Summary
SUMMARY
Carnitine (P-hydroxy-y-trimethylammonium butyrate) was discovered as a quantitatively important
compound in muscle tissue in 1905. Throughout nature it is found in high concentrations principally
in meat and milk products, the major supplier being red meat. Carnitine is also present in plasma. Its
physiological concentrations in human tissues are:
i) red meat: 1-4 mM;
ii) heart 0.6-1.3 mM;
iii) milk 0.05 mM;
iv) liver 0.5-1 mM;
v) kidney : 0.3-0.6 mM;
vi) brain : 0.5-1 mM;
vii) plasma : 0.004-0.007 mM.
Carnitine is present only in a low amount in plant and vegetables.
It has two main functions in mammals. Firstly, it is essential for maximal mitochondrial p-oxidation
of long-chain fatty acids. Free carnitine is transported from plasma into the cytosol by an
unidentified transporter, and consequently enters the intermembrane space of mitochondria by a
mechanism which probably does not require an active process, since the outer mitochondrial
membrane is leaky. In a reaction catalyzed by carnitine palmitoyltransferase I (CPT I), the
respective carnitine derivates of long-chain acyl-CoA's are formed. These long-chain acylcarnitines
are transported across the inner mitochondrial membrane by the carnitine/acylcarnitine translocase.
In the mitochondrial matrix, the long-chain acylcarnitine is reconverted to the respective long-chain
acyl-CoA derivative by CPT II which is located in the inner mitochondrial membrane. At this point,
the long-chain fatty acid may undergo P-oxidation.
Secondly, the formation of acylcarnitines from the respective acyl-CoA's is important to maintain
the cellular pool of free CoA. This carnitine function is called the "buffer function". Short-chain
acyl-CoA's can be converted to the respective acylcarnitines by the action of carnitine
acetyltransferases (CAT). Since the cellular CoA pool is generally smaller than that of carnitine, the
latter can be regarded as a sink for acylgroups. Interestingly, the reaction catalyzed by CAT liberates
CoASH which is essential for a number ofmetabolic pathways (e.g., p-oxidation, and Krebs cycle).
Unless acylcarnitines are reconverted to their respective CoA derivatives (which can be de novo
metabolized), excess molecules are excreted.
In the meal worm Tenebrio molitor, carnitine is essential for its life, therefore, carnitine's old name
vitamin Bt was postulated. In mammals, carnitine is provided by food and by biosynthesis.
Carnitine biosynthesis comprises six steps with six different enzymes. Most animal tissues are able
to perform carnitine biosynthesis up to butyrobetaine (the direct precursor of carnitine), but the last
enzyme in the synthesis pathway (y-butyrobetaine hydroxylase) is only found in liver and kidney.
Carnitine from the liver (originated either by biosynthesis or by absorption from food) can be
released in the plasma and is distributed to the other organs in the body. Since the carnitine plasma

concentration is typically 10 to 100 times lower than that in tissues, carnitine has to be transported
actively from the plasma mto various tissues
In the last 20 years carnitine deficiency has been reported in an array of pathological states The
primary carnitine deficiency is known to impair the renal carnitine reabsorption and/or to present
very low levels of carnitine m muscles The secondary carnitine deficiencies are disparate m
genetically determined metabolic errors they range from defects m enzymes of the p-oxidation, to
defects in proteins involved in the carnitine biosynthesis cycle, from branched-chain ammo acid
disorders to mitochondrial disorders Secondary deficiencies are also accounted for a number of
acquired medical conditions such as liver cirrhosis, extreme prematunty, and Fancom's syndrome
All these illnesses evidence that the carnitine level in tissues and plasma must be under tight
control Therefore, it would be of extreme interest to determine the characteristics of the
plasmalemmal carnitine earner (or transporter) of kidney, mainly to discover which is the defect in
patients with primary carnitine deficiencies Moreover, its structural determination may permit a
better merapeutical approach to all the secondary deficiencies affecting glomerular reabsorption
In the first part of mis work we tried to express and charactenze this unknown high-affinity
carnitine transport system from rat kidney in Xenopus laevis oocytes These studies revealed an
endogenous, Na+-dependent carnitine transport system m Xenopus laevis oocytes with a Km value of
66 pM In comparison, the Km value of the expressed carnitine transport was 149 uM Apart from
the differences m die Km values, the two systems could be distinguished from their different pH
dependencies and inhibition pattern by structural analogues (Berardi et al, 1995) The next step was
to size-fractionate mRNA from kidney cortex of rat and to construct a cDNA library in Escherichia
coh Size-fractionation on a sucrose gradient resulted in an ennchment by a factor of three of the
starting signal, and a cDNA library was successfully constructed Since the major problem appeared
to be the endogenous transport of carnitine in oocytes, it was extremely difficult to clearly identify
positive clones Unfortunately, we could not find a substance which specifically inhibited the
endogenous carnitine transport
In the second part we investigated several alternative strategies First, we assessed different cultured
cells in order to establish a cell line which possibly did not transport carnitine Almost all cell lines
tested transported carnitine only CHO cells did not transport carnitine Na+-dependently We tned to
establish the conditions in order to use this cell lme for transient expression, but the high Na+-
mdependent carnitine uptake rendered these attempts vain Furthermore, we worked on supposed
homologies between the plasma membrane carnitine earner and the known GABA/betaine-,
GABA- and serotonin earners First, the sequences of these three earners were searched for
homologies in the DNA data bank, then oligonucleotides with the highest consesus were derived.
These oligonucleotides were used as primers for cDNA screening m several tissues and cells Using
the reverse transcriptase PCR technique it was possible to detect different positive signals Only the
amplified product obtained with rat liver mRNA could be investigated Due to a lack of time, it has
not been possible to investigate the omer positive signals
In the last part of this Ph D work, the carnitine transport in rat skeletal muscles was investigated
using isolated sarcolemmal membrane vesicles The vesicle preparation was of relatively high
purity, since the sarcolemmal markers were enriched In order to determine L-camihne uptake m

these vesicles, the sidedness of the latter was tested. About two thirds of the vesicles isolated had
the right orientation. The L-carnitine uptake into sarcolemmal vesicles was stimulated if a Na+
gradient [Na+]out>[ Na+]j„ and a K+ gradient [K+]j„>[ K+]out were present. Besides this, carnitine
uptake was saturable with an apparent Km of 13 pM. This transport was inhibited in a
concentration-dependent manner by several carnitine antagonists. In comparison, the carnitine
uptake into rat renal brush-border membrane vesicles was inhibited by completely different
compounds, suggesting that the muscle carnitine carrier is different from that of kidney.

La carnitina e stata individuata come una componente importante nei tessuti muscolari gia nei 1905.
In natura appare in grandi concentrazioni principalmente nella carne e nei prodotti caseari: la came
rossa e il maggior fornitore, ma la si ritrova presente anche nei plasma. Le sue concentrazioni
fisiologiche nei vari tessuti umani sono:
i) carne rossa : 1-4 mM;
ii) cuore : 0.6-1.3 mM;
iii) latte : 0.05 mM;
iv) fegato : 0.5-1 mM;
v) reni : 0.3-0.6 mM;
vi) cervello : 0.5-1 mM;
vii) plasma 0.004-0.007 mM.
Nelle piante e nei legumi la carnitina e stata rilevata solamente in piccole quantita.
Ha due principali funzioni nei mammiferi. In primo luogo, e essenziale per una completa P-
ossidazione mitocondriale degli acidi grassi a catena lunga. La carnitina libera e trasportata dal
plasma nello spazio intermembranale dei mitocondri da un trasportatore ancora non identificato, e
acilata dagli acil-CoA a catena lunga in una reazione catalizzata dalla carnitina palmitoiltransferase
I (CPT I). Queste acilcarnitine a catena lunga vengono poi trasportate attraverso la membrana
mitocondriale interna nella matrice dei mitocondri tramite la carnitina/acilcarnitina translocase. Qui
la acilcarnitina a catena lunga viene convertita di nuovo in carnitina e acil-CoA a catena lunga dalla
CPT II, enzima localizzato nella membrana interna dei mitocondri, e a questo punto, gli acidi grassi
a catena lunga possono essere metabolizzati tramite la P-ossidazione.
In secondo luogo, la formazione delle acilcarnitine dai rispettivi acil-CoA e importante al fine di
mantenere un pool di CoA libero (funzione tampone). Gli acil-CoA a catena corta sono convertiti
nelle rispertive acilcarnitine tramite l'azione della camitina acetiltransferase (CAT). Poiche la
quantita cellulare di CoA e generalmente inferiore a quella della carnitina, questa ultima puo essere
considerata come un "tubo di scarico" per i gruppi acilici. E' interessante notare come la reazione
catalizzata dalla CAT fomisca CoASH, essenziale per una serie di importanti processi metabolici
(per esempio, la p-ossidazione e il ciclo di Krebs). Se le acilcarnitine non vengono riconvertite nei
loro rispettivi derivati del CoA (i quali possono essere metabolizzati de novo), le molecole in
eccesso vengono espulse.
L'antico nome di vitamina Bt venne attribuito per la vitale importanza di questo composto nei
verme Tenebrio molitor. Nei mammiferi la camitina e fornita dal cibo, ma puo' anche venir
sintetizzata dall' organismo stesso. La biosintesi della camitina comprende sei diversi passaggi e sei
corrospondenti enzimi. La maggior parte degli organi animali e in grado di sintetizzare il suo
precursore, la butirobetaina, mentre I'enzima coinvolto nell'ultimo passaggio prima della
formazione della carnitina (y-butirobetaine idrossilase) e riscontrabile solo in fegato e reni. La
carnitina del fegato, che puo' essere originata per biosintesi o assorbita dal cibo, puo essere

rilasciata nei plasma e distribuita agli altri organi. Poiche la concentrazione della camitina nei
plasma e normalmente 10-100 volte inferiore a quella misurata negli altri tessuti, si presume che il
passaggio dal plasma a questi ultimi awenga tramite un meccanismo di trasporto attivo.
Negli ultimi 20 anni la carenza di camitina e stata correlata a diversi stati patologici. Le malattie
legate alia camitina si dividono in affezioni primarie e secondarie. Un'affezione primaria e
caratterizzata da un alterato riassorbimento di camitina alterato nei reni e/o da livelli molto bassi di
camitina nei muscoli. Molteplici sono le affezioni secondarie: quelle legate a errori metabolici
ereditari vanno da difetti negli enzimi della P-ossidazione a difetti nelle proteine coinvolte nei ciclo
della camitina, da disordini del catabolismo dei residui ramificati degli amminoacidi a disordini a
livello mitocondriali. Le affezioni secondarie sono riscontrate anche in un gran numero di malattie
acquisite quali, ad esempio, cirrosi, nascita prematura e sindrome di Fanconi, tutte patologie in cui
il livello di camitina nei tessuti e nei plasma deve essere tenuto sotto stretto controllo. Alia luce di
qeusti dati risulta di estremo interesse determinare quali siano le caratteristiche del trasportatore di
camitina dei reni, al fine di identificare quale e il difetto responsabile delle affezioni primarie, e
migliorare l'approccio terapeutico alle varie affezioni secondarie che presentano un alterato
riassorbimento della camitina nei reni.
Nella prima parte di questo lavoro si e cercato di esprimere in oociti di Xenopus laevis e
caratterizzare il trasportatore della carnitina dei reni di ratto. Questi studi hanno rivelato l'esistenza
negli oociti di un trasporto di camitina endogeno dipendente da Na+ e con Km di 66 uM, mentre la
Km per il trasportatore dei reni di ratto espresso e di 149 pM. Oltre alia differenza in Km, i due
sistemi sono distinguibili per la loro diversa dipendenza dal pH e per la diversa sensibilita a vari
inibitori (Berardi et al., 1995). II passaggio successivo e stato la separazione per grandezza del
mRNA di rene di ratto (zona corticale) e la costruzione di una libreria di cDNA in E. coli. La
separazione e stata effettuata con un gradiente di saccarosio e il segnale e stato amplificato di tre
volte. Data la presenza del trasportatore endogeno, l'identificazione dei cloni positivi e risultata
difficile, poiche non e stato possibile trovare una sostanza che inibisse in modo selettivo il
trasportatore degli oociti.
Strategie alternative per la caratterizzazione del trasportatore sono state studiate nella seconda parte
di questo lavoro. Per prima cosa sono state testate alcune linee cellulari in coltura al fine di
identificare quale fra esse non trasportasse camitina. Le uniche a non mostrare trasporto di camitina
in presenza di Na+ sono state le cellule CHO. Abbiamo cercato di stabilire in quali condizioni
queste cellule potessero essere usate per l'espressione transiente. Purtroppo il trasporto elevato in
assenza di Na+ ha reso tutti i nostri tentativi vani. Successivamente, abbiamo approfittato delle
omologie nelle sequenze dei trasportatori di GABA, GABA/betaina e serotonina, e del fatto che tali
sostanze hanno proprieta simili alia camitina, per produrre degli oligonucleotidi adatti alio
screening di cDNA in vari organi e cellule. Abbiamo usato il metodo della "reverse transcriptase"
PCR per amplificare eventuali sequenze positive. Solamente il segnale ottenuto con mRNA di
fegato di ratto e stato preso in considerazione, gli altri segnali positivi non sono stati analizzati per
mancanza di tempo.
Nell'ultima parte di questo lavoro dottorato il trasportatore di camitina del muscolo di ratto e stato
studiato usando vescicole isolate dalla plasma membrana. Le vescicole hanno mostrato un grado di

purezza elevato, con i markers enzimatici della plasma membrana sensibilmente arricchiti.
L'orientazione delle vescicole e stata testata al fine di determinare il trasporto di camitina: circa i
due terzi di esse possedevano la giusta orientazione. II trasporto di camitina in queste vescicole e
risultato stimolato da un gradiente di sodio [Na+]0ut>[ Na+]ln e di potassio [K+],„>[ K*]^, e
saturabile con Km di 13 pM. Dal momento che questo trasporto e apparso inibito da determinati
antagonisti, mentre il trasporto di camitina nei reni e inibito da sostanze completamente diverse, si
puo' ipotizzare che i trasportatori di camitina dei reni e dei muscoli abbiano proprieta fisico-
chimiche assai diverse.

ZUSAMMENFASSUNG
Camitin (P-Hydroxy-y-trimethylammonium butyrat) wurde als ein mengenmassig wichtiger
Bestandteil des Muskelgewebes bereits 1905 entdeckt. Bei Tieren und Tierprodukteist es meist in
hohen Konzentrationen vorhanden, besonders in Fleisch, Milch und Milchprodukten, wobei rotes
Fleisch als der wichtigste Lieferant gilt. Camitin kommt auch im Plasma vor. Die physiologischen
Konzentrationen im Menschen erreichen folgende Werte:
i) rotes Fleisch : 1-4 mM;
ii) Herz 0.6-1.3 mM;
iii) Milch : 0.05 mM;
iv) Leber : 0.5-1 mM;
v) Nieren : 0.3-0.6 mM;
vi) Gehim : 0.5-1 mM;
vii) Plasma 0.004-0.007 mM.
Camitin ist in geringer Konzentration auch in Pflanzen und Gemiise auffindbar.
Bei Saugetieren erfiillt Camitin zwei Hauptfunktionen. Erstens erfiillt es eine wichtige Aufgabe bei
der mitochondrialen P-Oxydation von langkettigen Fettsauren. Freies Camitin wird mittels eines
unbekannten Transporters vom Plasma im Cytosol transportiert, und nachher in den
intermembranaren Raum durch ein Mechanismus, das wahrscheinlich kein aktives Transport ist, da
die ausser mitochondrialen Membran undicht ist. In einer Reaktion, die durch Camitin
Palmitoyltransferase I (CPT I) katalysiert wird, werden die entsprechenden Carnitin-Derivate von
langkettigen Acyl-CoA synthetisiert. Diese langkettigen Acyl-Carnitine werden durch die innere
Membran des Mitochondriums mittels der Camitin/Acylcarnitin Translocase transportiert. In der
Matrix des Mitochondriums wird das langkettige Acyl-Carnitin, mittels CPT II wieder in die
entsprechenden Acyl-CoA-Derivate umgewandelt. Am Schluss dieses Prozesses konnen die
langkettigen FettsSuren die P-Oxydation durchlaufen.
Zweitens ist die Umwandlung von Acyl-Carnitin aus dem entsprechenden Acyl-CoA wichtig, damit
ein Zellpool von freiem CoA aufrechterhalten werden kann. Diese Funktion des Camitins ist als
Puffer-Funktion bekannt. Kurzkettige Acyl-CoA konnen in die entsprechenden Acyl-Carnitine
mittels Camitin-Acetyltransferase (CAT) umgewandelt werden. Da der CoA-Zellpool meistens
kleiner als der von Camitin ist, kann letzterer als ein Reservoir fiir Acylgruppen betrachtet werden.
Es ist interessant festzustellen, dass die durch CAT katalysierte Reaktion CoASH freisetzt, was fur
eine Anzahl metabolischer Prozesse von Bedeutung ist (z.B. die P-Oxidation und den Krebs-
Zyklus). Wenn Acetyl-Camitin nicht in die jeweiligen CoA-Derivate zuriickverwandelt werden
kann, werden die uberzahligen Molekiile ausgeschieden.
Beim Mehlwurm Tenebrio Molitor ist Camitin lebensnotwendig. Daher wurde fruher fiir Camitin
der Name Vitamin BT vorgeschlagen. Die Zufuhr von Camitin bei Saugetieren findet durch die
Nahrung und durch endogene Biosynthese start. Die Biosynthese von Camitin durchl8uft sechs
Stufen und erfordert sechs verschiedene Enzyme. Viele tierische Gewebe konnen die Biosynthese

von Camitin bis zur Stufe von Butyrobetain - dem direkten Vorlaufer des Camrtms - vollziehen
Das letzte Enzym im Biosyntheseprozess, die y-Butyrobetainhydroxylase, kommt jedoch nur in der
Leber und in den Nieren vor Camitin aus der Leber (sei es biosynthetisch durch den Korper
aufgebaut oder mittels Aufhahme mit der Nahrung), kann dem Plasma abgegeben und in die
ubngen Korperorgane verteilt werden Da die Plasmakonzentration von Camitin normalerweise 10
bis 100 mal kleiner ist als diejenige im Gewebe, darf man annehmen, dass Camitin aus dem Plasma
in der verschiedenen Organe aktiv transportiert wird
In den letzten 20 Jahren wurden etliche pathologische Zustande beschrieben, die auf Mangel an
Camitin zuruckzufuhren smd Der pnmare Camitinmangel ist durch eine veranderte Camitin
Reabsorption seitens der Nieren charaktensiert, was zu massiven renalen Verlusten mit
aussergewohnlich niedrigen Carnitin-konzentrationen im Gewebe wie Muskel, Leber und Gehim
fuhrt Der sekundare Camitinmangel kann aus manmgfaltigen Ursachen entstehen Meistens wird
er durch genetisch bedingte Storungen des Metabohsmus verursacht, welche von Fehlem in der P-
Oxidation, Mangeln in den Enzym des Camitin Biosynthese, dem Metabohsmus von
verzweigtkettigen Anunosauren, bis zu mitochondrialen Defekten reichen Der sekundare Mangel
an Camitin wird auch durch erne Anzahl bekannter khmscher Krankheiten wie Leberzirrhose,
Fruhgeburten und das Fancom-Syndrom verursacht Diese Tatsachen lassen den Schluss zu, dass
der Carmtinspiegel in den Geweben normalerweise unter strenger Kontrolle gehalten ist Es ist
somit von grossem Interesse, die Eigenschaften des Plasmamembran Carnitin-Transporters der
Niere zu charakterisieren Dies wurde gestatten, die Ursachen der Fehler bei Patienten mit pnmarem
Camitinmangel zu klaren Die Bestimmung der Struktur dieses Transporters wurde zusatzlich eine
wirksamere Therapie des - die gestorte glomerulare Nieren-Reabsorption verursachenden -
sekundaren Camrtinmangels gestatten
Im ersten Teil dieser Arbeit wurde versucht, das Carnitin-Transport der Rattennieren in den
Oozyten von Xenopus laevis zu expnmieren und zu charakterisieren Die Versuche zeigten ein Na+-
abhangiges Carmtin-Transport-System in den Oozyten von Xenopus laevis mit einem Km-Wert von
66 pM Zum Vergleich hatte der expnmierte Camitin-Transport etwa Km-Wert von 149 uM Ausser
an den verschiedenen Km-Werten konnte die beiden Transportsysteme durch die pH-Abhangigkeit
und em diffenerendes Hemmungsmuster von strukturell ahnhchen Substanzen unterscheiden
(Berardi et al, 1995) Der nachste Schritt war die Grossenfraktionierung von mRNA aus dem
Kortex von Rattennieren, urn daraus eine cDNA-Bibhotfiek in Escherichia Coli aufzubauen Die
Grossenfraktionierung in einem Sukrose-Gradient ergab erne Verstarkung des Anfangssignals um
den Faktor 3 Die cDNA-Bibhothek konnte somit erfolgreich aufgebaut werden Oozyten konnen
betrachthchen endogenen Carnitin-Transport besrtzen, war es sehr schwieng, positive Klone
emdeutig zu identifizieren Ungluckhcherweise konnte bis jetzt kerne Substanz identifiziert werden,
die den endogenen Transport von Camitin spezifisch hemmt
Im zweiten Teil der Arbeit wurden verschiedene alternative Strategien angewandt Vorerst wurden
verschiedene Zellen untersucht, um erne Zellenlime zu finden, die womoghch kein Camitin
transportiert Fast alle untersuchten Linien transportierten Camitin Einzig CHO-Zellen
transportierten Camitin in einer von Na+ unabhangigen Weise Es wurde ebenfalls versucht, die
Bedingungen festzustellen, bei welchen diese Zellenhmen fur eine transiente Expression verwenden

werden konnten. Die hohe, von Na+ unabhangige Carnitinaufnahme liess jedoch die Versuche
scheitern. Es wurde eine zusatzliche Studie in Angriff genommen, um iiber angenommene
Ahnlichkeiten zwischen dem Plasmamembran Carnitin-Carrier und dem bekannten GABA/Betain,
GABA- und Serotonin- Transporter den Carnitine-Carrier zu klonieren. Zuerst wurden die
Sequenzen dieser drei Transporter auf Ahnlichkeiten in der DNA-Bibliothek untersucht, um
Oligonucleotide mit der hochsten Ubereinstimmung ableiten zu konnen. Diese Oligonukleotide
wurden als Primer fur ein cDNA-Screening in verschiedenen Geweben und Zellen verwendet. Die
Anwendung der RT PCR-Technik gestattete es, verschiedene positive Signale zu ermitteln. Es
konnte nur das amplifizierte Produkt, das aus dem mRNA von Rattenlebem kam, untersucht
werden. Wegen Zeitmangels war es nicht mSglich, die anderen positiven Signale einer PrUfung zu
unterziehen.
Im letzten Teil dieser Doktorarbeit wurde der Carnitintransport in den Skelettmuskeln der Ratte
untersucht, indem Vesikel aus dem Sarkolemma isoliert wurden. Die Vesikel waren von relativ
hoher Reinheit, mit starker Anreicherung sarkolemmale Marker. Um die Aufnahme von L-Camitin
an diesen Vesikeln zu bestimmen, wurde deren Orientierung untersucht. Etwa zwei Drittel der
isolierten Vesikel hatten die richtige Orientierung. Es konnte festgestellt werden, dass, wenn ein
Na+-Gradient [Na+]out>[ Na+]ln und ein K+-Gradient [K+]ln>[ K+]out vorhanden waren, die Aufnahme
an L-Carnitin in den sarcolemmalen Vesikel maximal stimuliert wurde. Die Camitineaufhahme war
saturierbar mit einem Km -Wert von 13 pM und konnte konzentrationsabhangig durch verschiedene
Camitin-Derivate gehemmt. Im Vergleich dazu wurde die Carnitinaufnahme in die Nieren durch
ganz andere Verbindungen gehemmt, was zur Schlussfolgerung Anlass gibt, dass der Carnitin-
Transporter der Muskulatur nicht der gleiche ist wie jener der Niere.
