Respiratory Therapists can be the lead in creating“High Reliability Organizations (HROs)

Preventable medical errors result in the loss of 200,000 lives peryear. Widespread preventable patient harm occurs despite increasesin healthcare regulations. High reliability organization theory

contributes to improved safety and may potentially reverse this trend.

High Reliability Organizations are organizationsthat conduct operations with minimal error,over an extended time, and consistently makedecisions that result in high quality and highreliability. HRO theory is interdisciplinary innature, and has been a part of programs toimprove safety in military, commercial aviation,public safety, and nuclear power organizations.Much of the early HRO literature originatedfrom within the firefighting industry. Safetyconcerns grew after several unnecessary deathsoccurred during two major wild-land fires.Principles stemming from HRO theory are alsoincreasingly used in healthcare. The structureof HRO centers on several basic principlesthat together promote a culture of safety.These structures include (a) developing andmaintaining standard processes;(b) implementing checks and redundancy tomitigate potential failure; (c) deferring toindividuals with the most information; and,(d) developing teams that openly communicateabout failure to prevent recurrence of unsafeincidents. An organization that is reliability-seeking is preoccupied with failure and activelyworks toward building and enhancing a cultureof safety. HRO principles are gaining ground inhealthcare. HRO influences the rate of patientsafety indicators (PSIs) in an organization,

human error attributable to fatigue and stress,and development of interdepartmental teamswith open channels of communication.Healthcare is unique compared to these otherindustries. In healthcare adverse events canhappen frequently and may affect a singlepatient rather than a large group. Healthcareorganizations need a structure to support safetyprocesses, and safety must be an organizationalgoal. An HRO requires a management-ledorganizational climate committed to safety,where management fosters a safety culture inwhich employees may internalize safetyinitiatives and contribute individually to thesafety climate.

Respiratory Therapists are a perfect fit forleading patient safety initiatives. We have beenusing clinical practice guidelines for decades,are able to communicate with all disciplinesand work in teams, and are used to workingwith redundant checks. Just add some HROeducation and principle training and we canbe a positive influence in decreasing patientharm and leading others on the patient safetyjourney. With the General Assembly session having ended, there

is a specific process that will be followed through the

summer for any legislation that has been approved.

From the date of final passage the General Assembly has 30 days

to verify that each bill met all constitutional requirements and

submit them to the Governor for his consideration (June 30).

From that day he receives any bill the Governor has 60 days

to act. Note: If the Governor takes no action a bill becomes law

automatically, unlike the President who has a “pocket veto.”

Since the adoption of the 1970 Illinois Constitution this has

only occurred once so it is highly unlikely that it will occur.

Senate Bill 2332 - Raises the age for whom tobacco products,

electronic cigarettes, and alternative nicotine products may

be sold from at least 18 years of age to at least 21 years of age.

Eliminates the penalty for possession of a cigar, cigarette,

smokeless tobacco, or tobacco in any of its forms by persons

under 18 years of age. Defines “electronic cigarette.”

Status: Sent To Governor

Senate Bill 2439 - Amends various acts to remove provisions

allowing or requiring licensing authorities to revoke professional

licenses for defaulting on an educational loan provided by

or guaranteed by the Illinois Student Assistance Commission.

Status: Sent To Governor

Senate Bill 2514 - Amends the Smoke Free Illinois Act.

Changes references from “fine” to “civil penalty” throughout

the Act. Provides that law enforcement agencies (rather than

local law enforcement agencies) shall enforce the provisions

of the Act through the issuance of citations and may assess

specified civil penalties. Provides that funds designated for the

Department of State Police or Department of Natural Resources

by provisions concerning the distribution of civil penalty

moneys shall be deposited by the respective Department into

a specified fund.

Status: Sent To Governor

Still No Word From Governor on “Tobacco 21”

Senate Bill 2332, legislation that raises the statewide age for

buying tobacco and other related products, has been sent

to the Governor for his consideration. The deadline for his

consideration is August 28.

It is important that as much support as possible for these

proposals be sent to the Governor’s Office for inclusion in the

bill file is being compiled in order for the Governor to consider

all aspects of the legislation. Letters of support should be sent

as soon as possible to Governor Bruce Rauner, 207 State House,

Springfield, Illinois 62706. Remember to put the bill number

(Senate Bill 2332) and a short note of support.

Raising the age for tobacco and related products has been under

discussion at the state and local level for a long time. In the

absence of state law to raise the age many municipalities have

undertaken the discussion and some, most notably Chicago,

have taken the lead and have raised the age. In fact, Five states

and 14 Illinois cities and counties, including Chicago, already

have adopted local ordinances setting the age at 21.

Changes are Coming to NBRC’sContinuing Competency Program

The NBRC’s Continuing Competency Program purpose is guided by two principles; toenhance and maintain competence as time passes, and to require participants in theprogram to provide evidence they continue to meet current competency expectations.

After more than 15 years, changes are coming to the Continuing Competency Program and thename is changing to the Credential Maintenance Program (CMP) to better represent the purposeof the program. As the new CMP emerges, the two original principles will continue as our guidingstatements for the goal of the program modifications. The new program is intended to strengthenthe relationship between competencies of credential holders and the evolving competencyexpectations linked to those credentials. Therapists, technologists, and specialists who haveachieved an NBRC credential since July 1, 2002 participate in the continuing competencyprogram. Currently, a practitioner has three options to document that he or she remains competent:

• Achieving another NBRC credential• Documenting 30 continuing education credits earned within the last five years• Passing the credentialing examination again

These program requirements will remain, but an annual competency assessment will be integratedover the next few years. These assessments may be completed online from a mobile device or apersonal computer and may be completed in one session or multiple sessions.
