1. Attached is a letter from AMELIA A. BIGLETE, Director IV, CHED Region VII and Regional Memorandum No. 58, s. 2013 re: CHED STUDENT FINANCIAL ASSISTANCE PROGRAMS for all interested High school graduating students.
2. All District Supervisors/District In-Charge and Main Secondary School Heads are requested to
facilitate the filling up of the application forms.
3. Duly filled up application forms may be submitted directly to the CHED Office starting February 1, 2013 to March 31,2013, or to the Division Office on March 14,2013, do MRS. RACHEL B. PICARDAL at the
Division Promotional Office.
4. Application Forms are available at the Division Office and at the CHED website:

Attached is the copy of the Regional Memorandum No. 58, Series of 2013 issued by Dr. Carmelita T. Dulangon, Director Ill and Officer-in-Charge of the Department of Education Regional Office VII, together with 200 copies of Application Form (maybe photocopied).

Qualification and Documentary Requirements:
A. General Requirements: 1. Must be a Filipino citizen of good moral character. 2. Must not be more than 30 years of age at the time of application. 3. Must be a high school graduate or a candidate for graduation. 4. Must have a combined annual gross income of parents/guardian not to exceed Three
Hundred Thousand Pesos (PhP300,000.00). For those who are exempted from filing income tax, there should be a Certificate of Tax Exemption from the Bureau of Internal Revenue (BIR).
5. Must not be a recipient of any government scholarship and financial assistance. 6. Must have at least 80% general weighted average (GWA) based on Form 138, as
follows: 90% above full-merit 85% above half merit 80-84% - grant-in-aid & student loan programs
7. Entering freshmen and/or college student in any curricular year level (for SNPLP applicant only);
8. For student-borrower: 7.1 must enter into a loan agreement with CHEDRO; and 7.2 must have a co-borrower who is a member of SSS/GSIS in good standing (at least paying contribution for six (6) months for the last 12 months);
B. Documentary Requirements are: 1. Accomplished StuFAPs Application OSS Form 01; 2. Duly certified High School Report Card for third year and grades for the first
three grading periods for fourth year; 3. Latest Income Tax Return or Certificate of Tax Exemption of parents/ guardian
from Bureau of Internal Revenue (BIR); 4. Certificate of good moral character from the high school principal/guidance
counselor; 5. Certified true copy of grades during the previous semester attended (SNPLP
only); 6. Notarized contract between CHED and student borrower (SNPLP only); 7. Proof of SSS/GSIS premium contribution of co-borrower (SNPLP only).
