SAFETY OF CONTRACTORS A. PURPOSE :

This procedure is to be provided to ensure all contractors and their employees who are working with or involve in PT. H&H Utama International operation activity know and follow the Safety Program of PT. H&H Utama International :

- For incident prevention to protect all workers, Equipment /Client Properties, Process Loss and Environmental from the Risks of the operation activities.

- It is also as the guidance to the supervisor and all workers concerned for performing the work safely

B. RESPONSIBLE PERSONS :

1. Operation Manager/ Authority. He is responsible for :

Ensuring that all procedures and requirements are fully implemented by Contractor

Ensuring that personnel engaged on the project are adequately qualified, experienced, and trained to carry out their task safely.

Ensuring all Persons act in accordance with these procedures. Ensuring all persons attend the pre job Safety Briefing that conducted by Safety

Dept.

2. Safety Manager / Authority. He is responsible for : Providing Hazard Identification & Risk assessment for the job that will be done. Conduct a safety briefing to the workers before sending to the client Ensuring that all safety equipment/PPE & instructions stated on the Risk

Assessment & Preventive measures are available at the work site and used correctly

3. Contractor/Project ; Manager/Supervisor/ Foreman (Performing Authority).

He is responsible for : Obtaining the Work Permit & following the instructions in Work Permit. Ensuring that all personnel under their control act, and the task is carried out in

accordance with the procedures Conducting daily Safety Toolbox meeting to the workers prior the work.

Ensuring that all safety equipment/PPE & instructions stated on the work Permit or on Risk assessment & Preventive Measure is at hand and is used correctly.

Reporting any problem or any incident at Project directly to Batakan Base HH. Safety Manager.

Providing Weekly Report of Safety performance, statistic etc. and send the Report to HH Safety Manager.

4. Man Power / Workers.

Each person has a responsibility to take reasonable care for health and safety of himself, others and environmental that may be affected by his action or omission, including : Following Safety, Health & Environmental Instruction and Regulation. Mandatory to attend Safety Induction Using Personal Protective Equipment. Taking good care of equipment Reporting all hazards and any accident Reporting work related injuries or harm to health. Co-operating with other workers and employee

C. PROCEDURES / PROSEDUR

Introduction.

It is the policy of PT. H & H UTAMA INTERNATIONAL that the Safety of Contractors employees working for us on our sites should be of the same high standard as that of our own people.

Every Contractor that will do business with PT. H&H Utama International must fill in the Pre qualification questionnaire form in Attachment 1. All employees involved on the job must attend the Safety & environmental Induction Training.

The following observation suggest some ways and means to monitor the safety performance of contractor.

1. SELECTION OF CONTRACTOR

When selecting a contractor do not only look at his price, his job record, his technical competence and his efficiency but also ask for his safety record.

It is suggested to probe the contractors safety a long the following lines: a. Technical Safety , e . g.

(a) The quality of the tools he uses, (b) His Scaffolding, and other appliances (c) His hoisting equipment, does he test it from time to time. (d) The state of repair of his vehicles and other engine powered

equipment. b. Personnel Protection , e . g . :

(a) Does he provide protective clothing of adequate standard where

needed e . g . for welders. (b) Does he provide protective equipment like helmets, goggles, etc. ,

where needed. (c) Have his supervisors been made aware that they are responsible for the

safety of their men. (d) Has he got any medical first aid equipment. (e) Are any of his men capable of administration medical first aid. (f) Can he show any figures to support his record?

c. Damage Control , e . g . :

(a) Has he caused fire or damage, in his previous jobs, (b) Does he know the difference between extinguishing of oil fire and

wood fire.

The least that an enquire into the contractors safety record will do is to make him aware of the increasing importance of the matter. 2. The Contract. A number of safeties related items are best laid down or paper and agreed between

yourselves and the contractor before the work starts. If new or more stringent safety measures are introduced during the work (while these measures are not specified in the contract), this may cause delays and the contractor may use it as a lever to either increase his price or as an excuse for late completion.

Note that the following is meant for consideration in respect of the many contracts concluded with local contractors e.g . for maintenance , plant change on shutdown, grass cutting, cleaning , etc.

It is at this stage not meant to relate to major construction contracts. The following items are suggested for inclusion in the contract or as appended clauses

a. Adherence to Company General Safety and Fire Regulations.

These General Safety Regulations should be on paper and handed to the Contractor. The following items should be considered: - Smoking limitations - Adherence to the permit system - Traffic discipline in general and no blocking of roads - Specific action of out of bounds areas. - Contractor movements should be restricted to their work sites. Plant areas

tank farms, loading areas, oil separator areas etc. Should be out of bounds if the work is not actually in one of these areas. Unless this is done, safety supervision cannot be as effective as one would like.

- It is better in general to prohibit the use of the fire water system for cleaning tools, washing etc. Unless special permission is granted in individual cases.

b. Provision of Safety Equipment.

The contractor should be made responsible for the provision of safety equipment for his own men. It may be, however, to our advantage to help out in the event of rather specialized equipment, like breathing sets that may not be readily available.

Where possible, specify what equipment, will be needed for the job reserve the right to refuse equipment that you regard unsuitable or of inferior quality.

Note that the required safety equipment should also be specified on our permit (though, unfortunately, by the time these are issued the contract has already been awarded). Should safety equipment be needed which was not anticipated at the contract stage, it may be a good form of economy to provide it from our own sources.

c. It is considered desirable to give safety instructing (latrine course) to the

contractors supervisory personnel before the job starts, it is best to include this also in the contract.

d.. No Change of Contractor Supervision During the Contract.

This is not always possible e . g . in the case of the supervisor resigning. The contractor, however, should be obliged to consult you if he wishes to change his supervision during the contract. The need for this clause is obvious. Continuity is essential in order to attain supervision with adequate understanding of typical petroleum / Contruction industry safe practice especially in relation to a specific job.

3. PERMITS The work permit system is probably the most effective single method for monitoring

the contractors safety performance.

a. General. The rules and regulations regarding permits are the same for our own and for contractors personnel. But in the case of the later we have to be more precise and thorough to cater for any possible ignorance of safe practice in the petroleum industry and any failure to understand safety instructions etc. ( Defects of Persons ). The basic types of permits we use should be explained e.g. Cold work, entry, hot work, and Electrical permit. It should be explained that an application for permit is required how and when permits can be obtained and who should keep the permits and where. Note : That contractors supervisors should be familiar with all types of

permits but everybody should at least know that application for work permit is required prior to undertaking any work outside of routine maintenance etc.

b. Filling in the Permit Performance.

Instruction should be specific. Sentences like Have a 6 FE standby may not mean very much because the contractor may not know that a instruction should read. Have a 6 kg dry chemical hand extinguisher standby at the work site. Furthermore , reference should be made to specific vessels, lines, tanks,

etc. and to mention be specified what safety equipment is needed, e . g . goggles to worn, safety belts to be used etc.

c. Starting the job.

(a) Point out specific hazards on the spot. It is for instance not

sufficient to state on the permit that drains within 15 meters should be covered with wet sacks points out which ones on the spot.

(b) Inspect contractors safety equipment: tools, welding machines, electric cables for breaks in insulation, gas flexes, scaffolding. Enquire as to when hoisting machinery and slings, etc, were last inspected.

(c) Inspect contractors safety equipment or ask safety department to do this.

Recording to Contractor Safety Performance. Accidents to contractor staff and our own personnel must be investigated recorded and reported in the some way ( using the same preformed as for our own personnel ) if any useful comparison of the two is to be made. Meaningful comparison of overall performance is possible if a separate contractor lost time accident frequency rate is computed. Either the man hours worked can be supplied by the contractor or a fairly accurate estimate can be made by the department for when the contractor carries out the work.

D. EVALUATION

Evaluation Team consist of all Safety Staffs, Safety Manager assigns one staff as a Coordinator and other Department concerned will act as Evaluation Team. Contractor Evaluation Team will evaluate : 1. Company Profile & data

2. Other reference 3. Answering of Pre qualification Questionnaire If necessary the team will visit to Contractors Place or Work Shop Result of evaluation will be reported to HH General Manager for approval that Contractor meet or not meet to HH Safety requirements.
