Patients complete the questionnaire by

Each completed answer automatically advances the program to the next question with the

option to go back and alter the previous answer.

How does it work for the Physician?

The web-based program automatically calculates the patient’s risk factor for a specific

sleep disorder and displays a report on a designated Tablet or Smartphone.

Physicians and designated medical staff can send an electronically signed referral with a single

finger touch to any sleep testing provider programmed into the system.
