Work related to shifting of utilities like Gas Pipe lines, Water supply pipe lines, Sewage pipelines, Suwali Canal, Electrical lines, Electrical Towers, etc. are practically nit, mainly in Hazira area. Though Concessionaire has given a list of works, they have not provided any time-lines for completion of these works. Details of the coordination they had if any with the utility service providers and follow up meetings are not indicated after many reminder and letter from IE. 3. During our site visit, we observed that in Hazira Industrial area concessionaire is not serious for the issue of safety. IE has repeatedly requested to provide proper attention for safety issues to the road users for following locations. Dear Sir

The comments of the IE on the MPR - 63 for the month of May, 2015 submitted by the Concessionaire are

submitted herewith:

1. As per S curve, the actual progress on the revised cumulative amount of work is shown as Rs. 1672.67 Crore against target cumulative amount of Rs. 2017.00Crores by end of December 2013. S Curve does not show any financial progress beyond December 2013. However we see that progress achieved is just about 82.93 % of that programmed and the progress is too slow or more or less stalled. Concessionaire has restarted the works from 15th of December 2013 in section-B but IE observed that the progresses of works in section-A is stalled and the works in section-Bare too slow. The gap between the cumulative planned and cumulative achieved is continuously increasing which confirms that the progress of work is abnormally slow.

2. Work related to shifting of utilities like Gas Pipe lines, Water supply pipe lines, Sewage pipelines, Suwali Canal, Electrical lines, Electrical Towers, etc. are practically nit, mainly in Hazira area. Though Concessionaire has given a list of works, they have not provided any time-lines for completion of these works. Details of the coordination they had if any with the utility service providers and follow up meetings are not indicated after many reminder and letter from IE.

3. Concessionaire has to make continuously more afford to complete the work for shifting of utilities, cutting of trees and other works like taking possession acquired land, etc.

4. The work progress between Km 111 +000 to Km 125+000 including two ROB I Flyover are totally stalled in Hazira Industrial Area. except part of lchhapore FOB @ Ch 115+600.

5. Concessionaire has yet to start the construction of service road, covered drain, Guard railing, etc between Km 111 + 000 to Km 125+000.

6. During our site visit, we observed that in Hazira Industrial area concessionaire is not serious for the issue of safety. IE has repeatedly requested to provide proper attention for safety issues to the road users for following locations. Many complaints are received by NHAI for traffic jam and safety issues.

a) Deep vertical cut made since last two years along balance approach work on Hazira side of ROB cum Fly Over Bridge near lchhapore Junction for the safety. IE had repeatedly requested in our inspection note to complete this work and make it safe for traffic.

b) Unauthorized median openings along the project highways from Km: 0+000 to 131+500 should be closed in view of safety of traffic.

c) Diversion provided between Ch 116.400 to 116.780 is too narrow and unsafe and not as per required standard, is making traffic jam during the pick hours

7. Material Testing Report:-a) As per the requirement of the specifications, the total numbers test results of Absolute

Viscosity test of bitumen (VG-30) up to month of May 2015 do not tally with the required test frequency of the quantity/lots (1086 nos.) in lsolux section mentioned in the MPR of May 2015.

b) Any test result of ductility test of bitumen (VG-30) is not mentioned in the MPR till date. c) Upto last month Dec-2014 358 Nos of pass lot of steel testing conducted but without

conducting any testing of steel (lot) in January/ February/March/April/May 2015 month, increasing one no of test(lot i.e. 359 nos) in January/February/March/April/May 2015 month MPR (lsolux section).

d) As per your MAY 2015 MPR in ISOLUX Section WMM Material Consumed at site 445.4 Cu-m and shown Combined Flakiness & Elongation Test 01 nos. in Table at Page-177 but nos of test carried out are not as per section 900 of MORT& H specification.

e) As per your MAY 2015 MPR in ISOLUX Section DBM Material Consumed at site 417.0 Cu-m (5560 Sq-m) and shown Combined Flakiness & Elongation Test 03 nos, AIV Test 03 nos. and even Marshal Stability Test nil in Table at Page-179 but nos of test carried out are not as per section 900 of MORT& H specification

f) As per your MAY 2015 MPR in ISOLUX Section BC-I Material Consumed at site 138.0 Cu-m (2760 Sq-m) and shown Combined Flakiness & Elongation Test 01 nos., AIV Test 01 nos. Marshal Stability Test 01 nos. and Individual & Mix gradation found 01 nos. each in Table at Page-179 but nos of test carried out are not as per section 900 of MORT& H specification.

g) As per your MAY 2015 MPR in ISOLUX Section DBM & BC-I Material Consumed at site 417.0 Cu-m (5560 Sq-m) and 138.0 Cu-m (2760 Sq-m) respectively but Prime coat and Tack coat test shown nil but as per MORT& H table no-900-4 of cl.-903.4 it shall be 01 test per 500 Sq-m and not less than two test per day.

8. Physical progress status (Completed/in progress) of Culverts (HPC/BQX/SLC) as per page no (8) of Page no-(72 to 75) is not found realistic as per site condition in MPR May 2015. Many numbers of culverts (HPC& Box) are still not completed (please refer IE letter no-5244).

9. Length of DBM & BC shown in physical progress sheet is not matching with actual length available on site (Refer page-7).

10. The PUP at Km. 71 + 231 (BHS) was shown completed in physical progress at page no. 07 of your May 15 MPR but actually the PUP is not completed at site and sub structure work are in progress (Soma section).

11. After passing of nearly 5 years, the work related to project facilities like rest area, way side amenities, traffic aid post, water harvesting etc. as per CA, are yet to take up.

12. Work of Bardoli Bypass was started long before but no remarkable progress observed. It is observed that almost all land has been taken into possession in Bardoli Bypass long back but no any progress of work noticed though the work related to C & G has been already completed long back. This work has been started but progress is very slow.

13. ROB for right side service road near Sachin at Km 95+906 is not yet started. Concessionaire has submitted the drawings for RHS service road by diverting the same over the existing ROB in middle which was not agreed by IE. However the same drawing was submitted to Railways for their approval and as far as records show, the same is not yet approved. Though concessionaire has submitted the GAD to Railways, approval by Railways is not yet received. Though the Concessionaire agreed to construct the ROB on RHS for service road, till date concessionaire did not make any study or submit any drawings for the same. IE has pointed out thisissuemany times since over a year but looking at the situation, IE opines that the project will be delayed due to the inaction of the Concessionaire on this issue. IE advises that the Concessionaire must follow up to accelerate the issue of construction ROB (RHS) for service road at Sachin flyover seeking NHAl's help to coordinate with the Railway. Railway also expressed their views that ROB shall be provided north of 6 lane Highway of NH-6.

14. Shifting of Electrical line I Poles are not completed from ROW of Project Highway at following locations.

16. 3 NCR of Soma section and 10 NCR of lsolux section are yet to be closed without more delay by complying with the comments communicated by the IE.

17. Work for the construction of major structures like Flyover Bridges and Major Bridge (Km: 70+334) are stalled at many locations and progress is abnormally slow without any acceptable and valid reasons.

18. Work of ROB near village Gandhinagar at Km 3+895 has been more or less stalled since last three years. This work has been visit started.

19. Work of service road at many locations particularly at Ch 15+901 , 18+396 near Minor Bridge on both side and between Ch 15+387 to 20+252 are yet to complete.

20. Concessionaire had not taken possession of land which has been already acquired since more than 2 years at many locations, particularly in Hazira Industrial Area.

21 . The work of one Flyover Bridge at Ch 120+300 near Reliance Industries Ltd and three VUPs between Sachin and Magdalla in section-A as a change of scope item are yet to start. IE had repeatedly requested to start the above wo~ediately.

22. The encroachments on the land of project highway are increasing day to day. No result oriented action is being taken by concessionaire to remove the encroachments for the land of project highway of NH-6, IE had repeatedly requested for the same, but concessionaire had not taken proper action till now.

23. The work for developing the major and minor junctions is not taken up for more or less all the locations on project highway as per Clause no. 3.1 of IRC -SP-41 -1994 standard with approach roads of different villages joining project highway but the same are not as per approved drawing and with proper gradient.

24. The work for shifting of Suwali canal and shifting of water supply pipe line of GWSSB is stalled since long.

25. The work for the avenue plantation was just started in section-A but IE observed that the same avenue plantation work has been stopped and at present the same are not available on site. The drawings of avenue plantation for section-Bare not yet submitted for review to IE.

26. Concessionaire has to yet repaired I replaced at many locations for the destination boards, cautionary boards, Sign boards etc on project highway for the safety to vehicular traffic at some desired locations.

27. Boundary pillars showing the demarcation of ROW are yet to installed for whole length of project highway of NH-6.

28. Providing the Kilometer & Hectometer stones are nearly at completion stage but the same are not provided on edge of side shoulders with proper foundations and platform for the stability of stone and these stones are without written any signs/letters in Soma section (section-B).

29. The stone pitching works over the high embankment of road for protection of embankment slope are not carried out as per specification of MORT&H. at many locations in both the section (A&B).The slope of the high embankment of the road is not protected by providing pitching or turning as per provision available in Schedule-0 of CA. The damaged slopes of high embankment are not yet rectified.

30. Safety consultant of NHAI had pointed out time to time observations against the deficiencies in safety to traffic for our project Highway NH-6. Concessionaire has submitted the compliance and same has been reviewed by IE after due inspection of site and no any remarkable progress was observed.

31. We observed that the covered drain has been damaged, lids of drains are broken/collapsed at most of the locations and required to be repaired/rectified immediately. Repairing work is in progress but same is very slow and yet to complete.

32. Kutcha earthen drains are to be prepared along the road for easy and smooth discharge of storm water.

33. The cross drainage structures are not provided for the approach road joining the project highway at minor junctions for discharge of storm water through approach road.

34. The work of providing the service road near Ena bypass is yet to complete and the service road near Palsana town is yet to start.

35. Progress of both Toll Plaza work is stalled and the building work for administration offices including is stalled since long. The work of Weigh Bridge is just started.

36. No median openings are provided between Km 111+000 to Km 116+000 (5 Km) hence contra flow of traffic was observed on right side of road.

37. More safety measures are to be providing between Sachin Magdalla lchhapore Junction stretches to minimize the road accidents.

38. The works at flyover at Km. 1+174and Km. 69+068 (Bardoli) has been started but progress is very slow.

39. The approach works of VUP at Km. 74+820 and Km. 75+682 are stalled since more than since long two and half year.

40. Approach works of Flyover at Km. 115+900 (lchhapore) it is just started and approach works of flyover at Km. 118+584 (KRIBHCO) is yet to start as forest clearance has been already received.

41 . Proposed Railway crossing near lchhapore ROB & KRIBHCO ROB is to be fina lized for the continuation of service road near both ROB on both sides.

42. Unauthorized median openings along the project highways from Km. 0+000 to 131+500 should be closed in view of safety of traffic.

43. For Package-A, IE observed that they had not their own sufficient machineries, equipment & manpower etc. on site. At present no work is in p~s since one month.
