Select your preferred Salutation from the drop-down list

SAGE Required Fields

Fields with a red asterisk (*) are required. In SAGE-speak, you and your organization are an “Agency”

SAGE Title – Executive Director

In the Title field, enter “Executive Director”

SAGE Vendor Number

Vendor Number – not required at this point You will need it to receive funding if you are awarded a grant. • Click “Obtain a Vendor Number” • Download, complete, and submit the form.

SAGE Mailing Address

Agency Address – Number and Street in top field, PO Box (if applicable) in “Agency Address continued” field City and State – mailing address Zip Code – must have Zip + four, click hyperlink, if needed

SAGE Location – County

Select the County where your agency is located* first.

Then, select the Municipality where your agency is located.*

SAGE Initial Agency Type

Select any one of these Initial Agency Types from the drop-down list.

SAGE Federal Employer ID Number

If you do not have a Federal Employer ID Number, enter nine zeroes (000000000) in this field.

SAGE Access Requested for RFP/Program

Access Requested for RFP/Program – Select “Landlord Incentive Program” from the drop-down list.

SAGE Federal Congressional District

To find your Federal Congressional District, go to— https://www.federalreporting.gov/federalreporting/congressionalDistricts.do

SAGE DUNS Number

If you don’t have a DUNS Number, click the hyperlink, follow the instructions provided, and enter the number in the field.

SAGE Revenue Questions

If you answer “No” to questions 1A and/or 1B, answer “No” to question 2.

SAGE Save and Submit

When you have completed the form, click “Save”

Registering to use SAGE

You will be brought back to the login screen and a message in the Login box will say that your registration form is being reviewed. A Username and Password will be emailed to you.

Login to SAGE • Enter your Username and Password • You will be instructed to change your password

immediately • Copy the password from your email and Paste it into the

“Enter your old password” field • Type your new password into the two remaining fields

and click Save

Agency Information Update • DCA will require more extensive information from you

before you can Submit a grant application • On your Home Page, click the “Click here to create an

updated Agency Information Update” hyperlink • Complete and Submit the form • Click the Help icon (top right of screen) for instructions

Apply for a Grant

Select “Landlord Incentive Program” from the drop-down list. Click on the “Apply for a New Grant” button.

You may Logout or go to your Start Menu (Home Page) and return to work on the application later.

Getting Back to the Application • Login to SAGE • At your Home Page, under Application In Process,

click on the application number 2014-02326-000# • You will be taken back to the application screen
