The Civil Service Commission promulgated CSC Resolution No. 1500088 dated January 23, 2015 which adopted the attached Revised SALN Form and amendments in the guidelines in the filling out the SALN Form.

The public officials and employees who have filed their SALNs in compliance with CSC Resolution No. 1300173 dated January 24, 2013 prior to the effectivity date of the amendments shall be considered to have substantially complied with the filing of the SALNs for the year 2014. However, public officials and employees who will file their SALNs after March 14, 2015 are mandated to strictly comply with the use of the attached Revised SALN Form.

In connection to this, the names listed below who have filed their SALNs but due to amendments on the form, we opt to return in compliance with the aforementioned statement, to wit: 1. TeresitaCorrea-RCC III

2. Marie Peachy Reyes-RCC III

We are requesting you to facilitate the dissemination of the old SALN Forms of your staff.

Furthermore, those who have not yet filed their SALNs, the Revised SALN Form of 2015 is required to use.
