0 Binding character of the QPM
This Quality Policy Manual (QPM) describes the National Metrology Laboratory's structure and
organization with regards to quality assurance in the technical services it provides. Compliance
to it assures that organizational and technical activities are planned, supervised and controlled.
The quality management system is based on the requirements of ISOIIE C 17025.2005 "General
Requirements for the competence of testing and calibration laboratories ", also referred to as
ISOIIEC 17025 in this documentation, as assessed by the Deutsche Akkreditierungsstelle GmbH
(DAkks).
The staff members of the National Metrology Laboratory NIVfl-) and other organizational units
of the Industrial Technology Development Institute (ITDI) which are directly or indirectly
involved are herewith bound to carry out their tasks in accordance with the quality policy laid
down in this QPM.
The quality management system of all NML Sections is integrated into the quality management
system of the NML of ITDI. The Quality Manager takes the lead role in the documentation and
development of the management system for the NML. The NML Chief coordinates on behalf ofthe ITDI Director the planning, execution and review of quality assurance
Laboratory and is responsible for the maintenance and control of the QPM.

1.0 General
The National Metrology Laboratory NML) is one of the divisions of the Industrial Technology
Development Institute (ITDD of the Department of Science and Technology (DOST).
The NML is mandated by law to provide metrology and related services to industries and other
sectors, and is committed to continually provide reliable and accurate service as documented in
this Quality Policy Manual (QPM).
The NML accomplishes its mandate by providing the following services:
- testing, calibration and verification of measuring instruments;
- consultation and training in metrolog,t;
- proficiency testing; and
-te chnical as s e s sm ent of cali br ati on lab or at or i e s.
The NML provides testing and calibration of measuring instruments in thefollowing disciplines:
- Mass, Force and Pressure
- Length
- Viscosity, Density, Volume and Flow
- Thermometry and Hygrometry
- Electrical, Time, Frequency, and Photometry.
When requested and on meritorious cases, the NML provides on-site services at customer's
location in most ofthe listed disciplines. All services are performed in accordance with the
requirements contained in this }PM. The guidelines contained in this Quality Policy Manual
(QPM) applies to all employees ofthe NML and meets all the requirements set forth bythe
ISOIIEC 17025 "General requirements for the competence of testing and calibration
laboratories" (Edition August 2005).

1.2 Memberships
1.2.1 Asia Pacilic Metrolory Programme (APMP)
The Philippines through the Industrial Technology Development Institute (ITDD maintains full
membership in the APMP, the Regional Metrology Organization (RMO) for the Asian countries.
ITDI has been a full member since 1980.
The NML, as the national metrology institute of the Philippines, represents the country in the
APMP and participates in the Developing Economies Committee (DEC) meetings, Technical
Committee meetings and General Assembly held annually.
Participation of NML in the Technical Committee (TC) meetings ensures that the NML is
informed of international developments in the relevant metrology areas where existing
measurement capability exists. Inasmuch as the NML would want to have representatives in all
the relevant TCs, participation in the TC meetings is limited in the fields having DAkkS
accredited scopes.
The NML also takes part in technical training courses and workshops organized by the various
technical committees of APMP and the DEC aimed at further enhancing the technical
capabilities ofthe NMIs in specific measurements.
The NML also engages with other NMIs in the Asia-Pacific Region in improving regional
metrological capability by participating in intercomparison measurements that are linked to the
worldwide intercomparison measurements. The NML is required to participate in the APMP-
organized intercomparison measurements particularly in the fields where there is an existing
capability.

1.2.2 Asia Pacific Legd Metrology Forum (APLMF)
The Philippines through ITDI is a full member economy of the APLMF since September 3,
L999. The NML represents the country in the regional meetings held annually and participates
in the training courses and workshops offered for the members.
1.2.3 General Conference on Weights and Measures (CGPM)
The Republic of the Philippines through the ITDI became an Associate State of the General
Conference on Weights and Measures on June 1, 2OO2 and has signed the Mutual Recognition
Arrangement referred to as the International Committee for Weights and Measures - (CIPM
MRA) in Decemb er 9, 2002.

The NML management is committed to.
- provide its customers the highest quality of measurement services, and integrity of data
in certificates and reports;
- implement goodprofessional laboratory prdctices; and
- comply fully with the requirements of ISO,4EC 17025:2A05 cmd continuously improve

The management system is the framework under which the quality of NML activities is defined,
organized and maintained. This system is documented in its Quality Policy Manual (QPM) to
de{ine formally and clearly qualrty policies, rules and procedures . The QPM is made available to
concerned staff so that they are guided on their respective roles, duties and responsibilities
towards a competent laboratory.
All NML staffhave the responsibility to familiarize themselves with and adhere to all relevant

We shall establish, maintain, and disseminate the national
standards of units of measurements to provide international
traceability to the measurements done in the country.

3.2. Major Policies
The NML operates in conformity with the ISOiIEC 17025:2005 standard.In order to ensure
a high-level of quality in the services it provides to its customers, the National Metrology
Laboratory Nlvtr-) implements and maintains a management system which is described in
this Quality Policy Manual (QPM) and related documentation.
All staff are guided in their work by the following quality policies that are expounded in the
succeeding sections of this QPM and accompanying procedures:
1. Measurements are carried out in accordance with stated specifications and
customers' requirements.
2. In the conduct of measurements, the Laboratory only employs qualified
resources, i.e.:
a. evaluated and authorized staff;
b. validated test methods; and
c. verified or calibrated equipment, where such affects the qualitv of the
results-
3. Corollary to item 2, the Laboratory only accepts work that it is competent to
perform and has the resources to undertake.
4. The Laboratory maintainsa high-level of quality in implementing its procedures
(handling of test samples, preparation, etc.) which affects the accuracy of the
results.
5. The Laboratory has a systematic program to monitor the reliability of its
calibration or test results. This includes quality control on method
implementation.
6. The Laboratory protects confidentiality and proprietary rights of the customer.

7. The NML staff is free from undue pressure, if any, which migtrt influence their
technical judgment.
The Laboratory does not engage in any activities that would diminishconfidence
in its competence, impartiality, judgment or operational integrity.
AII NML staff, in their plans, decisions, and actions, emphasize the prevention
of problems over the correction of problems after they occur.
10. Corollary to item 9, all the NML staff, with the NML Chief in the lead role,
closely monitors the activities of the Laboratory to immediately detect potential
problems and implement the appropriatepreventive actions.
I 1. The Laboratory periodically conducts internal quality audits (IQAs) in addition
to the activity described in item 9. The IQAs determine whether the activities and
procedures of the NML conform to the requirements of ISO/IEC fiA25.2005 and
the management system it established.
12. The NML management meets at least once a year to review the effectiveness of
its management system and makes the necessary amendments.

I shall commit myself to work for the attainment of the Laboratory's mission and
implementation of the major policies defined in this section of this Quality Policy Manual

4.1 NML Organization
4 1.1 The National Metrology Laboratory G.Ovfl-) is one of the major divisions of the Industrial
Technology Development Institute (ITDI) of the Department of Science and Technology
(DOST) recognized under Execative Order No. 366, series 2004 (Government
Rati onali z ati on Pr o gr arn) .
ITDI and DOST are government entities created under Executive Order No. 128 of 1987
(Reorganizing the National Science and Technology Authority).
Office and sections of NML are located at the Metrology Building, DOST Compound,
General Santos Avenue, Bicutan, Taguig City, Metro Manila, Philippines. Annex A shows
the layout of the ffices and sections of NML. Details of the sections are described in their
respective Laboratory Quality Mamtal (LQM).
The NML is covered by policies and procedures of ITDI with respect to the hiring of
personnel, procurement of equipment and materials and other administrative matters.
Complimenting the ITDI policies and procedures are the NML poticies and procedures
contained in this Quality Policy Manual (QPM)

NML's base laboratory. Works are guided accordingly by appropriate calibration
procedure s and work instructions.
4 1 4 ITDI provides budget and administrative support for the procurement of equipment and
services, and hiring of personnel of the NML. However, this does not diminish the
independence of the NML on the selection and acceptance of equipment, services and
personnel. Other Divisions of ITDI have their own responsibilities and are not involved in

4.r.s.3 NML CHIEF (DAkkS Head):
The NML Chief is responsible for ensuring that the activities of the NML relatedto the
provision of the metrological services conform to its policies and the requirements of
ISO/IEC 17025. This responsibility encompasses planning, monitoring, and controlling
technical and administrative activities and resources to support the operations of the
NML.
In discharging this responsibrhty,the NML Chief delegates:
-operational control of metrologicalservices to Section Heads;
- management of the NML quality system to the Quality Manager.
The NML Chief authorizes arrangements to review and audit the management
system at appropriate intervals, appraise the results, and implement any corrective
and preventive actions. The procedures are given in OM 4.14: Internal Audils and
OM 4.15: Management Review. The Quality Manager ensures these are
implemented.
The NML Chief reports directly to the ITDI Director and is responsible to set up an
organizational structure, allocate appropriate resources, and formulate policies and
procedures necessary to maintain the management system.

d. Plan and implement quality control activities (interlaboratory comparison,
external calibration, in-house calibration) to ensure that measurements
done are traceable to the definition of the SI units.

4.2.1 A managemeilt system has been established, implemented and maintained in accordance
wrth ISO/IEC 17025. The management system covers all work carried out at the National
Metrology Laboratory NI\dL) base laboratory, or on-site at the facilities of the customer.
The system's documentation are communicated to, understood by, and available to NML
stafi and implementedby the all.
4.2.2TheNML's management system policies related to quality, including its quality policy

The Laboratory Quality Manual GaIO is a manual which contains detailed and specific
policies, standard procedures, technical requirements, and forms u,hich are appropriate and
applicable only to a specific Section. However, it follows the concepts outlined in the
Quality Policy Manual and Operations Manual.
4.3.1.1 Document Control Procedure:
Procedure for control of documents and authorized persons responsible for the
changes in and the review and approval of documents are described in OM 4.3:
Document Control.
4.3.2 Document/Procedure Approval and Issue
4.3.2.1Procedures and documents are reviewed and approved for adequacy by an
authorized person prior to issue, and are reviewed as the need arises to ensure
appropriateness and applicability. OM 4.3: Document Control identifies NML staff
authorized to review and approve quality system documents. A Master List ofDocuments identifies the cutent version of a document and is easily available to
prevent the use of invalid andl/or obsolete documents.
4.3.2.2 The procedures adopted ensure that:
a. authorized editions of appropriate documents are available at designated Sections
where operations essential to the management system are performed,
b. reviews on all documents within the management gtstem are conducted as the

c. invalid or obsolete documents are removed from all points of issue or use, or
otherwise assured against intended use before the revised and approved
document is issued; and
d. obsolete documents being retained for legal purposes are properly marked and
archived.
4.3.2.3 All quality documents generated by the NML contains at least the following
information:
a. title of the document/section;
b. document/section number;
c. version number
d. page number (including the total number of pages per document/section);
e. printed name and signature of preparer, and date prepared; and
f printed name and signature of approving authority, and date approved.
4.3.3 Document Changes
4.3.3.1 Changes to procedures and documents in the form of additions or deletions are
reviewed either by the Section Head or Quality Manager, and thereafter approved
by the NML Chief before being issued for use.
4.3.3.2 Modifications/alterations to procedures or documents are suitably identified and
required a conesponding change of the preparation date and version mtmber, and

4.3.3.3 Amendtnents of documents by hand pending the re-issue oJ the documents are
made by the Quality Marruger for the Quality Policy Marual, Operations Mamral,

4.4.1 Procedures are established for the review of requests, tenders and contracts (OM
4.4.1. Review of Requests and Contracts). This review process addresses the
following concerns:
4.5.1.1. Services are adequately defined and documented;
4.5.1.2. NML has the ability and resources to meet the customers'
expectations, which may be communicated orally or in writing,
4.5.1.3. Appropriate test or calibration method to conduct the requested
services is selected;
4.5.1.4" Any differences in expectations and services to be performed are
resolved before any work is performed. This ensures that the
requests, tenders, and/or the contract are acceptable to both the
NML and the customer.
4.5.2.Records of the customers' requirements, and any pertinent information exchanged
during the required service are mai*uned as quality record.
4.5.3.The review process covers any work performed by the NML.
4.5. .The customer ls informed of any deviation from the contract.
4.5.5.If the contract has to be amended after work has coflrmenced, the same contract

4.6 Purchasing Services and Supplies
4.6.1 It is the policy of NML to purchase supplies and services of acceptable quality
and from reputable sources. Procedures are established to cover the purchase,
reception and storage of items necessary for the operation of the laboratory. (OM

4.6.1.1 Vendors may be qualified based on past performance records,
certifications, audits and surveys performed by acceptable sources. This
process allows a vendor to be used for occasional purchases without
going through the approval process.
4.6.2 All purchased supplies and consumable materials that affect the quality of
calibrations are not used until they have been inspected or verified as complying
with specifications or requirements defined in methods for the calibrations
concerned. Inspection reports/records are maintained by the Laboratory.
4.6.3 Purchasing documents will be reviewed and approved for correctness prior to
being released. The document will contain appropriate data for the services or
products being requested.
4.6.4 It is the responsibility of the Bids and Awards Committee (BAC) of the ITDI to
maintain a list of approved vendors. All potential vendors will be evaluated as to
their ability to consistently comply with quality requirements. Records are
maintained to prove compliance to any purchase order issued by ITDI.

4.9.1 ltis the policy ofNML to manage and control work or activities that do not conform to its

calibration work are to be handled.

4.11 Corrective Action
4.11.1 It is the policy of NML to implement corrective actions promptly when any
nonconforming work or departures from its policies and procedure is identified.
4.11.2 An investigation to determine the root cause of the problem is carried out at the start of
the corrective action procedure.
4.11.3 The Section selects and implements appropriate corrective action as identified to resolve
the problem and prevent its recurrence. If there are any changes resulting from the
corrective action, these are documented and implemented.
4.11.4 The Quality Manager reviews the root cause of the problem and corrective action taken
and, ensures its appropriateness. Implementation of the corrective action is monitored for
its effectiveness.

5.4 Calibration Methods and Method Validation
5.4.1 The NML uses appropriate and validated methods and procedures. These cover handling,
preparation, use, operation and storage of test/calibration items, as well as estimation ofmeasurement uncertainties and statistical analysis of data. All are kept uplo-date and
readily accessible to authorized stafl,
Deviation from standard methods is allowed provided that it is validated, documented,
accepted by the customer, and authorized by the NML Chief or Section Head.
5 4 2 lhe National Metrologt l-aboratory @ML) uses test and/or calibration methods that meet
customer needs, appropriate for the testing/calibration work undertaken. These methods
are based on the latest valid edition published either in international, regtonal, or national
standards, or by reputable teclmical organizations, or in relevqnt scientific texts or
iournals, or as specified by the manufacturer of the equipment. If needed, NML uses
laboratory-developed methods that have been validoted catd documented however they
must be appropriate and acceptable to the customer. NML informs its customers if the
method proposed is irnppropriate or otttdated.
A list of technical procedures and worlc instructions are found in the Table of contents ofthe Sections' Laboratory Quality Manuals.
5.4.3 Development of test and/or calibration methods by the Sections for their own use is a
planned activity and performed by qualified personnel equipped with adequate resources.
As development proceeds, plans are updated and effectively communicated to all
concerned.

5.4.4 If there is a need to use non-standard methods such as methods developed by the NML, it
seeks the approval of the customer and includes a clear speeification of the customer's
requirements and the purpose of the test and/or calibrqtion. The methods are validated
prior to use.
5.4.5 Laboratorydeveloped methods, non-standard methods, modfied sandnrd methods or
those used oatside their intended use are validated. Validation is as extensive as necessary
to confirm that they are fit for their intended use. The validation process is documented
which includes the results obtained, validation procedures used, and indication the method
is fit for its intended use. The NML ensures that the scope of the validated method is
relevant to the customer's need.
5.4.6 The NML adapts the ISO Guide to the Expression of Uncertainty in Measurement, 1993;
as basis for expressing its uncertainty in measarement. The general procedure for
estimating uncertainty of measurement is given in OM 5.4.6: Estimation of Uncertainty.
All uncertainty components affecting the results in a test and,'or calibration work are taken
into accountwhen estimating the uncertainty of measurement.

5.5 Equipment
5.5.1 The NML is equipped with reference standards and equipment required/or the coruect
per/ormance of measurement services.
5.5.2 NML uses laboratory equipment and associated software that are capable of achieving the
accuracy required for a particular test/calibration work. A calibration program for
reference standards and equipment has been established. They are calibrated before use
and regularly thereafier at established intervals.
5.5.3 Laboratory equipment are only operated by authorized staff. Their operation, maintenance
and service manuals are filed and stored to allow easy retrieval and access.
5.5.4 Laboratory equipment, including its software, which affects signrficantly measurement
resalls are assigned a unique identification number.
5.5.5 Records are maintained for each equipment and its software. These ,ecords include the
following informati on :
5.5.5.1 identity of the equipment and its software;
5.5.5.2 manufacturer's name, type identification and serial number or other unique
identilication;
5.5.5.3 compliance of the equipment to specification;
5.5.5.4 the current location, where appropriate;

5.5.12 All laboratory equipment a.re safeguarded against unauthorized adjustments using tamper
proof seals/stickers or passwords. If tampering is evident or suspected, the equipment is
treated as nonconforming. Broken seals require the equipment to be removed from service
until an evaluation has been made to ensure that the equipment is suitable again for
service. Access to and use oif measurement areas are only limited to Laboratory staff and
its management. Persons not worhng with the Laboratory are allowed access to
measurement areas only if they are accompanied by l-aboratory stafi. Laboratory doors
are kept locked when not in use and at the close of day. Laboratory room keys are only
gwen to Laboratory personnel. Access is made lcnown to all by placing a signboard at the
entrance. Cleaning andfacilities maintenance staffs are only allowed sapervised access to

5.6 Measurement Traceability
5.6.1 All NML standards and measuring instruments are calibrated before being placed into
service and regularly thereafter according to an established program and procedure. Refer
to OM 5.6: Measurement Traceability.
5.6.2 Specific requirements
5.6.2.1 Calibration
5.6.2.1./ The link to SI units is achieved by the use of reference standards that
are calibrated by National Metrology Institutes CNA/tr$ with Calibration
and Measurement Capabilities published in the Bureau International
des Poids et Mesures (BIPM) Key Comparison Database of the CIPM
MRA in the relevantfields. Auxiliary equipment are calibrated either by
these NMIs or calibration laboratories accredited under ISO,/IEC
I 7025:2005.
5.6.2.1.2 NML participates in regiorml interlaboratory comparison programs
wherever available and possible. Intercomparison results, reports and
evaluation are filed and kept as a record. See LQM Section l0Recording and Filing System.
5.6.2.2 Testing

5 . 6. 3 . i NML has a program and procedure for the traceable calibration of its reference
standards (OM 5.6: Measurement Traceability). These reference standards of
measurement are used only for calibration. Reference standards are calibrated
before and after any adjustments are made.
5.6.3.2 Reference standards are traceable to SI units where possible.
5.6.3.3 Intermediate checks on reference and working standards are performed at
specified intervals according to the defined procedures to maintain the confidence

5.8 Handling of Test and Calibration Items
5.8.1 The National Metrolog,t Laboratory NML) has established procedures for the
transportation, receipt, handling, protection, storage, retention and releasing of test and
calibration items of its customers. OM 5.8: Handling of Test and Calibration ltems
provides procedures to protect the integrity of the test and calibration items as well as the
interest of the NML and its castomers.
5.8.2 Test and calibration items received from customers are individually and uniquely
identified ttsing Technical Service Request Numbers (TSR No.) which is retained
throughout its stay with NML. This ensures that the items are not confused or mixed with
other items, or when referred to in records or documents.
5.8.3 Upon receipt of items for calibration, records are made of any abnormalities or departures
from conditions specified in the test/calibration method. If there is doubt on ils suitability
or does not conform to requirements, the NML informs the customer and documents and
records
5.8.4 The NML has procedures for avoiding deterioration, loss or damage to the test and
calibration item during storage, handling, and preparation. When items need to be stored or
conditioned under specified conditions, these conditions are maintained, monitored, and
recorded.

5.9 Assuring the Quality of Tests and Calibration Results
5.9.1 The National Metrologt Laboratory \IML) implements a monitoring scheme to ensure the
qualrty of the measuremenl results it reports. It is found in OM 5.9.2: Assuring the Quality
of Test and Calibration Results. With this proce&tre, the NML reqrds resulting data and
applies statistical techniques to detect trends. The NML plans and reviews this scheme
which include but not be limited to the following:
- internal quality control schemes using statistical techniques;
- participation in interlaboratory comparison or proficiency testing programs,
- regular use of certified reference materials, check standards or secondary reference
materials; and
- replication of tests and calibrations using the same method.
5.9.2 Quality control data are analyzed and if found to exceed pre-defined limits, a review of
the measurement system is initiated, and actions are taken to correct the problem lftzrs
preventing incorrect results from being reported.

Sample test reports and calibration certificates are presented in the annexes of the

5.10.3 Test Reports
For verification tests, the report contains statement of compliance or non-compliance
with requirements or specifi cations.
The test report only relates to the items tested. If a statement of compliance to
specifications is made, pertinent clauses of the specifications that are met or not met are
identified.
If required, uncertainties of measurement are taken into account on the determination ofcompliance.
5 . 10.4 Calibration Certificates
5.10.4.1 The following additionat information appears in the calibration certificates,
where necessary for the interpretation of the calibration results:

Terms of quality assurance
This QPM uses the designations and definitions of quality as$Irance in accordance withthe latest versions of the standards ISO/IEC 17025.
Quality Policy Manual (QPM) contains the mission and policies of the NML andcommitments of the top management to the quality aspects of its calibration activities.

TraceabilityThe process of relating measurement results to national standards through an unbrokenchain of calibrations. (DKD)
National (Measurement) StandardA measurement standard recognizedby national authority to serve in a state or economyas the basis for assigning quantity values to other measurement standards for the kind ofquantity concerned. (VIM 5.3)
Reference (Measurement) StandardA measurement standard designated for the calibration of other measurement standardsfor quantities of a given kind in a given organization or at a given location ryq4,5 6)
Working (Measurement) StandardA measurement standard that is used routinely to calibrate or verifii measuringinstruments or measuring systems. (VIM 5.7)

Calibration and Measurement Capabilities"In the context of the CIPM MRA and ILAC Arrangement, and in relation to the CIPM-ILAC common Statement, the following shared aerriition is agreed upon:a CMC is a calibration and measurement capability available to customers under normalconditions: a) as published in the BIPM key comparison database (KCDB) of the CIPMMRA; or (b) as described in the laboratory's scope of accreditation granted by asignatory to the ILAC Arrangemenr.u (WD CIPM 2AO7-tl)
Quality RecordsThe Quality Records contain the results and evidences of the implementation of thequality policies and procedures. They include reports from internal audits, managementreviews as well as records of corrective and preventive actions.
Technical RecordsThe Technical Records are records of original observations, derived data and informationto establish calibration records, staff records, and copies of calibration certificates issued(see QPItd, section 4.13).
