The Division continues to support GPs

to provide nurse practitioner support to provide primary care to the mentally ill and homeless population of the Salvation Army Caring Place homeless shelter with a vision of

reducing emergency service use.

During the assess-ment and planning phase of our work within the provincial A GP for Me initiative, our medical community and the general public identified that there was no central

priority amongst members, and top of mind for the Ridge Meadows Division of Family Practice Board of Directors in every one of our initiatives. The last two years the Division has created and distributed a popular Medical Community Resource Guide with GP and SP photos, and contact details with Medical and Community Service information as a way to stay up to date and connected.

We have successfully recruited eight new physicians to our community this year, replacing retiring physicians, support-ing clinics that are overloaded with patients, mentoring

an International Medical Graduate to our community, and adding a GP to the Emergency Department in the Hospital and the Maternity Clinic. The Division continues to

work hard attracting GPs to our commu-nity by coordinating tours, developing helpful “how to get started” guides, showcasing the community, and developing a Red Carpet Program.

The Physician Recruitment Committee also developed a Physician Supply Model that tracks GPs retiring now, next year, in 3-5 years, and in 5-10 years. This enables a proactive approach to fulfilling our physician replacement needs.

A solid GP retention plan has also been put in place with our many collegial “clubs”:

Young Docs Club, Old Docs Club, Welcome Spouses Club, and GP Mentorship Program.

A new Education Initiative address-ing members’ needs was launched, reviewing current education inventory, and strategizing an annual education plan. In 2014/15 there were thirty four learning sessions (offered through the Division or by other partners) provided to physicians and MOA’s in our com-munity reaching over two hundred participants. Sessions included topics such as Dermatology, Addictions Workshop for MOA’s, PSP module on Office

Efficiency, De-Mystifying the ER, Billing Codes Event, webinars, and the Grand Rounds offered at Ridge Meadows Hospital.

A GP for Me contin-ues to play a critical role in helping us pilot Team-Based Care solutions for GPs with a Practice Nurse providing support to eight GPs in three clinics and a GP Extender in five clinics to prove a busi-ness case to increase physician capacity and satisfaction. A business model and orientation manual will be shared with clinics in fall 2015.

We continue to support GPs with a MOA Network for their staff providing three professional development sessions this past year with first aid, Patient Conflict Resolution, and vaccinations. ■

The Division applied for and received funding

for three new initiatives to support patients in the

community and will continue supporting the Maternity Clinic with physician recruitment and sustainability strategies.

GP members asked us to improve Senior Patient Care, Residential Patient Care, and Child and Youth Mental Health

& Substance Use. A new Seniors Initiative was launched this year through the work of GPs and the community to develop a Collaborative Care Model. Working with home health, community services, specialists, and seniors/families, our

goal is to improve care for seniors throughout the system of care. Improving residential patient care was also a priority identified by our members, and a new local model supporting GPs is in development. Over 50 community stakeholders, SPs, and GPs came together to identify challenges for children, youth, and their families accessing programs and services for mental health and substance use. A project is being developed to identify solutions for the community. ■

Acting on member priority that EMR and

technology clinic support is in need, the Division developed an IT strategy, in partnership with PSP to execute a local clinic support strategy. Local visits will include custom EMR training at various GP/MOA levels, in addition to Pathways and UpToDate training. Small group sessions are also being coordinated to facilitate problem solving and idea sharing among clinics.

Pathways and UpToDate are provided to members at no cost to ensure that the latest physician and patient resources are at your fingertips.

GPs and Ridge Meadows Hospital continue to work on improving GP and hospital communica-tion through the Acute Care project. A

survey in 2015 indicated that only 45% of GPs consis-tently receive notifications about patient death while 28% never receive notice. Strategies to improve communica-tion around death notices, discharge and follow-up care, as well as dialogue around ER referrals have been worked on, as a result:

• 40% increase in GP’s receiving notification about patient death.

• A trial of patients discharged from RMH left

with a booked appointment to see their GP. This process worked well and plans are to spread this to the rest of the hospital units. 90% of physicians felt that the knowledge gained from the ER breakfast and learn will change their approach to referring patients to the ER.

Elements of the Division’s Health and Wellness

Strategy include engaging GPs, SPs, patients, and community members in committees; providing advice; and organizing educa-tional and social events.

The Division members collabo-rated with many partners in the community, including Ridge Meadows Hospital Foundation and corporate sponsors including Scotiabank, Society of General Practitioners of BC, Seniors Health Network, Community Network, municipali-ties, RCMP, School District, Chamber of Commerce, Ramada Hotel, private schools, and realtors.

These members were involved in project committees and

social events such as our Golf Tournament, Summer BBQ, Annual General Meeting, February Member Billing Event, and Community Spirit Walk and Cycle event.

The Division hosted 129 committee member meetings this past year engaging 69 GP members, and 59 community stake-holders in 28 projects.

This is an investment of 7500 GP advisory hours making an impact in our projects.

This type of engage-ment is outstanding and truly a testament to the strength of our medical community. ■

resource listing of GPs who are accepting new patients. To address this, we

recently announced the launch of www.findadoctorpitt-meadows.ca and

www.findadoctor-mapleridge.ca, that the Division will also use as a recruitment

tool to attract GPs and help develop their patient roster before they begin practicing in our community.

The Maternity Clinic was also hard at work creating a matching tool system to attach mother/child to GPs in our community. Approximately 200 mother/child dyads were attached as a result of this initiative. The learnings from this tool will be used to develop an attachment tool that will extend to the wider medical community. ■

With 69 GP members engaged in

over 28 projects, contributing 7500 hours over the past year, we can’t help but feel we are moving local health care mountains.

On behalf of the Board

and Staff, we thank the dedication, effort, and countless contributions our GP members are bringing to the Division to improve our medical community.

None of this work could be possible without this exem-plary contribution by our hard-working members coming to the committee and event tables and sharing their wisdom.

As we embrace the arrival of fall, the Division will be work-ing hard completing the evaluation and business cases for our A GP for Me work by February 2016. We will also be starting an Addictions Inquiry in our community as we know this is a concern for GPs and patients alike. We also will be strategizing the work of our Child and Youth Mental Health and Substance Use project, IT Strategy, Seniors project, Member Education

project, and reviewing our medical com-munity’s impact.

The Board will be relying and reflecting on member feedback in the new year, and making decisions of new project direction in 2016 and 2017.
