RESIDENTIAL SOIL EROSION PERMIT CHECKLIST IN ORDER TO SECURE A SOIL EROSION PERMIT THE FOLLOWING ITEMS MUST BE PRESENTED AT THE TIME OF APPLICATION. THE COMPLETED APPLICATION MUST INCLUDE THE FOLLOWING: SECTION 1 PERMITTEE Name of the permittee may be the property owner or a designated agent. Designated agents must provide a declaration in writing by the landowner naming them agent and the home address and contact number of the landowner. SECTION 2 LOCATION Location of the project, township, village and the complete property tax number and the full street address. If applicable a street address must be posted at the site prior to the initial inspection. SECTION 3 PROPOSED EARTH CHANGE Select your project type and describe it. What are you building? How much ground will you be disturbing and how much of an earth change will it be, more or less than an acre? Name of and the distance to the nearest lake, stream, or county drain. Date of the project start and estimated completion date. SECTION 4 SOIL EROSION AND SEDIMENTATION CONTROL PLAN Scaled drawings are not required, but drawings should contain the following information: R 323.1703 Soil erosion and sedimentation control plan requirements. Rule 1703. A person shall prepare a soil erosion and sedimentation control plan for any earth change identified in R 323.1704. A person shall design the plan to effectively reduce accelerated soil erosion and sedimentation and hsall identify factors that may contribute to soil erosion or sedimentation, or both. The plan shall include, but not be limited to, all of the following: Minimum SESC Plan Requirements (Rule 1703): ! Scaled drawing ! Site location sketch ! Proximity of the earth change to lakes and streams ! Predominant land features. ! Contour intervals or slope description ! Soil survey or description of soil type(s) ! Description and location of the physical limits of each proposed earth change ! Description and location of existing and proposed on-site drainage and dewater facilities ! Timing and sequence of each proposed earth change ! The location and description for installing and removing all proposed temporary SESC measures ! Description and location of all proposed permanent SESC measures ! Maintenance plan for temporary and permanent SESC measures

Required Information Rule 323.1703 promulgated under Part 91 requires that all SESC plans include, at a minimum, the following: 1. Site location map, legal description of property, and scaled map showing property boundaries (Figure

If a silt fence is required on your project, the bottom 6 inches of the fence must be toed in (buried). No exceptions.
