Human Performance Onsite Coach Annual Fee Annual Fee covers placement and management of full-time onsite ADURO-certified Health and Productivity Coach at location of client's choosing. ADURO will hire, train, and place a full-time coaching employee for client, to provide in-person 1:1 and group coaching interactions to client's employees throughout the year. Includes access for coach to back-end coaching dashboard, all required ongoing certifications as outlined and required by ADURO, and inclusive of all salary, benefits and taxes for hired candidate. Assumes maximum 5% annual salary increase based on merit and initial onsite coach training and set-up fee of $15,000.00.

Human Performance Onsite Coach Recruiting / Training / Turnover Fee Onsite Coach set-up fee covers recruiting and screening of candidates, hiring and training of a mutually agreed upon candidate for the position by both ADURO and client, and covers cost of 30-day inclusion training at ADURO's Human Performance Center in Redmond, Washington including all travel and expense associated. Also covers needed equipment and initial certifications for chosen candidate. One-time fee applied upon hiring a chosen candidate and applied at time of both initial placement and of turnover of onsite position.

Physician Fax Form Participant can easily download and take a simple form to their physician to be completed by the physician's office with all pertinent biometric screening stats. 1.  Pricing reflected on this quote is effective for ninety (90) days from date of issuance. 2.  All services are designed as add on/drop off options. Placement on this quote is for

purposes of pricing only, and does not bind client to utilization. 3.  Any non-employee groups (spouses/dependents) are priced at the same employee rates

shown above unless a difference in incentive at stake exists in which case this rate may be negotiated.

4.  Standard ADURO Implementation timeframe is 90 days from the point of agreement dependent upon desired program components.
