PATIENT REFERRAL INFORMATION

Your doctor has referred you for an {EXAM CODE} TO BOOK AN APPOINTMENT, please call the bookings centre 24 hours after your GP appointment to allow us time to process this request, on: 01823 343509. Telephone booking opening times: 8:30 am to 4:30 pm Monday to Friday A trained appointments clerk will arrange a convenient date and time for your examination and may offer you a choice of hospital. For adult patients, please drink 1.5 pints of water one hour before your appointment and do not empty your bladder during this time, so that you attend with a full bladder. This is important to improve the quality of your scan. If you are under 16 years the booking clerk will give you appropriate alternative instructions. Please bring this information sheet with you to the X-ray department as this will enable the receptionist to book you in more efficiently. If we have not heard from you within 3 working days of receipt of your referral, we will either contact you by telephone or by post; however this may delay your examination. If you find you are unable to make the appointment that is given to you, please contact us as soon as possible on the above number to rearrange. What happens when I arrive? - The receptionist will check your personal details with you i.e., name, address, and date of birth. Once you have been booked in, you will be asked to take a seat in the waiting area. You will then be called by a radiographer or health care assistant either directly to the x-ray room or to a cubicle to change into a gown, which will be provided. Pregnancy – Please inform clerical staff and the radiographer if there is any chance you are pregnant, to avoid foetal exposure to radiation. What is an Ultrasound? – This is an imaging test obtained using ultrasound waves to help diagnose certain conditions, performed by either an ultrasonographer or a radiologist doctor. A probe lubricated with gel is passed over your skin. Don’t be a DNA! - A DNA is someone who “did not attend”. While we know that there are sometimes very good reasons why you cannot attend your appointment it is extremely important that you let us know. Patients who DNA will no longer be routinely given another appointment and your request will be returned to your referring doctor. Parking– is available at all sites on a Pay & Display basis. Patient Transport – for advice & bookings contact the Patient Transport Advice Centre on 01278 727444. Other Information: - unfortunately we are unable to supervise children who accompany adults having an examination. We therefore ask that unless it is absolutely necessary, accompanying children should not be brought to the department (unless they themselves are having an ultrasound).
