HEBAHAN BERKAITAN KEMASUKAN PELAJAR UNIVERSITI SAINS MALAYSIA (USM)
Berikutan peningkatan kes positif COVID-19 serta rekod pengesanan kes baharu yang membimbangkan di seluruh negara sejak 1 Oktober 2020, Universiti Sains Malaysia (USM) berada dalam keadaan bersiap siaga serta mengambil pendekatan dan langkah berjaga-jaga bagi memastikan keselamatan dan kesejahteraan warganya. Pihak Universiti mengambil beberapa inisiatif untuk mengawal situasi dalam kampus termasuk hal-hal berkaitan kemasukan pelajar ke kampus. Antara keputusan yang perlu diberi perhatian adalah: 1. KEMASUKAN PELAJAR PASCA-SISWAZAH KE KAMPUS
• Semua pelajar pasca-siswazah (tempatan atau antarabangsa) yang berada dalam negara dibenarkan akses keluar-masuk ke kampus atas urusan pembelajaran dan penyelidikan DENGAN SYARAT: ✓ Mempunyai kelulusan khas Dekan Pusat Pengajian (Borang A, dilampirkan) – Sila
dapatkan kebenaran baharu untuk Semester 1 2020/2021. ✓ Menetapkan tarikh untuk memasuki kampus dan mengisi borang deklarasi kesihatan
dalam talian tiga hari sebelum tarikh pulang ke kampus. ✓ Hadir dan terus ke fasiliti kesihatan dalam kampus untuk saringan COVID-19 pada
hari pertama memasuki kampus. ✓ Mematuhi Standard Operasi Prosedur (SOP) Universiti dalam membendung
penularan COVID-19 (PTJ juga diminta memastikan semua kemudahan mematuhi Garis Panduan Operasi Makmal Pasca PKP COVID-19 yang telah dikeluarkan oleh Bahagian Penyelidikan dan Invovasi).
✓ BUKAN dari ZON MERAH (dengan merujuk kepada carta info terkini Crisis Preparedness and Response Centre-CPRC, Kementerian Kesihatan Malaysia).
2. KEMASUKAN PELAJAR IJAZAH PERTAMA (TAHUN 2, 3 & 4) KE KAMPUS
• Pelajar lama TIDAK DIGALAKKAN untuk pulang ke kampus sehingga tamat cuti pertengahan semester iaitu pada 7/12/2020 (Tertakluk kepada perkembangan semasa kes COVID-19).
• Pun begitu, kebenaran masuk pelajar lama ke kampus DIBENARKAN kepada kes-kes tertentu mengikut jadual seperti di bawah:
Pelajar tahun akhir yang perlu menyiapkan projek tahun akhir (FYP) dalam semester 1, atau keperluan mendesak lain

24 & 25 Oktober 2020
Pelajar tahun 2 dan 3 (kumpulan B40 dan B10) yang berkeperluan khas/mempunyai masalah capaian internet yang terhad untuk pembelajaran dalam talian
Pelajar tahun 2, 3, dan 4 yang diminta masuk ke kampus oleh pusat pengajian

• Kemasukan pelajar yang seperti dalam jadual di atas di dalam kampus HANYA DIBENARKAN DENGAN SYARAT: ✓ Mendapat kelulusan Dekan (Melalui Borang Kebenaran Khas - Borang A,
dilampirkan). ✓ Menetapkan tarikh untuk memasuki kampus dan mengisi borang deklarasi kesihatan
dalam talian tiga hari sebelum tarikh pulang ke kampus. ✓ Hadir dan terus ke fasiliti kesihatan dalam kampus untuk saringan COVID-19.
• Pelajar dari ZON MERAH tidak dibenarkan pulang ke kampus. (Rujuk hebahan rasmi dan carta info terkini dari Crisis Preparedness and Response Centre-CPRC, Kementerian Kesihatan Malaysia).
• Pihak Universiti akan mengaktifkan semula Borang A, Borang Kuning, Borang Merah dan Borang Putih (Borang-borang khas kebenaran keluar masuk ke kampus).
3. KEMASUKAN PELAJAR IJAZAH PERTAMA (Pelajar Baharu) KE KAMPUS
• Pelajar baharu USM akan memulakan pengajian sepenuhnya secara dalam talian.
• Tertakluk kepada keputusan universiti pada masa akan datang, tarikh terawal pelajar baharu akan masuk ke kampus adalah selepas cuti pertengahan semester (7 Disember 2020). Sila beri perhatian kepada pengumuman yang akan dilakukan pada masa akan datang.
Pelajar-pelajar di Kampus Kesihatan dan Kampus Kejuruteraan adalah tertakluk kepada kebenaran dari Pengarah Kampus masing-masing.
Pelajar perlu sentiasa peka kepada pengumuman dari universiti dan pusat pengajian masing-masing.
Sekiranya mengambil keputusan untuk kembali ke kampus, pelajar adalah tertakluk kepada sebarang keputusan semasa pihak Universiti. Jika sekiranya Universiti mengambil langkah menghadkan akses keluar-masuk (kawalan pergerakan keluar-masuk kampus), pelajar perlu mematuhinya bagi menyokong usaha Universiti mengawal risiko dan memastikan kampus bebas dari sebarang kes-kes berkaitan COVID-19.
Dalam tempoh Perintah Kawalan Pergerakan Pemulihan (PKPP) ini, semua pelajar digesa supaya sentiasa mengamalkan kawalan kendiri termasuk penjarakan fizikal, memakai pelitup muka, kerap mencuci tangan dengan air dan sabun atau hand sanitizer di samping penjagaan kesihatan bagi mengelakkan diri dari dijangkiti penyakit.
Sekiranya mengalami gejala COVID-19, pelajar hendaklah segera mendapatkan rawatan dari fasiliti kesihatan yang berdekatan. Rawatan awal dapat mengurangkan risiko penyakit menjadi lebih kritikal. ANNOUNCEMENT REGARDING THE ADMISSION OF STUDENTS
TO UNIVERSITI SAINS MALAYSIA (USM)
Following the increase in COVID-19 positive cases and the worrying trend of recently identified cases throughout the country since 1 October 2020, Universiti Sains Malaysia (USM) is ready to take the necessary approach and precautionary steps to ensure the safety and wellbeing of its community members. The University is taking several initiatives in dealing with the situation on campus including matters regarding the admission of students to the campus. Among the decisions reached and to be given attention to are: 1. ADMISSION OF POSTGRADUATE STUDENTS TO THE CAMPUS
• All postgraduate students (local or international) currently staying in the country will be allowed access into and out of the campus on matters relating to study and research PROVIDING THAT: ✓ A special approval from the Dean of the School (Form A, as attached) has been
obtained – Do request for the latest approval for Semester 1, 2020/2021. ✓ A date has been set to enter the campus and to fill in the online health declaration
form three days prior to returning to the campus. ✓ Return and immediately go to the Health facility on campus for COVID-19 screening
on the first day of returning to the campus. ✓ Abide by the Standard Operating Prosedures (SOP) of the University in curbing the
spread of COVID-19 (PTJs are also requested to ensure that all facilities comply to the Guidelines on Lab Operations Post-MCO COVID-19 which has been issued by the Division of Research and Innovation).
✓ Not coming from the RED ZONES (by referring to the latest information chart on Crisis Preparedness and Response Centre-CPRC, Ministry of Health Malaysia).
2. ADMISSION OF UNDERGRADUATES (YEARS 2, 3 & 4) TO THE CAMPUS
• Returning students ARE NOT ENCOURAGED to return to the campus until the mid-semester break ends, which is on 7/12/2020 (Subject to change depending on current development of COVID-19 cases).
• Nevertheless, returning students who fall under these categories WILL BE ALLOWED to return to the campus based on cases, according to the schedule below:
Final-year students who need to complete their final-year project (FYP) in Semester 1, or other pressing needs
Saturday & Sunday
24 & 25 October 2020
Students year 2 and 3 (from the B40 and B10 groups) with special needs/having internet connectivity problems for online learning
Students year 2, 3, and 4 whom are requested to enter the campus by the respective Schools

• Admission of students into the campus as shown in the table above ARE ONLY ALLOWED WITH PERMISSION: ✓ Approval from the Dean (through a Special Approval Form - Form A, as attached)
has been obtained. ✓ A date has been set to enter the campus and to fill in the online health declaration
form three days prior to returning to the campus. ✓ Return and immediately go to the Health facility on campus for COVID-19 screening
on the first day of returning to the campus.
• Students coming from the RED ZONES are not allowed to return to the campus. (please refer to the latest information chart on Crisis Preparedness and Response Centre-CPRC, Ministry of Health Malaysia)
• The University authorities will reactivate Form A, Yellow Form, Red Form and White Form (Special forms for permission to enter and exit the campus).
3. ADMISSION OF UNDERGRADUATES (New Students) TO THE CAMPUS
• New USM students will continue their learning session fully online.
• Subject to the decision of the University in the future, the date that new students can enter the university the soonest is after the mid-semester break (7 December 2020). Please be aware of announcements which will be made later.
Students at the Health Campus and Engineering Campus will be subjected to the permission from the respective Campus Directors.
Students need to always be aware of announcements from the University and the respective Schools.
If a student decides to come to the campus, the student will be subjected to any decision made by the University. If the University sets measures to restrict access on entry-exit (campus entry-exit movement control), the student needs to abide by it in supporting the efforts of the University to manage the risk and ensure that the campus is free from cases relating to COVID-19.
Throughout the Recovery Movement Control Order (RMCO) period, all students are urged to always practice self-regulation including physical distancing, wearing a face mask, frequently washing the hands with soap and water or using a hand sanitizer in addition to taking care of their health as preventive measures against being infected. If there are symptoms of COVID-19, a student needs to seek treatment at the nearest medical facility. Early treatment can reduce the risk of the illness from becoming more critical.
