GENESIS XTREME ALLSTARS Eddie Roberts Athlete Scholarship

Dear Future GXA Athlete,

Thank you for taking the time to consider participating on one or more of our

GXA AllStar Teams. GXA Athletes train from ages 6 to 18 years of age with

diligence and dedication to AllStar Cheerleading. This letter briefly provides a

further understanding of our AllStar Organization and what the GXA

Scholarship means.

Established in 2009, Genesis Xtreme AllStars is Suwanee, GA’s most

distinguished AllStar Cheerleading & Training Facility. GXA CHEER takes great

pride in teaching athletes’ discipline, determination, self-motivation, confidence,

respect for others, teamwork, sportsmanship and perseverance. We administer

these teachings above through coaching, mentoring and being actively involved with each child. GXA CHEER also promotes healthy development of each

athlete physically, mentally, emotionally, and morally through athletics and

training.

GXA believes in recognizing these young athletes for their time commitment and sacrifices on and off the floor. These athletes will show astonishing work ethic in

Stunting, Tumbling, Jumps, Motions, Dance, Flying/Basing, Training and

Personal Leadership Qualities for themselves and their GXA Team. As you can

imagine, these attributes are not given to an individual, but earned by an

individual.

Athlete Scholarship(s) will go to the Top 1 or 2 Athlete(s) on each Full Year Level

team. The number of Athletes selected from each team will be at the Coach’s

discretion. Scholarships will be determined from above listed attributes, practice

attendance, team voting, and Coach evaluations. GXA scholarships will not be

given out lightly, they will be EARNED!! Scholarships will be given out at the

end of the season. This is to adhere that the proper athlete(s) are chosen with

wisdom and discernment. Full details of Scholarship amounts will be discussed at

the GXA AllStar Parent meeting in August.

We invite families to partner with us to promote excellence in the GXA AllStar

Section 2 – Practices

All Practice are Mandatory – Be on Time, Prepared & Practice Ready!!! Athletes will have a Sunday and

weekday/s practices depending on level. Additional practices will be added as the coach deems necessary.

Practices are subject to be changed and/or added throughout the season and the coaches will try to give as much

notice a possible for these changes. (For Absence policy see page 6.) Athletes are expected to maintain all level appropriate skills. If not kept, they will be moved to lower level team or

forfeit all competition fees. No refunds will be given. No one can yell onto the floor or to speak to any team member or coach while practice is in session. Please email

any concerns you need addressed [email protected] Please do not drop your cheerleader or siblings off early for practice or pick them up late. We do not offer

babysitting. We will not be responsible for your child after hours. The gym will open 5 minutes prior to the start

of practices. The gym will close promptly at the end of practice.

Acceptance of Practices

Section 3 - Dress Codes Practice Attire until Apparel Arrives is Black on Black, White Bow.

PRACTICE TOPS - will be given out in the fall and should be worn to each practice with black shorts and white

tumbling shoes. If practice clothes are lost or damaged, a new piece will need to be purchased at your own

expense. Girls should wear their hair in a high ponytail with a bow. Participants will keep a neat appearance at

each practice. Black on Black, White Bow is worn at practice till practice attire is given out.

NO JEWELRY/NO FINGERNAILS - Jewelry is not allowed at any practice or events. Fingernails must always be kept

short . NO FAKE FINGERNAILS ALLOWED! You will be asked to remove nail tips if we see them at practice.

Both jewelry and fingernails could cause serious injury to your child or another child.

Competition Dress Code - All athletes must have the proper attire for competitions with them at every event. You

may wear uniform top and warm-up pants before and after competition times. You may wear your warm-up jacket

anytime. Under no circumstances are you allowed to wear jewelry at an event. Wearing jewelry or hair bands on

wrists during the competition performance can get your team disqualified or cause point deductions to the team.

All hair and make-up should be done before entering the event. We want our cheerleaders to represent the gym in a

professional manner.

Acceptance of Dress Codes

Section 4 – Quitting/Being Dismissed Quitting/Being Dismissed - All fees, payments, costs, physical items, deposits, (Warm-up, Uniform, etc.)

unclaimed goods, will be and are forfeited by anyone who quits or is removed from our program. There will be NO

REFUNDS for items issued (competition fees, clothing costs, gym fees, clothing, etc.) to anyone who quits or is

asked to leave the program. Anyone who is dismissed, quits, or injured/sick is still responsible for all debts to be

paid and agrees to turn in all Uniform & Warm Up item as well as pay any/all legal fees surrounding such events.

Quitting/Dismissal Acceptance

I have received a copy of the Genesis Xtreme AllStars Quitting/Dismissal Policy. I hereby accept the policies as

outlined. I understand the Quitting/Dismissal Policy can be altered at any time under the owner’s desecration with

reasonable notice. I understand the AllStar Contract will be enforced and upheld within a court of law I have had

sufficient opportunity to read this entire document. I have read it, understand it, and agree to its full

entirety. I agree to No Refunds of any Tuition or Team Payments past, present & future. I agree to be

bound by the terms and conditions of the Quitting/Dismissal Agreement.


Section 5 – Risk of Injury

Parents need to note that cheerleading is a highly competitive sport that involves risk. Stunting, tumbling, and

jumping could lead to injuries. These injuries include but are not limited to: bruises, pulled and strained muscles

or ligaments, broken bones, dislocations and paralysis. GXA takes every precaution to prevent these injuries from

happening. Unfortunately, we cannot prevent them all. In the event that your child is injured, we will take every

necessary step for your child’s well-being.

Safety is the number one priority of this program. Our gym is a proud member of the USASF which regulates the

safety of AllStar cheerleading. Unsafe behavior is strictly forbidden. If at any time the gym staff feels an

individual’s behavior is jeopardizing the squad in any way, the staff may warn, suspend or remove that individual

from the squad.

If a child becomes injured or sick and cannot fulfill their agreement to compete, their fees will go to fund another

child to replace them during the season and no refunds will be given.

Acceptance of Risk of Injury

I have received a copy of the Genesis Xtreme AllStars Risk of Injury. I hereby accept this contract as a whole and

the policies as outlined. I understand the Risk of Injury Policy can be altered at any time under the owner’s

desecration with reasonable notice. I understand the AllStar Contract will be enforced and upheld within a court of

law I have had sufficient opportunity to read this entire document. I have read it, understand it, and agree

to its full entirety. I agree to be bound by this contracts terms and conditions and understand its fully

enforceable .

Section 6 - Tardiness & Absences

Being on an AllStar squad requires a time commitment. Other team members are counting on your attendance.

The team cannot practice the full routine if one or more persons miss practice. Please plan vacations and other

activities during the scheduled gym breaks. Any member absent for the week of a competition may be removed

from the routine for the upcoming competition. All competition fees for the event will be forfeited to cover the

cost of the replacement.

If you are more than 5 minutes late, you will be required to do extra workouts. If you miss more than 30 minutes of

any part of practice, you will owe $25.

Unexcused Absence Break Down

You are required to be at every competition & practice (scheduled practice or called practice). For every

unexcused practice, you follow the chart above. If you are excessively tardy / absent you will be dismissed from

the team. Please understand that if you miss practice (unexcused), your child may be replaced with an alternate

and he/she will have to earn their spot back on the squad. NO REFUNDS!

Excused absences for practice include

Death in the family, Doctor’s excuse for being ill/seriously injured or Mandatory School Grade. (A normal school

activity, last minute papers, homework, etc……or an out of town family event IS NOT an excused absence.)

*Items that must be filed for a school event absence*

1.) A 2-week notice is required for practice adjustments for a team/s.

2.) Syllabus from class to show it’s a mandatory grade.

3.) An Event Flyer.

4.) Grade Assessment Form from GXA SIGNED by your Teacher & GXA AllStar Director.

**If you are caught lying about missing practice under the pretenses of Mandatory Grade, a fee of $75 will be

assessed to your next tuition bill and your athlete will be suspended for an Event.**

Acceptance of Tardiness & Absences

I have received a copy of the Genesis Xtreme AllStars Tardiness & Absences. I hereby accept this contract as a

whole and the policies as outlined. I understand the AllStar Contract will be enforced and upheld within a court of

law. I understand the Tardiness & Absences Policies can be altered at any time under the owner’s desecration with

reasonable notice. I have had sufficient opportunity to read this entire document. I have read it, understand

it, and agree to its full entirety. I agree to be bound by this contracts terms and conditions and understand

its fully enforceable .

Section 7 - Costs & Fees/Payment Schedule

Choreography / Music - Outside resources are used to create a customized track of music to be used during the

routine. In addition, choreographers are hired to create a unique routine for each team.

Apparel – Items of apparel included: Competition Hair Bow, Practice Tops, Bow’s & Make Up. The same Warm

Up & Uniform will be used as the 2019 season.

Fundraising - Fundraising will be done by AllStar Athletes for GXA CHEER. Fundraising will be for the gym,

facilities & upgrading equipment. Athletes will be expected to Raise $200 on each fundraiser. Your Buyout if you

choose not to do Fundraising is $400 for the year. Total Fees - $3,300 Includes Coaches Travel Expenses (Doesn’t include Monthly Tuition / Uniform Pieces)

First installment payment is your Team Deposit & Evaluation Fee.

*Season Payoff made Cash by May 15, 2020 will receive a $300 Discount. Total Fees will be $3,000 per athlete*

All payments are due in full on the scheduled date. Late / Partial payments will owe an additional $25.

AllStar Monthly Tuition: $135 (Due before the first of each month). Monthly fees are for AllStar practices

& 4 hours of tumbling a month. If your child wants to upgrade to 8 hours a month, the cost is $190. AllStar

Tuition begins May. Tuition will upgrade after Evaluations. During the summer, AllStar practices will be held

along with Skills & Conditioning Classes. Practices will focus on Routines elements, jumps, stunting &

Choreography. Specific times are TBA. .

Acceptance of Financial Obligations

I have received a copy of the Genesis Xtreme AllStars Financial Outline. I hereby accept the Financial obligations

outlined above and in this entire contract. I understand the Financial obligations may be altered at any time under

the owner’s desecration with reasonable notice. I understand the AllStar Contract will be enforced and upheld

within a court of law I understand there are NO REFUNDS. I have had sufficient opportunity to read this

entire document. I have read it, understand it, and agree to its full Financial entirety. I agree to No Refunds

of any Tuition or Team Payments past, present & future. I agree to be bound by the terms and conditions of

this Financial Agreement.

Section 8 - General Information

• No GOSSIP about any other team (AllStar or school), another team member, other children, coaches or

any other gym employee in the sitting area, parking lot, competitions, or any other time. You will be

warned on your first offense and then dismissed from our program on the second offense.

• If you have questions or concerns about the team or any other matter, please our communication form.

Do not approach the coach with questions during practices or competitions. The best time to address

concerns is outside of gym hours. The preferred method of communication is through email (no text

messages).

• No arguing or questioning the coaching staff’s decisions at competitions or in the gym.

• Every year each team will go through losses and additions to the roster. The dismissal or addition of an

athlete is at the coach’s desecration and should never be questioned. Anyone threatening to quit or pull

his/her child from the team will be dismissed from the program and will not be welcomed back.

• It is the parent’s responsibility to know the schedule changes and competition schedules. Please check

all emails and Group ME app. for updates to ensure that you are receiving all of the most current

information.

• Parents are to remain in the designated parent seating area. Parents will not come onto the spring floor

for any reason. (Ex. Pictures, Injuries & Video your child.) Parents will refrain from opening the gym

Door to Talk to Athlete or Coach.

• When going to events out of town, each cheerleader must have a chaperone at every competition. It is

not your coach’s responsibility to be your child’s chaperone.

• All teams will watch and support each other as a gym.

• Parents, relatives, friends, and cheerleaders are not allowed to speak with competition officials for any

reason or question the placement of any team. You will be dismissed if you contact anyone from any

competition hosting company.

• Genesis Xtreme AllStars does not allow production of products representing our name. This means that

under no circumstances should you make, sell, or distribute items of any sort with “GXA”, “Genesis

Xtreme AllStars”, or any other variance of the name. Anyone participating in such a practice will be

asked to leave the program or expected to PAY ROYALTIES!

• All cheerleaders and Genesis Xtreme AllStar families are always expected to show good

sportsmanship. Sportsmanship is more than just being polite. Having good sportsmanship is letting

your actions and skills speak for you. Every team member, parent, and coach are asked to represent this

program with PRIDE.

• The AllStar Director may change, add or subtract any rule at any time.

Acceptance of General Information

I have received a copy of the Genesis Xtreme AllStars General Information. . I understand the AllStar Contract

will be enforced and upheld within a court of law. I understand the General Information Policies can be altered at

any time under the owner’s desecration with reasonable notice. I have had sufficient opportunity to read this

entire document. I have read it, understand it, and agree to its full entirety. I agree to be bound by this

contracts terms and conditions and understand its fully enforceable .

GXA CHEER Authorization & Agreement Form

Video and Photography Release: I understand that my child’s photograph/video may be taken during class instruction or

during a special event. I hereby grant my permission for the resulting video and/or photograph to be used for any and all

publicity and printing purposes.

Notice of Termination: I understand that GXCC LLC (Genesis Xtreme Cheer Company) requires all fees, payments, cost,

physical items, deposit (Warm-up, Uniform, Practice Clothing, etc.) unclaimed goods, will be and are forfeited and all items

returned that were received by anyone who quits or is removed from our program. There are and will be NO REFUNDS for

items or payments issued to anyone who is dismissed, quits, or injured/sick. Examples of these items but are not limited to

listed items (Monthly Tuition, AllStar Fees, Uniform, Warm-ups, Competition Fees, Clothing of any kind, etc.) Client agrees

in full entirety they are responsible for any and all debts to be paid in full and agree to turn in all items. Client is solely

responsible and agrees to pay any and all legal/attorney fees surrounding GXA CHEER, in the event this is taken to court.


Enrollment Fee: Upon entering the Genesis Xtreme Tumbling Program, an enrollment fee of $75 per participant or $100 per

family is due at the time of enrollment. To receive the family rate, all siblings must enroll at the same time. If enrollment is

done at separate times, the rate of $75 applies to each participant. This fee is not prorated at any time.

Annual Registration Fee: An annual registration fee of $40/ per student or $70 per family fee is charged every January.

Additionally, if you leave the program and return during the same year, you will be charged the enrollment fee of $75 per

participant.


Payment Options: Payment will be made by credit card draft or PayPal. A service fee is charged with all credit

card/PayPal transactions. Any/All Credit Cards numbers are to be kept current and any changes will need to be submitted so

they can be charged before the 1st of the month or a $25 late fee is applied. A late fee of $25 will be applied to all accounts

Medical Release: I understand that in any activity the potential exists for injury, minimal to catastrophic. GXCC LLC and

its employees, agents, officers and directors shall not be responsible for any injuries; minimal to catastrophic or any damages

associated with participation in any activity, exhibition, competition, recital or clinic or travel to or from any event in which

the above named are involved. I hereby release GENESIS XTREME CHEER COMPANY LLC

(GENESIS XTREME ALLSTARS) staff to render first aid in the event of any injury or illness, to seek medical assistance if

deemed necessary and to transport to a medical facility or to call an ambulance.


Acceptance of GXA CHEER AllStar Contract

I have received a copy of the Genesis Xtreme AllStars Contact. I hereby accept the all policies as outlined. I

understand the AllStar Contract will be enforced and upheld within a court of law. The Contract can be altered at

talented youth are trained from 6 to 18 years of age on fundamentals of cheerleading,

tumbling and fitness. This letter briefly provides a further understanding of our company and what your sponsorship will mean to this athlete.

Established in 2009, Genesis Xtreme AllStars is Suwanee, GA’s most distinguished AllStar Cheerleading & Training Facility. GXA CHEER takes great pride in teaching athletes’

discipline, determination, self-motivation, confidence, respect for others, teamwork,

sportsmanship and perseverance. We administer the teachings above through coaching, mentoring and being actively involved with each child. GXA CHEER also promotes healthy

development of each athlete physically, mentally, emotionally, and morally through athletics

and training.

AllStar Cheerleading is one of the fastest growing & competitive sports throughout our

country, and the world. National Competitions are regularly shown on television networks such as ESPN, ESPN2, CBS SPORTS and FOX SPORTS. As you can imagine, fundraising

is necessary in order to assist our cheerleaders with their competition expenses. These young

athlete’s work diligently to raise money through various methods as well as their families, however they only cover a small portion of their expenses.

Now, we invite you to please help this young athlete, who presented this Packet to you, by sponsoring them. Your sponsorship will go towards competition expenses and tuition. As a

sponsor, your business will enable these cheerleaders to participate in this once-in-a-lifetime

activity. Show your commitment to the future of these athletes as well as the community. Donations can be written off as advertising expenses. Our sponsors will be listed on our

website, practice shirts, advertised in our gym and shared with our members via email.
