Case Study 1While a leveller was climbing along a bamboo scaffold inside a liftshaft of a building under construction after ink marking work to return to the landing on 7/F, he suddenly lost his balance and plunged to the bottom of the liftshaft. He sustained severe head injury and was certified dead on spot.

Case Study 2A loader under repair at a machinery depot was raised at the front by pressing its bucket against the ground. A maintenance worker undertaking the repair work was somehow trapped between the lifting arm of the bucket and a front wheel of the loader. He was certified dead later on the same day.

Case Study 3A worker while performing pre-use check of a permanent suspended working platform installed on the upper roof of a residential building, fell about 8 metresfrom an unfenced edge of the upper roof to the roof below. He sustained serious injury and passed away on the same day. 5
Section 10 OSH Ordinance, Cap. 509Commissioner May Serve Suspension Notice
The commissioner may serve a suspension notice on an employer who is responsible for, or on an occupier of, premises where a workplace is located, if of the opinion that because of—
(a) an activity undertaken on the premises; or
(b) the condition or use of the premises or of any plant or substance located on the premises,
there is an imminent risk of death or serious bodily injury.6

Section 10 OSH Ordinance, Cap. 509Commissioner May Serve Suspension Notice
(4) Where a suspension notice is in force in relation to an activity, premises, plant or a substance, the commissioner must, by notice in writing served on the employer or occupier concerned, revoke the suspension notice on being satisfied that—
(a) the undertaking of the activity on the premises; or
(b) the condition or use of the premises, or of any plant or substance located on the premises ,
no longer creates, and is no longer likely to create, an imminent risk of death or serious bodily injury. 10
