6).

Page 43: speedflex

Fig. 6

Displayed in Figure 6 (Fig. 6) is sample data resulting from a one set test.

RF = “Rotary Force”

ROM = “Range of Motion” measured in degrees.

Notice at the bottom of figure 6 the window displaying “Saving Data” with a green progress bar. This is the data

being saved to your memory stick in real time. If you are in the Isometric Training Mode, the Reps Done will always

be “0”. After 15 seconds or the time it takes to save your data, you will be returned to the Isometric Training Mode

screen (Fig. 4). At this point you may repeat the test or go to a different test or exercise mode.

Page 44: speedflex

Isotonic Training ModeIf you desire to workout against a fixed resistance, train with “Isotonic Training Mode”. If you want to record the

data to your memory stick, be sure to enter your personal identification number in the ID block before starting,

otherwise, enter “0” to continue.

To proceed from the Manual Training Menu (Fig. 3) choose Isotonic Training. When you see the Isotonic Training

Mode screen (figure 7), enter the amount of weight you want to work out with by pressing on the RF UP LBS or RF

DWN LBS button. Once selected a keypad will appear allowing you to enter a value between 10 and 325. Anything

outside of that range will cause an “error” message to appear and require the user to amend the value. Once you

input a recognized value, press Enter and the keypad will vanish, leaving the value that you selected in the field. If

you are satisfied with the values on the screen, you can press START TEST and a “5” will appear in the START

TEST button and the countdown will begin on screen (and audibly) counting backwards from 5 to 0. When the

countdown reaches 0, begin exercising.

Fig. 7

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

44

Page 45: speedflex

While exercising, the touchscreen will display the amount of Rotary Force (in pounds) being applied to the arms (in

real time), the velocity of the arms (how fast you move them measured in Degrees Per Second) and the total time

exercising for the set. If you are recording data using your personal ID, the test will continue until you stop moving

the arms. If you stop and immediately resume, the screen will continue to show your progress, however the

recording of data for your report ceases. Remember- you can review progress in real time by using the Graph

feature.

After 10 seconds of inactivity, the results screen (figure 6) will appear and your data will be saved to your thumb

drive if you entered your personal ID. Anytime during your workout that you deem it necessary to stop, simply

press the Blue Back button in the lower right hand corner and you will be returned to the previous screen.

If you entered “0” in the ID box when you started, SpeedFlex continues to display the screen in Fig. 7 or the graph in

Fig.5, until the blue Back button is pressed. When 10 seconds of inactivity occurs, the RUN TIME clock resets to 0

and the graphing mode stops. With the commencing of any new exercise activity the clock and the graphing both

start recording again until there is another 10 second period of inactivity. This works the same in VARIABLE

TRAINING MODE. As a result, user(s) can continue doing various exercises in the same mode without having to

reset values every time between sets. This feature can be extremely useful for Circuit Training.

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

45

Page 46: speedflex

Variable Training ModeThe SpeedFlex Training System provides the unique ability to train “Fast Twitch Muscle” groups through higher

velocity Training. In “Variable Training Mode”, as you increase your celerity, SpeedFlex proportionately increases

the resistance you encounter. This combination of dynamics we’ll call Velocity Training, and is a powerful feature of

the SpeedFlex training System.

Fig. 8

To begin from the Manual Training Menu (figure 3) press Variable Training Mode. The screen seen in figure 8 will

appear. This menu works identically to the Isotonic Training Mode menu (Fig. 7) previously discussed. The only

difference is (1) you enter a resistance value between 1 and 20 for up and down and (2) everything is measured in

Degrees per Second.

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

46

Page 47: speedflex

To set the resistance value you desire, select either RESIST UP or RESIST DWN and a keypad will appear. Enter a

value from 1 to 20, with 20 having the greatest amount of resistance and 1 being the lowest resistance setting.

Since Variable Training Mode measures velocity (Degrees per Second), there is no value in pounds that relates to

the resistance. Instead, there are 20 increments of resistance that increase with each greater number that you may

use for your workout. The greater the value, the slower the speed you will be able to move the arms.

Results are displayed on the touchscreen (figure 8) in the Degrees per sec box, located in the middle of the screen.

You may also watch in graph view by pressing the blue GRAPH button at the bottom left corner of the screen.

Recording your results to a thumb drive works identically to the method covered in the last section on “Isotonic

Training” as does ten seconds of inactivity resetting the graph and the RUN TIME values. Setting the “ID” to “0”

causes the same effects that is does in “Isotonic” mode. After 10 seconds of inactivity, you are likewise brought to

the SpeedFlex Training Results screen which will display all of your results from the time you started until the very

first time you stopped. If you accidentally stopped too soon, immediately press the Blue “BACK” button until you are

back to the “Manual Training Menu” (Fig. 3), and start over again.

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

47

Page 48: speedflex

Auto Trainer SetupWith Auto Trainer you can design your own personal workout consisting of up to 10 different routines, selected from

a list of 29 different exercises. All these can be done in three different modes (virtually 87 exercises) and you can

select from 1 to 150 reps, or 15 to 300 seconds per exercise. Rest time between exercises can be set from 15 to

120 seconds and all your workout data can be downloaded onto a flash drive so you can track your progress over

any selected amount of time with Excel on a spreadsheet.

To begin, select Auto Training Routine (Fig. 2) from the Main Menu. You will be brought to the Auto Trainer Setup

menu (figure 9). The first screen is a list of the attributes of the SpeedFlex Auto Trainer, most of which was

covered previously.

Fig.9

To move forward, press the Blue Continue button (Fig. 9) at the bottom of the Auto Trainer Setup screen. You will

now be making your selections from the Auto Trainer Start Menu, seen below in Figure 10 (Fig.10).

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

48

Page 49: speedflex

The first choice to consider is if you are going to record your results by using your personal ID number, which

requires you to plug your thumb drive into the USB port, or simply enter the “0” value into the Red ID box to not

record a value.

Fig. 10

If this is the first time setting up the Auto Trainer or perhaps you need to make changes to a routine that is already

saved, select Goto 1st Exercise (in Blue on the right side of the touchscreen) and you will be taken to the Exercise

Selection screen (Fig. 11). From here you will begin to build your routine one exercise at a time. Once you design

your routine, it will always be there for you. You may go back at any time to make changes of any type. Some

changes can be made on the fly, as you will see shortly.

Fig. 13

You can toggle back to the previous screen by pushing Back, or you can make your choice from this screen. To

choose, press Select EXERCISE Type Number which will pop up the keypad screen. Make your choice by

entering the corresponding number of the exercise from the menu into the keypad. For example, if you want your

first exercise to be Military Press, you would select the number 4 and then press Enter. Next you will see the

Select Exercise (figure 11) screen. Looking at the top, you will notice that the Gold Exercise # field now displays “1”

and the Select/ed EXERCISE field now reads “Military Press”.

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

51

Page 52: speedflex

The next step is to select a mode for this exercise. The choices are Isometric, Isotonic or Variable. To do the more

traditional type exercise, you want Isotonic. The default setting is “Isometric”, so toggle the button labeled Isometric

Mode Selected until you see it displays Isotonic Mode Selected. Because you selected Isotonic the Resist UP &

DWN boxes on right are shaded in gray and the RF UP LBS & DWN LBS are highlighted in red. SpeedFlex will not

allow a Resist UP or DWN value to be entered because Resist values only pertain to “Variable Mode Training”.

Select your values, for instance RF UP, “150” lbs and RF DWN, “0” lbs, since you only want to encounter resistance

in the upward direction. There may be times when you will want to have resistance in both directions, but not for

this example. Next you can define the length of this exercise by sets or by time. Sets (overall volume of work) and

rest (time between sets) are important physiological variables in adaptation.

Now that you have completed your first exercise in your routine, touch the Blue Next button at the bottom right of

the screen to move to the next screen (figure 12). Now the Gold field will read Exercise #2. Follow the same

procedure for entering a new exercise into your Auto Training Routine.

When you have completed selecting all of the exercises you want included in your routine, press the blue Finished

Exercise Setup Button at the top of the screen in Fig.11. At this time you will be brought to the screen shown in

Fig. 10.

Auto Trainer “Review” Screen

Fig. 14

After you have completed your “Auto Trainer” setup, you may want to look at your entire routine. To toggle through

all the screens is time consuming and a nuisance. To simplify this process, you can access the “Review” screen

(figure 14) from the Auto Trainer Start” screen (figure 10) anytime you wish to review your exercise regiment. All of

your times, sets, reps and values are in one convenient place for you to easily edit your workout.

If you wish to make any changes, press the Blue “Back” button and you will be returned to the “Auto Trainer Start”

menu (figure 10). From there select Goto 1st Exercise and you will be taken to the Select/ed EXERCISE screen

(figure 11). From there push the Blue Next or Back buttons to navigate to the exercise you wish to modify, make

your changes and exit out.

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

53

Page 54: speedflex

You can access the “Review” screen and make changes to your Auto Trainer Workout anytime you deem it prudent

to do so. NOTE: Once you have set up the Auto Trainer, the program will remain in the SpeedFlex Training

System memory, even if you unplug the system.

Fig. 15

By selecting the gold Start button on the Auto Trainer Start screen (figure 10), a countdown begins inside the Start

button, counting from 5 to 0. When it reaches “0”, the screen in figure 15 appears and your “Auto Trainer” workout

begins as soon as you move the arm (or press LOCK ARM if your first exercise uses Isometric Mode).

The gold Exercise # field at the top informs you of which number exercise you are on and the gold EXERCISE field

at the right tells you which exercise you should be performing (for example, “Bench Press”). SETS REMAINING,

Reps Done and REMAINING (time) are all self explanatory. Rotary Force Lbs tells you in real time how much RF

you are applying to the arm. Deg per sec informs you how fast the arms are moving.

CONFIDENTIAL DRAFT (C) 2012 SpeedFlex User Manual Version 2.1

54

Page 55: speedflex

Fig. 16

If you are performing an exercise in Variable Mode, you will follow your progress on a screen identical to the one in figure 16. Most everything is identical to the “Isotonic Mode” screen in figure 15, except the Degrees per sec and RF lbs are flip-flopped in priority.

Fig.17

55

Page 56: speedflex

Should you include an exercise in your routine that is performed in Isometric Mode, you will be working with the

screen displayed in figure 17. Again, most of the fields are similar to Isotonic and Variable, with the main difference

being the gold LOCK ARM button in the middle of the screen. When you have an Isometric exercise in your routine,

make sure the arm is in the position it needs to be in for your exercise, then press “LOCK ARM”. SpeedFlex will

begin counting down (and recording if you input your ID number). The arm will remain locked until your set is

complete and the Green UNLOCK ARM button (which replaces the gold LOCK ARM button) is pressed. Abort and

Exit will also release the arm.

Fig. 18

At the completion of each exercise set, you will be shown the Exercise Results screen (figure 18). This screen is

almost identical to the screen in figure 6. You can see the results and the amount of time remaining in your rest

period. At the completion of the final set of your entire workout, the Saving Data bar will appear at the bottom of the

screen while all of the data of your Auto Trainer Workout session is saved to your memory stick. When all the data

is saved, you will be returned to the Auto Trainer Start menu.

PHYSICAL ACTIVITY READINESS

PAR-Q

For most people physical activity should not pose any problem or hazard. PAR-Q has been designed to identify the small number of adults for whom physical activity might be inappropriate or those who should have medical advice concerning the type of activity most suitable for them.

Common sense is your best guide in answering these few questions. Please read them carefully and check the yes or no opposite the question if it applies to you.

1. Has your doctor ever said you have heart trouble?

2. Do you frequently have pains in your heart and chest?

3. Do you often feel faint or have spells of severe dizziness?

4. Has a doctor ever said your blood pressure was too high?

5. Has your doctor ever told you that you have a bone or joint problem such as arthritis that has been aggravated by exercise, or might be made worse with exercise?

6. Is there a good physical reason not mentioned here why you should not follow an activity program even if you wanted to?

7. Are you over age 65 and not accustomed to vigorous exercise?

If you answered YES to one or more questions: Consult with your personal physician by telephone or in person before increasing your physical activity and/or taking a fitness test. If you answered NO to all questions and you answered PAR-Q accurately, you have reasonable assurance of your present suitability to begin exercise under qualified supervision.

Dear Doctor:

SpeedFlex is a resistance training product that promotes muscular balance and full body movements, and exercise will be performed under qualified supervision. The Staff may perform a fitness assessment to determine current fitness status, which may include a sub-maximal cardiovascular fitness test and measurements of body composition, flexibility, and muscular strength and endurance.

After completing a readiness questionnaire and discussing their medical condition(s) we agreed to seek your advice in setting limitations to their program. By completing this form, you are not assuming any responsibility for our exercise and assessment program. Please identify any recommendations or restrictions for your patient's fitness program below (Physician's Recommendations).
