Optimal Treatment • Goal of primary therapy is to cure the tumor and

maximally preserve function and cosmesis • Superficial Therapy, Surgery, or Radiation

• Best results generally obtained with surgery • Control and cosmesis

• Consider function, cosmetic outcome, age, comorbidities, cost, treatment availability, and patient preference

Superficial therapies: Topical 5-FU or imiquimod, photodynamic therapy, cryotherapy. Cure rates lower, so reserved for cases where surgery or XRT are impractical or contraindicated. May be considered as first line for low risk shallow cancers (Bowen’s, superficial BCC). Surgical options: Curettage and electrodessication (no for hair-bearing areas, subcutaneous involvement or HR features on bx), excision with post-op margin 4-6 mm, Mohs with intraop frozen section (recommended for HR tumors), SLN mapping for HR SCC, parotidectomy if parotid LNs involved Local therapy options: surgical excision, curettage.electrodessication, cryosurgery, Moh’s, Topical agents, RT

Xeroderma pigmentosum (XP) is a rare autosomal recessive genetic disorder of DNA repair in which the ability to repair damage caused by ultraviolet (UV) light is deficient.[2]:574 In extreme cases, all exposure to sunlight must be forbidden, no matter how small; as such, individuals with the disease are often colloquially referred to as "Moon child".[3] Multiple basal cell carcinomas (basaliomas) and other skin malignancies frequently occur at a young age in those with XP; metastatic malignant melanoma and squamous cell carcinoma[4] are the two most common causes of death in XP victims. Nevoid basal-cell carcinoma syndrome (NBCCS), also known as basal-cell nevus syndrome, multiple basal-cell carcinoma syndrome, Gorlin syndrome, and Gorlin–Goltz syndrome, is an inherited medical condition involving defects within multiple body systems such as the skin, nervous system, eyes, endocrine system, and bones.[1] People with this syndrome are particularly prone to developing a common and usually non-life-threatening form of non-melanoma skin cancer.
