    Non provision of dry

    riser for a 6-storey

    building, premises

    greater than 10m.

    RI admitted he overlooked and did not detect this

    irregularity on-site.

    The building was only 6 storeys, he was of the view

    that the fire engine could reach all levels of the

    building.

    RIIC

    He should have noted the omission of dry riser provision and

    reported accordingly.

    The assumption that the fire engine could reach all levels of the

    building is unacceptable.

    Non provision of

    automatic alarm system

    for building more than

    4 storeys.

    As the RI (Arch), he is responsible for the

    architectural aspects .

    The automatic alarm system was under RI (M&E).

    RIICRI (Arch) holds the duty to point out any form of deficiencies in fire safety

    provisions.

    He should have known that a 6-storey building has to be equipped with an

    automatic alarm system.

    He demonstrated bad practice.

    Since the staircase was enclosed, he assumed that it was

    mechanically-ventilated.

    RI (M&E) would always be engaged and be responsible to

    check on the ventilation requirements to staircase.

    Not to assume that mechanical ventilation would be provided to

    the staircase.

    He should communicate and verify with QP.

    We cannot foresee its future use and RI should have advised the QP

    to seek waiver

    Exit signs installed at 3.4m high

    Height of exit lights (only at mainhall entrance) were mounted

    higher than 2.5m.

    The hall had much higher ceilingheight and any smoke in the hall

    would not obstruct the exit lights.
