and agrees to abide by the terms and conditions outlined below and to all conditions under which venue space is leased to the

International Consumer Product Health and Safety Organization.

TERMS OF PAYMENT & CANCELLATIONAll payments must be received in FULL no later than July 1, 2019 and made payable to ICPHSO, ATTN: Emmy Mielcarz, 1120 Rte. 73, Suite 200, Mt. Laurel, NJ 08054. Tabletop space is assigned on a first-come, first served basis. Space will not be assigned or held without the signed agreement and all appropriate fees. Cancellations must be submitted to ICPHSO in writing regardless

of the timing of the cancellation. The date of receipt of supporter’s written notice of cancellation will be the official cancellation date. If support is cancelled on or before June 10, 2019, the supporter will be responsible for 50% of the total fee. Any supporter who cancels after June 10, 2019 will be responsible for the total fee. Failure to comply with any of the financial obligations listed above shall make the supporter ineligible for future ICPHSO events.
