1.1 FACILITY INFORMATION

1.1.3 HOURS OF OPERATIONContract services performed at the Emergence Health Network facility shall occur generally between the hours of 8:00am and 5:00pm on Monday through Friday. Delivery of materials be within these contract hours. If additional hours throughout the work week or weekend work are necessary to complete shall the furniture installation by the required completion date, permission to perform contract services during these additional hours may be granted.

1.2 SUBMITTALSYou are hereby invited to submit a proposal to the Emergence Health Network for their new main offices interior furnishings as instructed on “Notice to Interested Parties”

Questions – Please submit questions in writing as instructed on “Notice to Interested Parties”

1.3 EVALUATION FACTORSAward will be made to the Contractor that provides the best overall value to the Emergence Health Network. For example an award may be made to a higher priced Contractor with an earlier Installation Start/Completion Time for this request. The evaluation factors for the furniture award are:

Quality – All products shall be tested and meet or exceed ANSI/BIFMA guidelines while meeting or exceeding the requested specifications mentioned herein, carry a substantial warranty period and MUST BE MADE AND ASSEMBLED IN THE UNITED STATES OF AMERICA. No imports will be considered even if assembled in the States. This is the most important factor. Please include all testing documents in your response.

Sustainability – All Product shall be Bifma level 2 or 3 Certified and carry an Indoor Air Quality Gold CertificateSustainability is second most important factor. Please submit all certificates in your response. This is the second most important factor.

Price – Include supplies and services and one month’s storage costs. Pricing must be equal to current TXMAS Schedule or as this entity qualifies for. Price is third most important factor.

Earliest Installation Start and Earliest Completion Date. This is the fourth most important factor.

Quick Ship Availability – Availability to ship within the time needed for move in date.

Past Performance – The Contractor shall provide the Emergence Health Network with at least three (3) past performance references using the same product lines in an application similar to this project completed within the past three (3) years. References must include the project name, point of contact (POC) name, phone number and email for each of the POCs.

Technical Features – All line items must meet or exceed technical requirements to be considered for award. Please include all technical data sheets from the manufacture in your response.

Manufacturer Continuity – Number of manufactures provided in quotation should be kept to minimum for ease of maintenance, inventory control and aesthetic consistency.

Technical Factors – Special features, warranty considerations, and environmental and energy efficiency considerations will be evaluated with equal importance.

Special Features – include features goods/service required for effective program performance that increases the flexibility of use, quality, durability and longevity or are more aesthetically pleasing.

Warranty – Warranties shall include both parts and labor for 10 years or longer on all casegoods, 5 years or longer on pneumatics cylinders, 2 years or longer on task lighting, and 1 year or longer on fabrics. Please submit warranty documents from each manufacturer with you response.

Environmental Sustainability information – Provide an information sheet for each of the product lines used that outlines the following:Product SummaryEnvironmental SummaryBIFMA e3 CertificationContributions to LEED® – CI 2.0

1.4 SPECIAL REQUIREMENTS

1.4.1 PRE‐INSTALLATION SITE VISITContractor shall perform a pre‐installation site visit at the Emergence Health Network facility prior to ordering furniture for the contract. After award and prior to the Pre‐Installation Site Visit, the Emergence Health Network will initiate a teleconference to establish a timeframe for the Contractor’s Pre‐Installation Site Visit.

Contractor shall verify actual room sizes at the Emergence Health Network facility against room sizes identified in theFurniture Floor Plan included in this solicitation. Discrepancies between the actual room sizes and thesizes indicated on the drawings shall be noted, and adjustments to sizes of the proposed furniture shallbe made as necessary to fit the actual spaces. The furniture shall not be ordered until the pre-installation site visit occurs. Coordinate visit with contact or obtain construction update from contractor.

1.4.2 POST‐AWARD FURNITURE FLOOR PLAN SUBMITTALAfter contract award the Emergence Health Network will provide an electronic CAD file of the floor plan to the awarded Contractor. The Contractor shall use this CAD file to prepare a Furniture Floor Plan showing the furniture items selected from the manufacturer’s standard line of furniture. Any size discrepancies between the manufacturer’s furniture items and the Emergence Health Network spaces shall be noted and brought to the attention of Emergence Health Network.

1.5 SCHEDULE OF COMPLETIONManufacturing of furniture and equipment must be expedited to be completed within a four week window from placement of order to order ready for shipment. A letter(s) from the manufacturer(s) guaranteeing a four week manufacturing time period must be submitted as part of the quote.

Installation of furniture and equipment must be completed no later than May 27. Confirm with “Notice to Interested Parties” .

Furniture and equipment must be installed and the building must be functional and ready for occupancy by the completion date (with the exception of final incidental punchlist items)

1.6 PRODUCTS

1.6.1 QUANTITIES

Quantities of furniture and equipment shall be provided as specified in the solicitation package, however interested party is responsible for confirming quantity counts.

1.6.2 SUSTAINABILITY

LEED for Commercial Interiors is the green benchmark for the tenant improvement market. It is the recognized system

for certifying high‐performance green interiors that are healthy, productive places to work; are less costly to operate and maintain; and have a reduced environmental footprint. LEED for Commercial Interiors gives the power to makesustainable choices to tenants and designers, who do not always have control over whole building operations.

In an effort to support green initiatives and to assist in a LEED rating for Commercial Interiors, furniture and equipment must meet the following requirements:

ANSI/BIFMA Standards

Casegoods shall be rated level 2 (level 3 with FSC) ; Seating shall be rated level 2.Panel systems shall comply with ANSI/BIFMA X5.6.

Metal storage shall comply with ANSI/BIFMA X5.9.

Furniture and furnishings proposed in response to this request shall be independently third-party certified as compliant with the ANSI/BIFMA e3 Furniture Sustainability Standard at level 1 or higher. Signed documentation of level certification by a third-party certification body licensed to participate in the BIFMA level program shall be provided.Examples of acceptable documentation include signed certificates of compliance from a licensed third-party certification body as defined at level® certified third-party.

MR Credit 4: Recycled Content

Furniture and furnishings shall have at least 25% postconsumer recycled content and 50% total recycled content as determined by weight.

MR Credit 7: Certified Wood

Wood casegoods shall have an option to be manufactured from certified wood products in accordance with the Forest Stewardship Council (FSC).

Indoor Environmental Quality

IEQ Credit 4.5: Low‐Emitting Materials – Systems Furniture and SeatingSystems furniture and seating shall be Indoor Advantage Indoor Air Quality Gold Certified.

1.6.3 INTENTUnless otherwise noted, these specifications given are for purposes of functional performance, not for specific proprietary design conformance. Products quoted that meet functional requirements, will be considered responsive, if they demonstrate equal performance, quality and value. All quoters shall provide descriptions of how proposed solutions meet the functional requirements. All products in a given room, to include casegoods, storage, and seating shall be similar in style and in finish.

1.7 INSTALLATIONThe Contractor shall provide all necessary materials, equipment, labor, supervision, and management to coordinate deliveries; load and unload furniture; place/install in designated rooms; assemble items as required; secure and store as designated; and properly dispose of all associated packaging/crating materials.

The Contractor shall schedule a pre‐installation site visit to the Emergence Health Network facility prior to ordering furniture to verify the actual room sizes for furniture placement.

All quotes shall include field verification of spaces for furniture placement, delivery and installation.

1.8 DELIVERY

The prime contractor shall coordinate delivery from all sub‐contractors. The Contractor shall contact Emergence Health Network Facility point of contact at least fifteen (15) working days prior to shipping to ensure the building is ready to receive goods. Goods shall not be shipped until confirmation from the Emergence Health Network POC is received. The contractor is responsible for the storage of furniture prior to shipping at no additional cost to Emergence Health Network.

Once the contractor receives confirmation from the Emergence Health Network that the building is ready to receive goods, the Contractor shall notify Emergence Health Network POC by email at least 24 hours prior to the date the delivery truck is scheduled to arrive at the Emergence Health Network Facility.

Should a delay in delivery be requested by Emergence Health Network POC, or if the Contractor is unable to receive confirmation from Emergence Health Network that the building is ready to receive goods, the Contractor shall notify the Contracting Officer within one (1) working day.

The Contractor shall comply with all site security regulations and shall initiate procedures to minimize the impact to on‐site personnel.

Check‐in at the front desk is mandatory upon delivery truck arrival. Driver shall have valid driver’s license, vehicle registration and proof of insurance, shipping manifest with designated building “on the ground” as well as receiving POC name and telephone number. Driver’s license must be kept on the person at all times.

Drivers of the delivery vehicles shall not attempt to deliver any items before the scheduled delivery date. In the event the delivery date falls on a Saturday, Sunday, or recognized holiday, the delivery date shall be the next workday after the Saturday, Sunday, or holiday and you must notify Emergence Health Network point of contact.

1.9 RECEIVE AND UNLOAD

Upon delivery the Contractor shall unload, assemble and install goods during the working hours identified in the contract. The Contractor may only work outside of the designated working hours with prior approval from Emergence Health Network.

If the Contractor is unable to complete assembly and placement of unloaded items before the end of the work day, the contractor shall transport those remaining items to a location designated by Emergence Health Network for storage until the next work day. The Contractor shall be responsible for moving the items from the overnight storage site to the designated areas within the building.

The Contractor shall verify and inspect all items upon delivery. Any deficiencies (damage/overage/shortage) shall be brought to the attention of the Emergence Health Network POC. The Contractor shall document all deficiencies inwriting and shall forward deficiency reports to the Emergence Health Network POC on a daily basis. The Contractor shall repair or coordinate the replacement of damaged, defective or missing items.

1.10 ASSEMBLE, PLACE AND INSTALL

The Contractor shall unpack all items received and assemble in accordance with the manufacturer’s instructions.Contractor shall place items in rooms as identified on contract drawings. If no drawings are provided or item for placement is not shown on drawings, item shall be placed as directed by the Emergence Health Network .

Furniture, equipment and accessories shall be level, plumb, square, and in proper alignment with adjoining furniture.

Contractor shall remove all stickers, labels, etc. and ensure no adhesive residues remain on surfaces.

Contractor shall clean all items installed under this contract and shall ensure the items are free from surface dirt and defects.

The Contractor shall notify Emergence Health Network POC of any discrepancy between the actual furniture placement and the location identified on the contract drawings.

1.11 PROTECTION OF PROPERTY

Construction of or renovation to the Emergence Health Network facility may have been recently completed under a separate construction contract.

Prior to the start of work for the furniture contract, the Contractor shall perform an inspection of the building and grounds with the United Way. The Contractor shall document existing damage to the building, grounds or existing furnishing to remain, and shall submit documentation of damage to the Emergence Health Network POC.

The furniture contractor shall not be held accountable for existing damage incurred at the Emergence Health Network facility prior to the start of the furniture contract.

Existing damage documented prior to the start of the furniture contract is the responsibility of theConstruction Contractor to correct and may be covered under warranty by the Construction Contract.

The Contractor shall protect all items from damage. The Contractor shall take precaution against damage to the building, grounds and other furnishings. The Contractor shall repair or replace any item, building surface or grounds damaged due to negligence and/or direct action by the Contractor. Repairs and/or replacement items shall be with materials of the same quality, size, texture and color to match adjacent surfaces.

Prior to contract completion and transfer to the Emergence Health Network , the Contractor shall be responsible for securing the area where work is being performed. If keys were provided to the Contractor for access, the Contractor

shall maintain accountability and control of keys, and shall return keys to the Emergence Health Network POC upon completion of work.

1.12 PRODUCT DOCUMENTATION

The Contractor shall provide all owner’s manuals, warranty information, assembly instructions, maintenance and careinstructions, operating instructions, and any other information pertinent to the furniture and equipment to the Emergence Health Network POC upon completion of the installation.

The Contractor shall turn over any special assembly tools, security hardware wrenches, and spare parts provided with the furnishings to the Emergence Health Network POC upon completion of the installation.

1.13 CLEAN‐UP

The Contractor is responsible for the removal and disposal of all trash associated with the uncrating and assembling of furniture and items installed under this contract. Final acceptance of the furniture from the contractor to the Emergence Health Network will not occur until all furniture installation trash is removed from the site. Upon acceptance delivery ticket will be signed.

The Contractor is responsible for providing their own trash and recyclable receptacles on site as necessary and is responsible for routing trash and receptacles off site on a regular basis and at the completion of the contract.

Recyclable cardboard products from the furniture installation shall be disposed of as recyclable material by the Contractor at the Contractor’s expense.

Personal trash (food wrappers, drinks, etc.) generated by the Contractor shall be removed from the project site on a daily basisby the Contractor.

Packing materials generated by the Contractor shall not be stored within the building longer than 24 hours.

1.14 FINAL FURNITURE PLANS

Provide one copy of the FINAL furniture plan in the most recent version of AutoCAD, along with one hard copy no smaller than 24”X36” of each floor. The final plans will include the following: building conditions that affect furniture layout including but not limited to power, all furniture awarded drawn in the correct sizes. Submit final drawings and disc to Emergence Health Network within 48 hours of delivery/installation start date.

2.0 CASEGOODS

2.1 Freestanding Laminate Casegoods- Visuals # 1-8

Baseline: National Waveworks/Epic

Desks, bridges, credenzas, reception desks

1. Laminate furniture shall be from one manufacturer.

2. Product must be able to integrate into the manufacture’s modular casegood product line. Product line shall include a wide range of freestanding components. Freestanding components shall be available in multiple sizes, including bow-front desks, double and single-pedestal desks, returns, bridges, bullet- shape tables, P-shape tables, credenzas, service modules, two & four drawer lateral files, bookcases with & and without doors, corner units, mobile pedestals, storage cabinets. Products shall be fully compatible and interchangeable with each other throughout the suite. Furnish and install all supplementary or miscellaneous items, appurtenances, and devices incidental to, or necessary for sound, secure, and complete installation.

3. If required by the schedule in the contract, all products shall have common components available in an express shipment program that provides delivery within 12 days. This program shall include at least three different finishes.

4. Product shall have a minimum of 8 wood grain laminate options, 10 solid laminate options, and 5 patterned laminate option with a softened 3mm PVC Edgeband to match.

5. Grain direction shall be right to left on desks and credenza and front to back on bridges and returns.

6. Worksurfaces shall be constructed of a medium density 45 pound fiberboardcore with a high pressure laminate on the top and a backer material on the bottom.

7. Joints shall be securely fastened with screws, dowels, and fasteners to ensure maximum strength.

8. Clips, screws, etc., and construction elements shall be concealed and not exposed.

9. Bracketry shall be provided to ensure proper installation and dimensional stability.

10. Deviations of 1” are allowed from dimensions specified on accompanying cutsheet and furniture floor plan.

11. Reception desk shall be one 40”H piece with a transaction worksurface that sits on top.

Modesty Panels:

1. Full height shall be provided at front of desks and shall be a minimum of 3/4" thick, or as per model (resin, acrylic).

End Panels:1. Support panels shall be constructed of medium density 45 pound fiberboard with wood veneer on the faces and shall be a minimum of 1-1/8” thick.

2. Shall have adjustable glides which feature at least 1 inch of adjustment.

3. Supports shall include all hardware necessary for field installation.

4. There shall be an end panel grommet available.

Lower and Freestanding Storage

1. Storage shall match desk in construction, material and finish.

2. Cases shall be constructed of particleboard and assembled using hidden fasteners.

3. All visible faces shall be covered with high pressure laminate.

4. Hinged doors shall have “European-style” adjustable hinges opening at 120 degrees for easy access to contents.

5. Drawers shall be 5-sided with a separate, removable front that can easily be field replaced without disturbing the contents of the drawer.

6. Drawers front and backs shall be laminated woodgrain and ½" thick, sides shall be laminated woodgrain and 3⁄8" thick. Drawer fronts shall be 3-ply construction with matched woodgrain.

7. Drawers shall have full-extension, ball bearing, progressive action glides, that have a lifetime warranty.

8. Functional load for pedestal drawers shall be based on long term wear testingof opening and closing the drawer for 75,000 cycles or hard use.

9. Proof loads shall be the capacity beyond the functional load in excess of hardload use but before failure.

10. Operational Load Capacity shall be minimum of .017 lb. per Cubic Inch of Drawer Interior Filing Space

11. Product shall have a minimum of 5 different hardware pull choices for doors and drawers, with at least one ADA option and an integrated pull design

12. Storage shall have glides with a minimum of 1” of adjustability.

13. Storage cases must be finished so that components can be used in either right or left-handed positions.

17. Doors for storage components shall include a framed resin option, laminate doors or open.

18. Bookcases, towers and vertical cabinets shall have adjustable shelves.

19. Wardrobes shall have a coat rod.

20. FSC-certified substrate shall be available as an option provided to meet LEED requirements, please provide upcharge amount as a separate line item.

Locks and Keying:

1. Pedestals shall have a front removable lock core supporting the ability to remove or replace a lock plug as needed in both a black and silver option.

2. All doors and drawers within a workstation shall be keyed alike.

Upper Storage Cabinets

1. Shall have vertical hinged door and sliding door versions.

2. Shall be standard in: all closed; all open and combination of open and closed configurations.

3. Overhead storage must be available in hutch base or wall mounted.

5. Product shall have a minimum of 16 different hardware pull choices for doors and drawers, with at least two different finish options for each.

6. Product shall have a minimum of 6 different edge profile options available.

7. Veneer surfaces shall have a minimum of 2 heights including 1 3⁄16” or 1 9⁄16” .
