Les maladies infectieuses émergentes
Severe acute respiratory syndrome (SARS), Grippe H1N1 Clostridium difficile 027, Chikungunya, Dengue, Syndrome hémolytique et urémique (Escherichia coli) Grippe H7N9 Coronavirus Chik Ebola Des crises sanitaires
“When an epidemic strikes: time +++”
pharmaceutique

REsearch and ACTion targeting emerging infectious diseases (REACTing) Y.Yazdanpanah B.Murgue JF.Delfraissy
Financement
• Funding for implementation of research tools during inter-epidemic period
= 500 K€/year: 400 K€ secured for 2014 • Funding of research projects within the first
2 months of the epidemic alert = 500 K€: 300 K€ for Chik • Funding after 2 months during the
epidemic = €0,5-6m (330 K€ for Chik)

Et ailleurs en Europe? • European Centre for Disease
Prevention and Control (ECDC): but surveillance not research
18
• MRC Centre for Outbreak Analysis and Modeling at the London Imperial College: but a particular area of research
• International consortium focused on one particular disease
• To discuss opportunities to work together as an internationally coordinated multi-funder initiative on broader aspects of infectious diseases with the goal of providing a quick research response in case of emerging epidemics
A follow-up meeting planned in octobre 2013 in Annecy, France

Response to Ebola epidemic
• Diagnostic: CNR FHV/P4 lab Emergence of Zaire Ebola Virus Disease in Guinea-Preliminary Report
Baize S et al, N Engl J Med. 2014 Apr 16.
• First meeting, July 17th : assessment of French research teams involved and of ongoing research projects
• Second meeting, August 19th: research priorities identified (diagnosis, experimental use of antiviral on NHPs, clinical trial, SHS)
• Third meeting, September 2nd: more than 20 participants Three projects already funded, 4 proposals in progress, other proposals submitted
Diagnosis
• Standardized diagnostic procedure that will differentiate Ebola from Lassa fever, Yellow fever, Congo-Crimea and other haemorrhagic diseases that are endemic in the affected countries.
Xavier de Lamballerie, Sylvain Baize, Eric Leroy
Research on antiviral molecules
• Animal model: Impact of Favipiravir on NHPs Xavier de Lamballerie, Hervé Raoul, Sylvain Baize
• In vitro: Identification of molecules that could inhibit Ebola replication and be rapidly transfered to clinic
Vincent Lotteau. Funding: ANRS
Clinical Trial • Pilot phase II clinical trial, using favipiravir (T705) in adults at early stage of
EVD in 2 centres in Guinea • To provide rapid evidence of the T705 efficacy
– Main outcome: day 28 mortality (< 40%) – 2nd outcomes: plasma RNA evolution, grade 3-4 AE, viral mutations
• Anthropology study knowledge and acceptability of new drugs in a crisis context
• 60 participants from Nov 1st to Feb 28th • Final results for clinical outcomes: March 1st • Close partnership between Guinean Ministry of Health and Inserm • Submitted to r2hc call, 8 September Sakoba Keita, Denis Malvy, IP, Inserm, CM Nord REACTing, CM ANRS Côte d’Ivoire, Hospital in Donka
Social and Human Sciences
• Rumors, disputes and controversies: perspectives from the digital world
• Questions - What are the competing representations on Ebola? - What is the relationship propagated by the traditional media and by the social networks? - Is it possible to categorize social media users? - What factors influence the credibility of controversial voices and messages? - What factors reduce or increase acceptance of public messages? • Sites
