in Lakeland Regional Medical Center’s (LRMC) critical care units
determined there were still opportunities for improvement.
While they had the desire to implement changes, they needed
guidance from people who understood the challenges they
faced and could provide the support and resources to ensure
changes were measured and sustainable. They found this sup-
port in the Society of Critical Care Medicine’s (SCCM) Paragon
Critical Care Quality Implementation Program™ (Paragon).

Critical Care Congress will highlight quality and performance
improvement processes in critical care.
Since its inception in 1970, SCCM has maintained that the best
way to improve staff satisfaction, enhance patient outcomes
and boost the financial performance of hospitals is to deliver
the Right Care, Right Now™. This efficient and safe patient-
centered model of care can be achieved by establishing a
standardized intensive care unit (ICU) structure consisting
of an integrated multiprofessional team led by a trained
and present intensivist. These high-performance teams are
designed to achieve excellence in patient care.
To help healthcare providers implement the Right Care, Right
Now model and benefit from quality improvements, SCCM
created the Paragon Critical Care Quality Implementation
Program (Paragon). Paragon partners SCCM members who

Fundamental disaster ManagementJanuary 8, 2010Prepare yourself and your intensive care unit to man-age the surge in critically ill or injured patients after a disaster. Fundamental Disaster Management (FDM) provides the most recent information on disaster response, management of mass casualty injuries and allocation of scarce critical care resources. Disaster scenarios addressed include natural and man-made events (biological, chemical and radiation). Skill sta-tions provide hands-on opportunities to use alterna-tive positive pressure ventilation equipment and a higher level of personal protective equipment (PPE). Registration fee includes a copy of the FDM text-book, as well as breakfast and lunch. An instructor component is available for those interested in teach-ing FDM.
adult Critical Care Refresher CourseJanuary 8-9, 2010Valuable for all members of the multiprofessional team, this two-day course focuses on current stan-dards of practice in critical care and provides a review of basic concepts. Updates on select topics such as therapeutic hypothermia, noninvasive venti-lator support and quality and performance improve-ment issues also will be provided. Registration fee includes an in-depth syllabus, as well as breakfast and lunch. All speaker slides will be available to par-ticipants after Congress.
Current Concepts in Pediatric Critical CareJanuary 8-9, 2010Obtain the latest information available for both the clinical and basic science realms of pediatric inten-sive care. Topics include pediatric intensive care unit (PICU) technologies, management of burn injuries, relevance of biomarkers in sepsis management, cardiomyopathies and inflicted brain injury, transla-tional medicine, ethical challenges in the PICU, and how to build collaborative PICU networks and busi-ness models. This course is a must for all pediatric critical care practitioners. Registration fee includes an in-depth syllabus, as well as breakfast and lunch. All speaker slides will be available to participants after Congress.
Pediatric Fundamental Critical Care SupportJanuary 8-9, 2010Learn to manage unstable critically ill or injured pediatric patients. Pediatric Fundamental Critical Care Support (PFCCS) utilizes a combination of didactic and skill stations to address such topics as management of acute respiratory failure, post-operative management, sedation and pain man-agement of children, and transport of the critical-ly ill child. This two-day provider course parallels the adult FCCS course. Registration fee includes a copy of the PFCCS textbook, as well as breakfast and lunch. An instructor component is available for those interested in teaching PFCCS.
Fundamental Critical Care Support instructor CourseJanuary 9, 2010Become trained as a Fundamental Critical Care Support (FCCS) instructor and ensure that your pa-tient care staff has the confidence and skill to treat the critically ill and injured. This course curriculum incorporates a structured and formal approach to adult learning concepts with a focus on teaching strategies. It is strongly recommended that partici-pants be currently practicing in critical care, as the traditional two-day FCCS course is condensed into a one-day program. CoMMiT. TRanSFoRM. iMPRovE. CaSE STUdy (continued): After a pre-assessment and a two-day site visit, SCCM’s Paragon coaches helped the LRMC
team identify two crucial areas for improvement: early diagnosis and intervention of sepsis in the
emergency department (ED) and on the hospital wards, and reduction in ventilator-associated
pneumonia cases. SCCM’s Paragon coaches had extensive experience with integrating the sepsis
bundles and understood the barriers and strategies involved. The team began by applying the
basics of quality improvement using rapid-cycle change and immediately made progress.

criteria for and receiving the sepsis resuscitation bundle. Of those
receiving the bundle, 62% met the goal of a ScvO2 >70% -- one of the
highest rates of compliance seen by the Surviving Sepsis Campaign. The
ICU team worked collaboratively with the ED to implement processes to
improve early recognition and treatment of sepsis. This included alerting
the medical ICU charge nurse to potentially septic patients, revising the
screening/order sheet, educating staff in the ED, and developing an early
goal-directed therapy cart with supplies.

To reduce ventilator-associated pneumo-
nia (VAP), the team revisited processes
and focused on strategies that had been
successful in the past. They then audited
compliance with the standards of care
for the ventilated patient, such as ventila-
tor bundling, glycemic control, meticu-
lous oral care, and care during transport.
In addition, the team engaged the
bedside registered nurse and respiratory
therapist to create processes that would
improve compliance and raised aware-
ness by utilizing standardized tools for all
staff communications. The result was a
50% decrease in the VAP rate.

Acute hyperglycemia was once regarded as an
adaptive response to critical illness. However,
a plethora of studies have challenged this idea
by reporting increased risks associated with
hyperglycemia and benefits of improved glucose
control in intensive care unit (ICU) patients.
As a result, some experts promote routine
administration of intensive insulin therapy (IIT)
to normalize glucose levels. Others question
the practice due to concerns over the optimal
glucose level, the accuracy of measurements,
the resources required to attain tight glycemic
control (TGC), and the impact of TGC across the
heterogeneous ICU population. Furthermore,
increased variability in glucose levels during critical
illness and the therapeutic intervention thereof
have recently been reported to have a negative
impact on patient outcomes. The concept of
lowering morbidity and mortality rates using TGC
intervention remains controversial.
