Getting Started, It’s Easy We’re With You Every Step Of The Way

To begin the plan installation process, SMA Services, Inc. will need a fully executed Client Services Agreement and a completed Adoption Agreement checklist.

Existing plans converting their services to us will need to submit their current Plan Adoption Agreement and any amendments.

These items, if applicable, are required, but can be provided at a later date:

• IRS Form 5500 for the plan year ending prior to the conversion date

• Last plan year-end compliance testing results• Last plan year-end allocation report
