1 Introduction

1.1 Purpose of the Plan

The purpose of this document is to establish a system for the City of Palmerston to inspect and identify the risks associated with roads, pathways and stormwater drainage infrastructure so that Council will have minimized the potential insurance liabilities and better outcome on community safety.

2 Inspection Regime

For roads: the assessment of the risk is determined by the size of the various modes of failure/defects and the hierarchy of the location.

For pathways: the assessment of the risk is determined by the size of the trip hazard, slipperiness, unevenness and the location of the defect in terms of its asset hierarchy classification. The hierarchy itself will be determined by the function, types of users and user number.

For stormwater drainage: the assessment of the risk is determined by the potential risk and the location of the asset.

The inspections are undertaken using specifically designed data collection sheets, which can be used either in hard copy or on hand held data collection devices (iPad).

3 System Procedure

The Technical Services Department of the City of Palmerston (CoP) is to carry out the risk based inspection service for Council infrastructures under its control. The service will adhere and conduct inspections in accordance with this Manual and other relevant standards and statutory requirements.

The officer conducting inspections and/or administering the inspection system and issuing and managing of work, must be suitably qualified as determined by the Civil Operations Team Leader.

The Civil Operations Team Leader will be responsible for:

• Programming routine maintenance inspection • Appointing an appropriate inspector and assistant • Receiving inspection reports • Directing urgent responses to make conditions safe • Programming repairs • Having regard in the budget and other resources availability

4 Inspection Methodology

4.1 Inspection Team

The responsibility for the inspection is to be appointed to one person defined as ‘The Inspector’. Any second person appointed shall be the Assistant. All inspection personnel shall wear an appropriate PPE and as a minimum a safety vest.

4.2 Inspection Procedure

4.2.1 Roads:

Road inspections are carried out by observations from a car. Vehicle inspection may require two members of staff, with the passenger/observer defined as ‘The Inspector’ and the driver as the Assistant.

Inspections carried out on foot may generally only require one person, designated as ‘The Inspector’.

4.2.2 Pathways:

Pathway inspections shall be carried out on foot. A motorised cart may be used if approved by the Civil Operations Team Leader

4.2.3 Storm water:

Storm water inspections are split into 2 categories.

Category A – within the road reserve

These inspections may be combined with the road and pathways inspection. It is imperative that Open Unlined Drain (OUD) inspections are carried out with two members of staff.

It may be possible to inspect Drainage pits as a one person operation. Drainage pits should be visually inspected from above to determine that the pit is free from debris and operating.

OUD’s should always be inspected by two people. During these inspections a visual inspection of the OUD, should determine whether any obstructions are present or are likely to become present in the near future.

Category B – within Public Open Spaces

Inspections within Public Open Spaces are best undertaken on foot and by a two person team. They should follow the same procedure as for inspections of storm water assets within the road reserve.

4.2.4 Night Inspections

Night inspections may be carried out and are to be recorded as “Night Inspection”.

All vehicle inspections (Day or Night) are to be carried out at a speed so as to be able to see all defects or hazards.

5 Required Actions

5.1 Background

When a defect or condition is recorded, it is categorised according to the urgency with which repair works shall be carried out. The action to be taken is categorised as follows: -

5.1.1 Category 1 (Safety)

This category is reserved for defects that are likely to represent a danger to users. Defects recorded as a Category 1, require either temporary or permanent action to be taken urgently. Actions to be taken will include, for example, warning sign or highlighting the conditions. The temporary actions require follow up permanent treatment. The response time for such defects is indicated in Appendix A.

5.1.2 Category 2 (Monitor)

This category is reserved for defects that are unlikely to represent a significant hazard to users. Such defects require ongoing observation to ensure that the condition does not escalate to category 1. The defects in category 2 are monitored and, if the budget permits, will be actioned within the time frame mentioned in Appendix A.

5.1.3 Category 3 (No Action)

This category is used for safe and fit for purpose conditions. There will be no action for such conditions under the risk based inspection. If Category 3 conditions escalate to Category 2 or 1, then they shall be treated as such.

The above conditions are determined by visual observation and measurement where required.

When customers report a condition which may be likely to represent a hazard to users, then that shall be considered to be a category 1 condition and the response time shall be allocated accordingly.

All condition reports shall be forwarded to the Civil Operations Team Leader immediately following each day’s inspections. This will avoid the delays in attending to the urgent issues.

5.2 Condition Response

All Category 1 Conditions require urgent attention in accordance with the action response time shown in Appendix A. Immediate actions include, but not limited to

• Identifying the conditions on site using an obvious visual indicator such as spray paint,

crayon, flagging, safety hats or other appropriate means • Protecting the condition by fencing, traffic control devices or redirection of traffic and/or

pedestrians • Making the condition safe by installing traffic management measures or undertaking

temporary/permanent repairs • Job completion: all temporary safety measures have now been removed and permanent

measures implemented. All safety signs have been removed and the works are completely cleaned up. No further field actions required.

• Follow up: Customer has been advised that the work is complete. All reports have been signed off and sent off to the records.

All ‘Category 1’ defects shall be notified to the Civil Operations Team Leader as soon as is practicable. This notification is recorded as an ‘Immediate’ action. Extremely hazardous defects e.g. missing manhole cover, shall not be left by the Inspector who must notify the Civil Operations Team Leader and either wait until emergency help arrives, or where appropriate, sign and barricade the defect to make safe.

Staff who attend Road Traffic Incidents, shall record all relevant details of the incidents.

When carrying out inspections, the Inspector shall highlight and advise of matters which will need some form of additional investigations such as a structural visual inspection.

5.3 Weekends and holidays Category 1 works

If Category 1 conditions are found the day before any weekend or Public holiday, an immediate action is required and the Civil Operations Team Leader shall ensure that the condition is made safe and not left until after the holiday period.

6 Condition Definitions and Treatments

6.1 Background An Inspector shall provide a suitable description of the location for every item recorded, in such detail so as to ensure all other parties involved can location its position. Due to the limited size of the ‘Location Description’ field, any abbreviation used must be readily understood. Every effort is to be made to assign the appropriate treatments to the recorded conditions.

7 Site Risk Analysis

Attention to the safe and efficient movement of traffic around or through a work site will enhance employee safety. It should be noted that failure to correctly apply the principles of Traffic Management could result in death or serious injury to YOU, the employee, and/or other staff and road users.

In addition to recording a defect, the inspector shall also identify the Site Risk Rating by including the following information:

• The City of Palmerston road hierarchy shown on page 7; and • The site Risk Rating matrix shown below.

The Inspector may nominate a generic traffic management plan from the City of Palmerston Work Site Traffic Management booklet to assist the planning of the works.
