NOTICE Based on queries received over telephone and email regarding the online form fill-up process for WBJEEM-2014, especially on Domicile/Residential Certificates, Income Certificates, Date of Birth (DOB) certificates and self- attestation of photocopies of such certificates, the WBJEEB advises the applicants to carefully note the following points: Domicile/Residential Certificate 1. Domicile/Residential Certificate, if applicable, should be provided either in Proforma-A1 or in Proforma-A2 or in Proforma-B as given in the WBJEEM-2014 Information Brochure. Certificate in any of the three abovementioned proforma will serve the purpose, even if more than one proforma is applicable for the candidate. 2. If Proforma-A2 is used, Head of the Institution from which the applicant passed/will appear for his/her 10+2 level examination is the signing authority. In case Head of Institution is not available, the person officially holding the said position will be the signing authority. 3. If Proforma-A1 or Proforma-B is used, the signing authority must be a Government Official; a list of such authorized signatories is given in section 5.3.3 of WBJEEM-2014 Information Brochure (pages 8-9). Proforma-B should be used only if the applicant has not resided in West Bengal continuously for the last ten (10) years; even if he/she is residing in the state now. 4. In Proforma-A1 or Proforma-B, where the signatory is a Government Official, filling up of the following fields is not mandatory:  Mobile number of the signatory  ID number of the signatory However, the applicants should request the signatory to provide his/her office landline number so that, if necessary, for verification purpose the Board can contact his/her office. 5. Along with the Domicile/Residential Certificate the applicant should provide a photocopy of the voter ID card/passport (2 nd  page) of one of his/her parents (either father or mother). In case none of his/her parents possesses a voter ID card/passport, a photocopy of any valid photo ID card of one of the parents issued by either the Central or State Government bearing his/her address, should be provided instead. Note: In case none of the parents is the legal guardian of the applicant, all such documents pertaining to the parent’s ID should be substituted b y the corresponding ones of the legal guardian accompanied by an affidavit affirmed before a 1 st  Class Judicial Magistrate as a proof of his/her guardianship .

NOTICE

Based on queries received over telephone and email regarding the online form

fill-up process for WBJEEM-2014, especially on Domicile/Residential

Certificates, Income Certificates, Date of Birth (DOB) certificates and self-

attestation of photocopies of such certificates, the WBJEEB advises the

applicants to carefully note the following points:

Domicile/Residential Certificate

1.  Domicile/Residential Certificate, if applicable, should be provided eitherin Proforma-A1  or in Proforma-A2  or in Proforma-B  as given in theWBJEEM-2014 Information Brochure. Certificate in any of the threeabovementioned proforma will serve the purpose, even if more than one

 Income Certificate

Applicants who want to avail of the TFW scheme must provide the duly filled-

in Income Certificate  declaring the total annual family income for the year2012-13. The certificate should be signed by a Government Official; a list ofsuch authorized signatories is given in section 16.1.4 of WBJEEM-2014Information Brochure (page 24).For those who do not want to avail of the TFW scheme, Income Certificateneed not be provided. 

 TFW scheme is available only for applicants seeking admission to

undergraduate level Engineering/Technology, Pharmacy and Architecturecourses and not to Medical/Dental courses. 

DOB Certificates

 The Confirmation Page generated after completion of the online form fill-upprocess, lists a handful of documents to be provided by the applicant. The firstone of the list reads as:

“Photocopy of Admit Card of 10th standard/Registration Certificate of qualifying(10+2) examination duly attested for Date-of-Birth proof.”   The Birth Certificate issued by any MunicipalCorporation/Municipality/Panchayat under a State/UT Government is a validDOB proof. Applicants not possessing the Admit Card of 10 th standard/Registration Certificate of qualifying (10+2) examination or for whomsuch documents do not record the DOB, should submit a duly attested copy of

his/her Birth Certificate instead.

Self  –  attestation of photocopies of certificates

All documents and photographs, including those pertaining to the parents ofthe candidate, to be self attested should be signed by the applicant only.

Note: Applicant found guilty of furnishing incorrect information to obtain a

certificate or furnishing counterfeit certificate is liable to penal action

including criminal prosecution.
