ROSACEA - The different faces
Rosacea is a common chronic inflammatory dermatosis that primarily affects the central zones of the face. About 5-10 % of adults have rosacea, but only one in ten receives treatment. With stage-appropriate therapy matched to the clinical differences of appearance, such dermatosis can be successfully treated.
The primary diagnostic criteria of rosacea are transient as well as persistent erythema, inflammatory papules and pustules, and telangiectasia. Secondary features that may occur with or independently of the primary criteria include phymatoid growths, burning or stinging of the skin, erythematous plaques, dry scaly facial skin, edema, and peripherially-located ocular issues.
Although many of these symptoms may overlap and / or occur simultaneously, a simplified classification of four subtypes can be made:
1 | Erythrotelangiectatic rosacea, 2 | Papulopustular rosacea, 3 | Phymatous or glandular hyperplastic rosacea, 4 | Ocular rosacea.

ROSACEA - Skin in danger
Rosacea affects more than 3 million Canadians.1
By the age of 70 over 70 % of women have some thread veins.2
Rosacea is a disease which develops differently in each patient. Within Rosacea, there are 4 degrees of severity. However, as the symptoms, appearance and intensity of the disease can vary from person to person, it is not always possible to clearly define the degree of severity.

TYPE I: Erythrotelangiectatic rosacea
Spider VeinsSkin Redness Thread Veins
Erythematotelangiectatic rosacea, the most common type of Rosacea, is characterized by increasing and persistent redness, as well as the development of visible thread and spider veins.
DIAGNOSTICSTransient erythema (flush) or persistent erythema. Increased incendence of telangiectasia (mainly on cheek and nasolabial areas).
PUBLISHED TREATMENT STRATEGY:COUPEROSE THREAD & SPIDER VEINS BALMApplication study Couperose, paragraph 16EE3558, carried out in 2017 on behalf of Swissforce Pharma AG.After just 7 days of use, the clinical efficacy of the product is evident. Sendur; Acta Derm Venereol 2012; 92; 73-77“Isotretinoin is a therapeutic option in the treatment of rosacea. At a dosage of 20 mg / day, isotretinoin effects a rapid reduction in inflammatory lesions and erythematous rosacea.”
Topical and systemic therapies in the treatment of rosaceaAimee M. Two, Wiggin Wu, Richard L. Gallo, Tissa R. Hata; JAAD 72 (5): 761-770, 2015“Several studies have shown that oral isotretinone is an effective treatment for serious, persistent PPR.”

Eichenfield, MD; Linda Stein-Gold, MD; Diane Berson, MD; Andrea Zaenglein, MD; Cutis 93:18-28, 2014“The available data indicates that isotretinoin (0.3 mg / kg daily) represents an effective and well-tolerated option for the treatment of papulopustular rosacea and early phymatous rosacea.”
TYPE II: Papulopustular rosacea
Papulopustular rosacea is characterized by persistent redness with transient papules and pustules. This subtype is often confused with acne, which has the effect of many people suffering from rosacea for years, whilst being unaware of what they actually have.
DIAGNOSTICSSingle or multiple, inflammatory, reddened, sometimes flaky papules often persist over several weeks. Sterile papules or pustules colonized by normal skin flora. As a rule, scarless healing, though there is a frequential increase of relapses as the disease progresses.

Phymatous rosacea is characterized by skin thickening, which leads to irregular growths on the face, in particular an enlargement of the nose due to excess tissue. It can also affect the cheeks, ears or chin.
DIAGNOSTICSSignifi cant, infl ammatory nodules and pimples mainly present in men with diffuse connective tissue and seba-ceous hyperplasia, especially on the nose (rhinophyma, or so-called “nodular nose”).
PUBLISHED TREATMENT STRATEGIES:Use of oral isotretinoin in the management of rosaceaHyunhee Park, DO; James Q. Del Rosso, Do, FAOCD; Clinical Aesthetic Dermatology; September 2011, Volume 4, Number 9“Only the administration of oral isotretinoin at the dose of 0.3 mg / kg / day showed a statistically significant superiority over placebos. At the end of the study, 24 % of patients treated with isotretinoin had regression. Satisfaction was rated as “excellent” by 32.6 % of patients. Based on these results, the treatment of PPR and phymatous rosacea with isotretinoin appears to be a successful alternative to orally administered antibiotics.”
Systemic isotretinoin in the treatment of rosacea – doxycycline – and placebo-controlled, randomized clinical studyH. Gollnick, U. Blume-Peytavi, E. Laszlo Szabó, K.G. Meyer, P. Hauptmann, G. Popp, M. Sebastian, T. Zwingers, C. Willers, R. von der Weth; JDDG; 2010 • 8:505–514“Isotretinoin 0.3 mg / kg was found to be the most effective dose with significant superiority over both placebos and doxycycline, in addition to a reduction in lesions of 90 % compared to 83 % with doxycycline. Investigators diagnosed a complete response in 24 % of cases and a significant improvement in another 57 % of patients treated with isotretinoin. Isotretinoin is an effective and well tolerated treatment option for the rosacea subtypes II and III.”

TYPE IV: Ocular rosacea
PUBLISHED TREATMENT STRATEGY:State of the art: Systemic rosacea managementM. Schaller, H. Schöfer, B. Homey, U. Gieler, P. Lehmann, TA. Luger, T. Ruzicka, M. Steinhoff; DDG, JDDG 1610-0379, 2016, 14 (Suppl. 6) 29-37“Patients with persistent ocular symptoms require long-term systemic treatment, which usually involves the administration of tetracycline. The intake of low-dose isotretinoin (10 mg / day) also has a beneficial effect on patients with ocular involvement.”
Ocular rosacea is characterized by ocular issues such as dry eyes, tears and burning, swollen eyelids, recurrent styosis, and potential loss of sight due to corneal damage.
DIAGNOSTICSInfl ammatory ocular symptoms: Blepharitis, conjunctivitis, iritis, iridocyclitis, hypopyon iritis, keratitis with aversion to light, pain and risk of blindness.

Perceived effectiveness and overall assessment
In evaluation of subjects who presented with visible spider veins, thread veins and redness linked to couperose of the face, the efficacy of treatment with Couperose Thread & Spider Veins Balm after 7, 14 and 28 days of twice-daily use was demonstrated by:
81 % of the subjects having a reduction in skin redness. 77 % observed a lessening of the intensity of visible blood vessels.
After 28 days of twice daily facial usage of Couperose Thread & Spider Veins Balm, efficacy was demonstrated by:
91 % of the study participants seeing a decrease in skin redness. 82 % observing a lower intensity of visible blood vessels. 95 % of the study participants felt satisfied with the product and would like to continue to use it.
PROVEN RESULTS Studie 16E3558
Evaluation of the effectiveness of Couperose Thread & Spider Veins Balm when used twice daily at 7, 14 and 28 days, in 22 study participants (average age: 54 +/- 2 years) with visible veins and redness associated with couperose (telangiectasia):
Appearence of the skin: Analysis of veins, capillaries and redness of the skin according to a clinical score. Opinions of efficacy as well as overall impressions: Patients participating in the study filled out a subjective
questionnaire. Visual representation: Macrophotography of the face using the VISIA CAS system. Micro-circulation of the skin: Anaysis of the concentration of red blood cells using TiVi 600.
Skin appearance
After only 7 days of use, Couperose Thread & Spider Veins Balm presents a significant efficacy: red skin is reduced, veins are less visible and skin is more unified.
