RMNCH+A SERVICES- SKILL ASSESSMENT WORKSHOPS CONDUCTED AT KSIHFW, THIRUVANANTHAPURAM WITH THE SUPPORT OF NRHM STATE OFFICE, KERALA. REPORT INTRODUCTION. Competence encompasses knowledge, skill, abilities and traits. And it is a major determinant of provider performance and quality of healthcare outcomes. Competency is gained in healthcare profession through pre service education, in-service training, and work experience. Conventional educational and training practices has been focusing more on knowledge aspects and thus leaving a huge gap between the knowledge and skill of the students and trainees. The data released by MOHF&W on IMR and service data from HMIS also indicate that although services are being provided as per the numbers indicated in the HMIS but the same is not translated into proportionate decrease in outcome indicators like IMR, MMR, etc. The recommendations of expert group such as CRM & the JRM teams also recommend that the skills during ANC, INC & PNC and also for ENBC, resuscitation, IUCD, needs improvement. It was in order to address these issues that GoI has suggested a need based competency enhancement of nursing staff across the country with the objective of strengthening their competency levels and thus improve their knowledge and skills. In understanding the importance of competency enhancement of the its healthcare providers, Department of Health and Family Welfare has taken urgent step in streamlining the plan of action. A state level planning meeting was conducted on 15 December 2013, at Conference Hall, KSIHFW in order to lay down the roadmap for this. The meeting was attended by Dr. P.K Jameela, DHS, Dr. K.V. RMNCH+A SERVICES- SKILL ASSESSMENT WORKSHOPS CONDUCTED AT KSIHFW,

REPORT

INTRODUCTION.

Competence encompasses knowledge, skill, abilities and traits. And it is a major determinant of provider

performance and quality of healthcare outcomes. Competency is gained in healthcare profession

through pre service education, in-service training, and work experience. Conventional educational and

training practices has been focusing more on knowledge aspects and thus leaving a huge gap between

the knowledge and skill of the students and trainees. The data released by MOHF&W on IMR and

service data from HMIS also indicate that although services are being provided as per the

numbers indicated in the HMIS but the same is not translated into proportionate decrease in

outcome indicators like IMR, MMR, etc. The recommendations of expert group such as CRM &

the JRM teams also recommend that the skills during ANC, INC & PNC and also for ENBC,

resuscitation, IUCD, needs improvement. It was in order to address these issues that GoI has

suggested a need based competency enhancement of nursing staff across the country with the

objective of strengthening their competency levels and thus improve their knowledge and skills.

In understanding the importance of competency enhancement of the its healthcare providers,

Department of Health and Family Welfare has taken urgent step in streamlining the plan of

action. A state level planning meeting was conducted on 15 December 2013, at Conference Hall,

addressed the group and stressed the importance of providing quality of RMNCH+A Services at each

and every delivery points. For this skill assessment and competency development of service providers

especially nurses is an important strategy. Dr. Beena K.V, SPM, RCH presented in detail about the

concept of Skills Lab Training. Dr. Ramesh, Principal, KSIHFW presented the details on setting up of a

Skills Lab. Smt. Sobhana M.G, Senior Consultant, SHSRC presented the operational guidelines for skill

assessment of nurses for strengthening competency of Health care providers on RMNCH+A services.

The major decisions of the meeting are as follows.

Setting up of proposed skill lab at KSIHFW attached to W&C Hospital Thycaud,

Thiruvananthapuram.

Principal KSIHFW is designated as the state level nodal officer for setting up of skill lab,

coordinate the TOT for district level skill assessment and RMNCH+A skill lab training.

Skill assessment of all nurses working in delivery points across the state to be assessed in

a time bound manner during January to April 2014.

Four to Five TOT to be conducted in January 2014 for developing the district skill

assessment team

The composition of the district team and the time schedule for the TOTs identified.

The Skill Assessment team shall consists of

RCHO-1

DPM-1

Gynaecologist -1,

Paediatrician 1

JAMO-1

MO preferably trained in NSSK/ IMNCI-1

DNO-1

Tutors from Nursing College/School- 2

Staff Nurse with PG in OBG-2

Following the TOT the team has to conduct the skill assessment of the nurses from

delivery points. Time line for the activities also detailed in the annexure.

Based on the skill assessment the nurses will be given competency based training

RMNCH+A for six days as per Govt of India guidelines.

Decisions also taken to ensure the placement of trained Staff Nurses in the RMNCH+A

service delivery areas and not to shift them for two years after training

Skill assessment of those ANMs who are working in delivery points of high focussed

districts eg. Tribal hospital also to be done. It will be done in the second phase.

The state focus person will be SPM (RCH) and Principal KSIHFW.

Planning of District Level Orientation of Assessment Team.

Being the state nodal officer of Setting Up of Skills Lab and Orientation training of District Level

Assessment Team, The Principal KSIHFW has convened a planning meeting at the Principals Chamber.

The other participants of the meeting were Dr. Sanal Kumar. J.G, Assistant Director, KSIHFW, Smt.

Shobhana. M.G, Dy Director Nursing (Senior Consultant, Nursing, SHSRC), Smt. Jona.V. Consultant R.O

CTI, Mr. Bitty Thomas, Consultant R.O CTI, and Mr. Shafeek, J, Technical Assistant, CTI. The meeting

finalised the dates and schedule of the two day orientation Programme. Dr. Sanal Kumar and Smt.

Chandralatha were asked to coordinate the training and Smt. Jona, Mr. Bitty and Mr. Shafeek were

asked to assist them in conducting the training.

The Conduct of the Training

The orientation of District Level Team for Assessment Team for strengthening the competency of Health

Care Providers in RMNCH+A Services was conducted at Conference Hall, KSIHFW between 16/10/2014-

19/02/2014. The first two days workshop was attended by participants from Thiruvananthapuram,

Kollam and Pathanamthitta. The Inaugural meeting began by the registration of participants at 9.30 AM

on 16/01/2014. The training was inaugurated by Dr. P.K Jameela, DHS, Kerala who in her inaugural

address stressed the importance of Competency based Training in Kerala scenario and urged the

participants to implement the programme with utmost enthusiasm. Dr. M. Beena IAS , State Mission

Director, NRHM explained the objectives of the workshop.

The five orientation programmes (2 days each) conducted from 16/01/2014 to 19/02/2014 were

attended by 145 participants from different districts. During the 2 days programme the district teams

have also prepared the tentative action plan for the competency assessment of staff nurses working in

delivery points of their concerned districts. The trainings were conducted according to the following

schedule.

Monitoring of the Skill Assessment .

As a support for districts in skill assessment, state level observers are designated in order to monitor and

evaluate the implementation of the assessment. The principal suggested the state level monitors and it

is sent as a note to the DHS who will direct the person concerned to monitor the implementation of

Data Collection and Planning of Competency Based Training.

The Summated scores of the knowledge and skill assessment is to be collected from districts and the

data must be analysed. Based on the analysis further trainings must be planned.

District Level Activities

As per the guidelines of Govt of India and guidance given to the district level teams at the state level

orientation workshops, district level was prepared and all the staff nurses working at delivery points

were invited for a skill assessment of 5 basic skills. Total numbers of 3043 staff nurses were assessed

and the assessment score of the each skill they underwent are detailed below.

Conclusion.

From the Skill Assessment of the staff nurses working in delivery points in all the

districts, it can be inferred that they require basic skills training for improving the quality of

RMNCH+A services. It is suggested to have full fledged skill based training to the staff nurses on

RMNCH+ A Services.
