I drove to Salzburg from Prague, arriving right before dinner. I am really glad to have been able to come despite the COVID-related restrictions. After an introductory toast, we were welcomed and also informed about the anti-epidemic measures that have been put in place. Everyone of has been tested negative for COVID, masks and hand hygiene are compulsory, and air purifiers have been installed everywhere. After this short talk, we had a lovely introductory dinner, and we had the possibility to informally meet with everyone. Our amazing cohort is made of people at different carrier levels, from very different country and it has been amazing to hear everybody’s experience. Monday, October 19, 2020 After a very much needed good night’s sleep, we took the pre-test, which I thought was quite difficult! It highlighted my need to deepen my knowledge in some areas, which hopefully I will be able to do in the next few days. In the morning, we had lectures from top Austrian lecturers, which were able to come in person. Since the beginning of 2020, it has been a while that I had been able to enjoy a face-to-face lecture and this has been done in both a safe and interactive way. I think the other members in my cohort have been refreshed by this as well, as the lectures inspired a lot of questions. After lunch, we met with our American lecturers, which unfortunately had to be streamed via Zoom.

We braved the physical distance and time difference and mad it into a very enjoyable experience. While the human contact was missing, it has been amazing to interact with lecturers from Weil Cornell hospital in New York. In the evening, we went for a walk to Salzburg castle, and enjoyed some breath taking views of the Alps. Tuesday, October 20, 2020 In the morning, we were able to enjoy a lecture from algesiology on the topic of invasive chronic pain management. While it is something that is done in my hospital, I am not trained to do these procedures and I was inspired to learn more about them. Maybe pain medicine is the way to go! We continued with our case presentations, and I was in the first group of presenters – while presenting in front of a crowd of specialists is always tough, everybody made it very comfortable, and we were able to exchange views on different tough cases. In the afternoon, we participated in the international BJA webinar on pain management. This is probably the only advantage of this pandemic. We got used to streaming and web meetings, and international seminars have never been easier to attend. We continued with more cases and had dinner. After dinner, we went to explore the nightlife in Salzburg (only until 10pm, when everything has to close), and we were able to enjoy a drink outdoors, social distancing, at a terrace. Wednesday, October 21, 2020 We were met with Prof. Manhofer from Vienna, who is a world-leading specialist in regional anaesthesia. We were able to get some practical tips from his practice, which I hope to implement in mine as soon as I come home! Some f the fellows’ cases were very inspiring as well and we learned a lot from each other. In the afternoon, we had a Zoom with our lecturer from New York and talked about some universally applicable topics, such as fluid management and lung ultrasound, but also about a niche topic – anaesthesia in robotic cardiac surgery. I have no experience in cardiac anaesthesia, let alone with robotic cardiac surgery, so it felt very distant from what I normally do, but it was very interesting to see the intricacies of preparation and management of such cases. Who knows? Maybe one day, I will be able to apply this knowledge in practice. Thursday, October 22, 2020 Prof. Manhofer greeted us again in the morning with a lecture on the use of ultrasound in anaesthesia. It is useful to go back to the basics sometimes! More cases were presented by fellows and we were inspired by each other’s practice again. In the afternoon, our New York lecturers introduced us to how they manage cardiac and vascular surgery patients in a large centre with years of expertise. It was followed by a lecture on ARDS and respiratory critical care, which is an extremely relevant topic to all of our practice at the moment. Hearing the American perspective on things made me want to read some more on the topic. After dinner, we were able to socialize a bit more with the group and get a good night of sleep. Friday, October 23, 2020 This is the last day. We only had two more lectures. We had a lecture on obstetric anaesthesia, which was very relevant to my practice, and a lecture about the Austrian experience with the pandemic, which has been particularly enlightening. We took the post-seminar test, which I felt quite confident about – definitely more that at the beginning of the course, so I guess I did learn something!

While my experience in Salzburg has been tainted by a pandemic, I believe that I was able to have worthwhile educational and social experience while keeping as safe as possible in these difficult times. For that, I want to thank all the organising team, the lecturers and the staff of Schloss Arenberg to make this event possible.
