This packet contains information students should read before applying for a scholarship administered by

the Lake Park High School Educational Foundation. Students will find information about the various

scholarships available and the specific requirements for each scholarship.

Following is a brief description of the Foundation-administered scholarships.

Altier Scholarship – $500 will be awarded to four graduating students planning to pursue a two-year

degree program at the College of DuPage, another community college, or attend an accredited trade

school. (Pages 5-6)

Amanda Jane Dombrowski Memorial Scholarship - $1000 will be awarded to a graduating student

planning to attend an accredited trade school, technical institute, community college, or a four-year

college/university program leading to a certificate or degree; who is a current member of the Lake Park

High School girls’ soccer team and displayed leadership and sportsmanship while serving as a positive

role model for younger players; and who displayed a passion and commitment to the LPHS soccer

program. (Pages 7-8)

Barclay Family Scholarship - $1000 will be awarded to a graduating student planning to attend a four-

year accredited college/university, community college, technical institute, or an accredited trade school.

Chris Frizzelle Memorial Scholarship - $500 will be awarded to two graduating students who are

currently members of the boys’ soccer team; who overcame or are attempting to overcome some type

of challenge; and who plan to attend an accredited trade school, technical institute, community college,

or a four-year college/university program leading to a certificate or degree. (Pages 11-12)

Daniel Christophe, Jr. Memorial Scholarship - $1000 will be awarded to a graduating student who is a

current member of the Lake Park High School Marching Band, Student Council, Class Council, S.A.D.D.,

National Honors Society, or Snowball (planner or attendant) and who plans to attend a four-year

college/university program leading to a degree. (Pages 13-14)

Don Lovelace Memorial Scholarship - $1000 will be awarded to a graduating student planning to attend

an accredited four-year college/university program leading to a certificate or degree and major in

health, driver education, physical education, or related fields. (Pages 15-16)

DMG Perseverance Scholarship - $500 will be awarded to a graduating student. (Pages 17-18)

Dr. Gary Annunziata Scholarship - $500 will be awarded to two graduating students planning to attend

the College of DuPage. (Pages 19-20)

G. H. Elliott Memorial Scholarship – $500 will be awarded to a graduating student who has been

accepted as a full-time student in an industrial technology or vocational degree or certificate program at

an accredited school. (Pages 21-22)

Italian-American Club Scholarship - $250 will be awarded to two graduating students planning to attend

an accredited trade school, technical institute, community college, or four-year college/university in a

program leading to a certificate or degree. (Pages 23-24)

Itasca Lions Club Scholarship - $500 will be awarded to a graduating community service-oriented

student from Itasca who is planning to attend an accredited four-year college/university or community

college and who preferably desires to pursue a field of study that would help him or her foster a more

humanitarian world. (Pages 25-26)

Joseph Cychosz Scholarship - $1000 will be awarded to a graduating student planning to attend an

accredited college/university program leading to a certificate or degree, who demonstrates financial

need, and who plans to major in education, public service, engineering, or science. (Pages 27-28)

Lake Park Foundation EXCEL Scholarship - $1000 will be awarded to four graduating students who

demonstrated excellence while at Lake Park High School; who plan to enroll in a four-year

college/university or junior/community college; and who participated in athletics, activities, and or

community service activities. (Pages 29-30)

Lake Park Foundation Gene Swierczewski Scholarship - $1000 will be awarded to a graduating student

planning to attend an Illinois public state college or university and major in Languages, English, Political

Science, Sociology, or Communications. (Pages 31-32)

Kevin Neis Scholarship - $1000 will be awarded to two graduating students planning to enroll in an

accredited four-year college/university and participate competitively in cross country and/or track and

field. (Pages 33-34)

Scott Bradley Memorial Scholarship - $1500 will be awarded to a graduating student who was a

member of the Lake Park High School wrestling team that plans to attend an accredited four-year

college/ university program leading to a degree. (Pages 35-36)

***IMPORTANT: STUDENTS INTERESTED IN APPLYING FOR ANY OF THESE SCHOLARSHIPS SHOULD***

1. Read the detailed description of the scholarship(s) they are interested in (see page numbers above) and note the specific requirements for that/those scholarship(s).

2. Complete the Common Scholarship Application – available at www.lphs.org.

3. Answer the specific essay question(s) for the scholarship(s) for which they are applying.

4. For each scholarship being applied for, print a hard copy of (a) the completed Common Scholarship Application, (b) the essay(s) for that scholarship, and (c) the required letter(s) of recommendation for that scholarship. (Note: Students applying for more than one scholarship may use a recommendation letter more than once as long as it meets the requirements of a particular scholarship.)

5. Turn all materials noted in #4 to Ms. Marjorie Frank in the West Campus counseling office. NOTE: Applications will not be considered if incomplete or if the appropriate support materials are not turned in by the identified deadline date for the scholarship for which a student is applying.

Lake Park High School Educational Foundation

The Lake Park Educational Foundation is pleased to announce the availability of the Altier Scholarship to

provide financial assistance to four students who are planning to pursue a two-year degree program at

the College of DuPage, another community college, or attend an accredited trade school.

The Altier Scholarship, established by Noreen Altier, honors Lake Park community residents Donald and

Donny Altier.

To be eligible for the Alter Scholarship, the applicant must:

• be a Lake Park High School graduate of the Class of 2016.

• plan to enroll in a two-year degree program at the College of DuPage, another community college, or attend an accredited trade school.

• have a minimum grade point average of 4.0 on a 5.0 scale.

• complete the Common Scholarship Application.

• submit an unofficial transcript of all high school classes.

• submit a letter of recommendation from a teacher.

submit the essays as noted on the following page.

The completed application and required support material must be submitted to the West Campus

Career Center by no later than Thursday, March 10, 2016. The Altier Scholarship will be awarded to

four selected recipients at Scholarship and Awards Night on Thursday, May 5, 2016.

A $500 check will be processed after the recipient has submitted a copy of a tuition bill showing

enrollment as a full-time student at the College of DuPage. Please send this documentation to the Lake

Park Educational Foundation at 590 S. Medinah Road, Roselle, Illinois 60172. The check will be issued

after July 1, 2016, and the request must be filed no later than June 30, 2017.

(Altier Scholarship)

The following essays should be completed (using a word processor) and a hard copy of your essays along

with a copy of the Common Scholarship Application and required letter of recommendation should be

given to Ms. Marjorie Frank in the West Campus counseling office by Thursday, March 10, 2016. Make

sure your name, student I.D. number, and the name of the scholarship for which you are applying also

appear on your essays and letter of recommendation.

Essay #1

Describe in 1-2 pages (250 words maximum) an obstacle or personal challenge that you faced, how

you overcame this challenge, and what you learned from this experience that will help you in the

future.

Essay #2

Provide a 1-2 page (250 words maximum) biography of yourself, including any special circumstances

that the scholarship committee should be aware of in considering your application and how this

scholarship will help you attain your educational and career goals.

Lake Park High School Educational Foundation

The family of Amanda Jane Dombrowski and the Lake Park Educational Foundation are pleased to

announce the availability of one scholarship in her memory. Amanda graduated from Lake Park High

School in 2001 and later attended the University of Iowa. While at Lake Park, she was an integral

member of the girls’ soccer team. Amanda lived with great passion for her family and friends, which

carried into her life as a teacher, mentor, and caregiver for children. In her memory, Amanda’s family

wishes to award one $1,000 scholarship to a deserving Lake Park High School student.

To be eligible for the Amanda Jane Dombrowski Memorial Scholarship, the applicant must:

be a Lake Park High School graduate of the Class of 2016.

be a current member of the Lake Park High School girls’ soccer team who has displayed leadership and sportsmanship and has served as a positive role model for younger players.

have displayed passion and commitment towards the LPHS soccer program.

plan to attend an accredited trade school, technical institute, community college, or a four-year college/university program leading to a certificate or degree.

have a current cumulative grade point average of 3.5 or higher on a 5.0 scale.

complete the Common Scholarship Application.

submit an unofficial transcript of all high school classes.

submit a letter or recommendation from a teacher.

The completed application and required support material must be submitted to the West Campus

Career Center by no later than Thursday, March 10, 2016. The Amanda Dombrowski Memorial

Scholarship will be awarded to the selected recipient at the Scholarship and Awards Night on Thursday,

May 5, 2016.

It is the responsibility of the scholarship winner to determine that this award from the Lake Park

Educational Foundation will not interfere with NCAA eligibility.

A $1,000 check will be processed after the recipient has submitted a copy of a tuition bill showing

enrollment as a full-time student in an accredited trade school, technical institute, junior college, or a

four-year college/university program leading to a certificate or degree. Please send this information to

Lake Park High School Educational Foundation

The Lake Park Educational Foundation is pleased to announce the availability of a $1,000 Barclay Family

Scholarship to provide financial assistance to a student who is planning to pursue his/her post-

secondary education at a four-year college/university, a community college, a technical institute, or an

accredited trade school.

The Barclay Family Scholarship was established in honor of Barclay family members Kathy (Barclay)

Goedeker (Class of 1971), Andy Barclay (Class of 1974), Betty (Barclay) Bentley (Class of 1975), Kelly

(Barclay) Deterding (Class of 1979), Gavin Barclay (Class of 1980), and Ellen (Barclay) Watkins (Class of

1982), all alumni of Lake Park High School, and Kay (Barclay) Cahill who was a Lake Park High School staff

member for 27 years and currently serves as secretary on the Lake Park High School Educational

Foundation’s Board of Trustees.

To be eligible for the Barclay Family Scholarship, the applicant must:

• be a Lake Park High School graduate of the Class of 2016.

• have a minimum grade point average of 3.50 on a 5.0 scale.

have been actively involved in Lake Park athletics and/or activities for at least 4

semesters.

• complete the Common Scholarship Application.

• submit an unofficial transcript of all high school classes.

• submit a letter of recommendation from a teacher.

• submit an essay as noted on the following page.

The application for this scholarship is available on-line at the Lake Park High School website. The

completed application and required support material must be submitted to the West Campus Career

Center by no later than Thursday, March 10, 2016. The Barclay Family Scholarship will be awarded to

the selected recipient at Scholarship and Awards Night on Thursday, May 5, 2016.

Lake Park High School Educational Foundation

The family of Chris Frizzelle and the Lake Park Educational Foundation are pleased to announce the

availability of a scholarship in his memory. Chris was a valued member of the Lake Park High School

staff for twenty-one years. In addition to working in the Technology Department, he was also an

integral part of the Lake Park soccer team for many years, where he served in a variety of capacities,

including assistant coach and announcer for the boys’ team and head soccer coach and announcer for

the girls’ team. In his memory, his family wishes to award two $500 scholarships to deserving Lake Park

High School students.

To be eligible for the Chris Frizzelle Memorial Scholarship, the applicant must:

be a Lake Park High School graduate of the Class of 2016.

be a current member of the Lake Park High School boys’ soccer team.

have overcome or is attempting to overcome some type of challenge (i.e.: personal, family, or financial).

plan to attend an accredited trade school, technical institute, community college, or a four-year

college/university program leading to a certificate or degree.

Lake Park High School Educational Foundation

The family of Daniel Christophe, Jr. and the Lake Park Educational Foundation are pleased to announce

the availability of one scholarship in his memory. While at Lake Park, Daniel was an integral member of

the Marching Band, Student Council, Class Council, S.A.D.D. and National Honor Society. After

graduating from Lake Park, Daniel attended Northern Illinois University and was a Senator in their

Student Council. While at NIU, he held an intern position at the Price Waterhouse Coopers accounting

firm while pursuing a career as an accountant. He was also an active member of the St. Peter the

Apostle Church youth group. Daniel was tragically killed in a motorcycle accident when he was 21 and a

junior at NIU. In his memory, Daniel’s family wishes to award one $1,000 scholarship to a deserving Lake

Park High School student.

To be eligible for the Daniel Christophe, Jr. Memorial Scholarship, the applicant must:

be a Lake Park High School graduate of the Class of 2016.

be a current member of one of the following: Lake Park High School Marching Band, Student Council, Class Council, S.A.D.D., National Honors Society or Snowball (planner or attendant).

plan to attend a four-year college/university program leading to a degree.

have a current cumulative grade point average of 3.5 or higher on a 5.0 scale.

be in the top 25% of the graduating class.

be a resident of Itasca, Illinois or a parishioner of St. Peter the Apostle Church.

Lake Park High School Educational Foundation

The family of Donald C. Lovelace and the Lake Park Educational Foundation are pleased to announce the

availability of one scholarship in his memory to provide financial assistance to young men and women

who are planning on careers in fields related to health, driver education, or physical education.

Don Lovelace was a member of the Lake Park faculty for twenty-eight years. Don touched many young

lives in his career as a teacher of driver education and as a coach. To further assist young people, a

$1,000 scholarship will be given in his name.

To be eligible for the Donald C. Lovelace Memorial Scholarship, the applicant must:

• be a Lake Park High School graduate of the Class of 2016.

• be in the upper quarter of the Class.

• plan to attend an accredited four-year college/university program leading to a

certificate or degree.

• major in health, driver education, physical education, or related fields.

(Applicants are not limited to those who plan to teach.)

• complete the Common Scholarship Application.

• submit an unofficial transcript of all high school classes.

• submit a letter of recommendation from a teacher.

submit an essay as noted on the following page.

Career Center by no later than Thursday, March 10, 2016. The Donald C. Lovelace Memorial

Scholarship will be awarded to the selected recipient at the Scholarship and Awards Night on Thursday,

May 5, 2016.

Lake Park High School Educational Foundation

The Lake Park Educational Foundation is pleased to announce the availability of the $500 DMG

Perseverance Scholarship to provide financial assistance to a student who is planning to pursue post-

secondary education. The scholarship will be awarded to an individual who has demonstrated

determination in overcoming adversity.

The DMG Perseverance Scholarship was established by former Lake Park staff member Katie Gillies and

her husband, Jonathan Steinhauer, to honor her father, David Gillies. David is a living example of what it

means to persevere through a great deal of difficulty during one’s life. In his life, David has faced several

challenges yet he continues to maintain an ambitious spirit and positive attitude. On April 1, 2005 David

was struck by lightning and left with a traumatic brain injury and physical limitations. Nevertheless,

David continues to inspire others by reaching out to strangers, those in need, and anyone he encounters

with his generous heart and positive outlook.

To be eligible for the DMG Perseverance Scholarship, the applicant must:

• be a Lake Park High School graduate of the Class of 2016.

• have a minimum grade point average of 2.5 on a 5.0 scale.

• complete the Common Scholarship Application.

• submit an unofficial transcript of all high school classes.

• submit a letter of recommendation from a teacher.

submit the essays as noted on the following page.

hospital in Palm Springs, California. Dr. Annunziata established this scholarship to assist two students

planning to attend the College of DuPage, which he attended after graduating from Lake Park High

School. Two $500 scholarships will be awarded to students planning to attend the College of DuPage.

Gaylon H. Elliott was one of the founding faculty members of Lake Park High School and was a

During high school, Kevin competed on the Lake Park track and cross-country teams and continued to compete at the collegiate level at Kalamazoo College, where he earned a B.A. in Economics. Kevin later earned an M.A. in Economics at the University of Illinois at Champaign Urbana and had a successful career in the finance and insurance industry. A $1,000 scholarship will be awarded to two graduating students who have been accepted as full-time students at an accredited four-year college/university and meet scholarship guidelines.

Lake Park High School Educational Foundation

Scott Bradley was a Lake Park student and wrestler whose connection with our school began many years

before, when he wrestled with the Lancer Kids Club as a young child. Scott was a gentleman who

displayed sportsmanship, loyalty, and dedication to the Lake Park wrestling program. He constantly

strived for excellence through hard work on the mat and in the classroom.

Unfortunately, Scott’s wrestling career ended suddenly during his junior year when he was diagnosed

with leukemia. For six months, Scott battled his illness with the same determination that he battled his

opponents on the mat. On May 5, 1986, the Lake Park community suffered a great loss. The following

year his parents, Mr. and Mrs. Olav Bradley, established the Scott Bradley Memorial Scholarship as a

loving tribute to their son. In his memory, his family wishes to award one $1,500 scholarship to a

deserving Lake Park High School student. The student athlete considered for this award must display

similar characteristics to Scott: loyalty, commitment, dedication, and a strong work ethic.

To be eligible for the Scott Bradley Memorial Scholarship, the applicant must:

Career Center by no later than Friday, February 26, 2016. The Scott Bradley Memorial Scholarship will

be awarded to the selected recipient at the Scholarship and Awards Night on Thursday, May 5, 2016.

It is the responsibility of the scholarship winner to determine that this award from the Lake Park

Educational Foundation will not interfere with NCAA eligibility.

A $1,500 check will be processed after the recipient has submitted a copy of a tuition bill showing

enrollment as a full-time student in an accredited four-year college/university program. Please send this

documentation to the Lake Park Educational Foundation at 590 S. Medinah Road, Roselle, Illinois 60172.

The check will be issued after July 1, 2016, and the request must be filed no later than June 30, 2017.

The following essays should be completed (using a word processor) and a hard copy of your essays along

with a copy of the Common Scholarship Application and required letter of recommendation should be

given to Ms. Marjorie Frank in the West Campus counseling office by Friday, February 26, 2016. Make

sure your name, student I.D. number, and the name of the scholarship for which you are applying also

appear on your essays and letter of recommendation.

Describe in 1-2 pages (250 words maximum) how you display the following characteristics:

commitment to the Lake Park wrestling program, leadership, sportsmanship, dedication, and a

strong work ethic.

ESSAY #2

Provide a 1-2 page (250 words maximum) biography of yourself, including how this scholarship

will help you attain your educational and career goals.
