Eligibility Requirements (Pg. 3)-2013 Wellness Program Enrollment Form (Deadline November 15th 2012)

-New Hire (Hired before May 1st 2013 must complete “All” requirements, however those hired after May 1st 2013 will be expected to do as much as they can if signed up for the program).

 -Health Assessment (Must be completed April 1st – May 1st)

-Health Assessments will be accepted NO LATER than May 1st

-On-line or Paper form -On-line instructions

-www.1stopwellness.net-Click on “SEWS” main tab-Click on “Health Assessment”-Password is “wiredup”

 -Blood Work (Deadline June 1st)

-Done on site May 1st – June 1st, 2013-(Any blood work completed and submitted to One Stop

Wellness January 1st - June 1st, 2013 will be accepted for the Wellness Program).

“Wired Up” Quarterly Agenda1st Quarter (Pg. 5&6)

-Launching of 1st 10 Week On-going Health Management Program “How Low Can You Go”

-Jan 21strd – Mar 25th

- BMI “Lowdown”… This program will focus on lowering your BMI to a healthy range. You will get a baseline BMI and try to see how many points you can lower your number. There will be a competitive component pinning site locations against one another to see who can go the lowest. Good Luck!! (FYI, we’ll take the total points of BMI dropped for the group and divide it by the number of people to be fair)

sign up to receive information to help you lower your glucose number. You will receive credit for maintaining a healthy glucose number (<100) or lowering it by 7%

week. For added points answer the question of the week to fill in your crossword puzzle. Turn in a completed crossword puzzle for a final drawing. Put the puzzle pieces together and achieve your fitness goals.
