To inform the students who ask for the service of Registration Offi cerecognizing about the procedures and methods so as to quickly receive service. HISTORICAL BACKGROUND

Registration Office, Chiang Mai University, was established on

June 10, 1989.

IMPORTANT DATES AND EVENTS 1964 - Sub division under the General Affair Division, the President Office

1966 - An Academic Service Division under the President Office

1986 - Registration Office Establishment Project

1987 - Launched the Registration Office Establishment Project

1989 - Registration Office was approved in the gazette page 9

No.106 Section no. 93 on June 9, 1989

1995 - Chiang Mai University’s sectors of responsibility were divided

according to the University Administration announcement (No.3)

of 1995, which established the Computer Science Department

within the Faculty of Engineering, and divided sectors of

responsibility in the Registration

Office. The Secretariat of the Cabinet also announced this in the

gazette No.112, special section 50 D, on December 29, 1995.

July 4, 2008, as announced in a Chiang Mai University announcement. There are

To request a new student ID card: 1. Contact Registration Counter Service, Registration Office 2. In case of a student ID card being lost or not returned by an ATM, police notification is required. In case a student ID card is expired, damaged or requires change in details, such card must be submitted Submit a request form and a police notification or the existing student ID card to the Registration Counter Service staff person 3. The staff person will sign and stamp the form with a university logo 4. Take the form to a Siam Commercial Bank for the issuance of a new student ID card at the SCB, Faculty of Medicine branch, Chiang Mai University or Chiang Mai University branch

2.5 COURSE UNITS TRANSFER

There are 2 types of course unit transfers: 1. Course units transferred from Chiang Mai University 2. Course units transferred from other universities

1. Course units transferred from Chiang Mai University Course units transferred from Chiang Mai University enable Chiang Mai undergraduate students who have left CMU to re-enroll and transfer back course units taken within 5 years, according to the guidelines of the respective faculties, departments or specialization streams. The transferred grades are recorded in “CX”.

Procedures: 1. Fill out request form available at www.reg.cmu.ac.th 2. Submit the form with approval of the academic advisors and the following documents: 1) Transcript (original) 2) Chiang Mai University announcement for resignation of student or Chiang Mai University command or transcript record (in case of retirement from the grades) 3) Transfer fee payment receipt 4) If the course units transfer is approved, students are required to withdraw such courses from the enrolled list

The course units transfer is according to the Chiang Mai University announcement of August 26, 2011.

2. Course units transferred from other universities Courses units transferred from other universities enable a Chiang Mai University undergraduate student who passed the entrance examination or was chosen to attend Chiang Mai University to transfer course units to their new study program in Chiang Mai University. The transferred grades are recorded in “CX”.

Procedures: 1. Fill out request form available at www.reg.cmu.ac.th 2. Submit the form with approval from the academic advisors to the respective faculty within the first 2 weeks of the first academic year. The following documents are required: 1) Transcript (original) 2) Details of course description 3) Transfer fee payment receipt 3. If the course units transfer is approved, students are required to withdraw such courses from the enrolled list

The course units transfer is according to the Chiang Mai University announcement of August 26, 2011.

2.6 COURSE UNITS TRANSFER FROM NON-FORMAL EDUCATION TO CHIANG MAI UNIVERSITY

Additional requirements may be applied for some courses. As indicated in the respective faculty announcement, supplemental are submitted to the faculty/department before proceeding to the Registration Office

For a student to receive their degree, they must carefully follow the procedures. 1. Fill out the information about expectation to graduate via the Internet at www3.reg.cmu.ac.th/reg-expect on the specified date and check the correction of the data. 2. Present the completed graduation form for fee payment. For undergraduate students, please contact the Siam Commercial Bank. For graduate students, please contact at the cashier of the Registration Office. 3. Present the completed graduation form and receipt to take a photo at the Registration Office. 4. To complete the process for claiming degrees, a student should inform the Registration Office, Educational Services Section at the faculty and advisor within the set dates and time given by the Registration Office. 5. Any student who claims the degree but cannot graduate, must repeat this procedure in the semester expecting to graduate. Processing fees are charged only for the certificate of academic achievement and official transcript.

6. Dress code for computer photo taking 6.1 For undergraduate students should wear the student uniform, no hat, no glasses and no ornaments any kind. Neat hairstyle that does not cover the eyes. No highlights or coloring. Male students are not allowed to have long hair. 6.2 For graduate students should wear a suit or solid color shirt of any color expect white and wear no ornaments or glasses. 7. Fees for the graduation 7.1 Degree registration fee, 1,500 Baht (no recharge for the students who have filed for the expectation and paid for it before) 7.2 Certificate of academic achievement and official transcript (to be sent by post) (1 set = 100 Bath, 2 sets = 160 Bath, 3 sets = 220 Bath) 7.3 The faculty that has to request for a license for medical practice and a licence for professional practice required 3 sets of the certificate of academic achievement. 8. Students expecting to graduate for undergraduate level (student code 50…onward) who have P, I grades on records and/or not registered for courses in semester they expect to graduate, have to enroll for university services. 9. Graduation Approval for undergraduate level by University Council three times per year: the end of first semester, the end of second semester and the end of summer session. 10. A commencement ceremony is held once a year. The graduate will be informed of the exact date by post mail and online at Registration Office website.
