Please follow the directions in this packet. A completed packet must be turned in on or
before 3:00 p.m. on Tuesday, September 22, 2015. The completed packets are to be
placed in the box labeled NHS in Room A211.

DIRECTIONS
Your application must include the following to be considered for NHS:
1. Student Activity Information Form
Please read all directions on the form carefully.
Be sure to obtain all required signatures.
2. Special Person Letter
You must include this letter.
Please be sure the address of your special person is accurate and complete.
Please write legibly.
3. Three Recommendations Required
The people you select must receive both the cover AND recommendation
sheet.
Two recommendations from teachers at RHS who know you especially well.
Many teachers are asked to write these. It cannot be stressed enough that you
need to plan ahead and ask teachers to do this in a timely manner.
One recommendation from someone outside of school who knows you especially
well. Family member recommendations are not acceptable.
The recommendations must be received by Ann Froemming by Tuesday,
September 22, 2015.
It is your responsibility to follow up with the person recommending you on the
status of your recommendations to make sure that they have been completed and
received.

Write a 1-2 page revised and edited typed essay (Times New Roman, 12 pt, double
space) in which you address both of the following:
1) How do you want to be remembered when you graduate from Rogers High School?
2) What kind of legacy would you like to leave?

Co-curricular Activities.
List all activities in which you participated in at Rogers High School. Do not include
classes such as concert band, etc. for which you would receive a grade. Include clubs,
teams, musical groups, etc., and major accomplishments of each. The advisor or adult
in charge must verify that you were involved with their signature and not just their
initials.

Service Activities/Community Volunteer Service.
List service activities in which you participate. These can be service projects done with
a group either in or out of school, or done as individual projects performing service.
Generally speaking, service activities are those done for or on behalf of others (not
including family members) for which no compensation (monetary or otherwise) has
been given.
It is an expectation that candidates for NHS will have taken advantage of a variety of
volunteer opportunities.
A verifying signature of an adult involved (and not just initials) must be included. It
cannot be a family member who signs this section.

Rogers High School
Somewhere in your career as a student, a special teacher, coach, or other adult has had an extra
special influence on your life. We would like to recognize that person by inviting them to your
induction ceremony at 7:00 PM on Monday, October 12, 2015, in the Rogers High School
Commons.
Please identify your special person by giving their name and address (or if an Elk River School
District teacher, the school where they currently teach) and attach a letter written to your special
person explaining why you selected them.
Please include the letter and this form along with your NHS application. Completion is required
for induction process.
An invitation will be mailed to your special person along with the letter you wrote inviting them
to the ceremony
