This leaflet explains a little bit about how these services can support you. Your midwife can make a referral for you. You can also get in touch directly. Health Visiting Health Visiting Health visitor teams provide health advice and support to all parents from late pregnancy until your child goes to school. Every preschool family has a health visitor linked to their local GP surgery. All families are contacted before the birth and soon afterwards. You can also have an antenatal contact, new birth visit, 3-4 month review, 1 year review and a 2 year review. Child health clinics are held locally so you can just drop in or contact us for an appointment. To contact your health visitor look in your personal child Health Record (Red Book) or ask at your GP surgery or children centre. Midwifery Midwifery The midwifery service provides care and support throughout pregnancy, birth and immediately after the birth of your baby. Women ‘book’ with the midwife before 12 weeks of pregnancy. You can do this via your local GP or children’s centre. The midwife helps you plan your pregnancy pathway and where necessary will make contact with other services. Your Local Children’s Centre Children’s centres off er services to give children the best start in life and help them reach their potential. Services in centres across the county include:

• Health and parenting support • Information about training and employment • Access to play activities and childcare • Support for children, parents and carers with additional needs • Support for young parents • Crèche facilities • Access to toy libraries

Careers South West (CSW)Careers South West (CSW) CSW support pregnant young women, dads and partners aged 13-19 (up to 25 if you have special educational needs) to access job and learning opportunities.

• Independent and impartial information

• Practical support• Advice and guidance

There are lots of other services available for mums and dads. These can offer support around your specifi c needs or they might offer something extra in your local area. A goodstarting point is:

Young Mums Will AchieveYoung Mums Will AchieveFor young mums to be and mums aged between 14 and 19 years old. The groups meet twice a week to provide support, guidance and learning in a relaxed environment. The programme runs for a whole academic year but you can join at any time.

Family Nurse Partnership Family Nurse Partnership (FNP)(FNP)A programme for fi rst time mothers aged 19 and under. In order to have a family nurse you need to be referred, or make a referral before you are 26 weeks pregnant. This is because the programme is designed to prepare mums and their partners for parenthood as well as provide support once baby arrives.

• Weekly or fortnightly home visits• Support from pregnancy until your

baby is two• Supportive relationship
