CHT Partner uses the Logbook in profiling, master listing, giving

Family Health Record Form

NDP checks the entries in the LOGBOOK and helps the CHT Partner in:• Correctly filling-out the

logbook, Family Health Record Form and Masterlist

• Polishing the messages

RHM provides overall supervision to CHT Partners and NDP, provides services including counseling, records in

the facility and home-based records, provides appropriate health messages and uses the logbook to

find out who else needs services and should be followed-up by the CHT Partner
