Box 2206 Saihat 31972, Kingdom of Saudi Arabia. The company is headed by the founder Engr. Romeo Quilas Diaz a retired Saudi Aramco employee and managed by highly renounced professional in its field of discipline. We are specialized in the field of lifting equipment inspection, crane/lifting LMI system calibration, crane/lifting equipment maintenance and repair. WE are also specialized in the field of calibration of welding machine, mechanical instrument, and pneumatic instrument. Our major clients are Saudi Aramco contractors, SABIC contractors, and SCECO contractors all over the Kingdom of Saudi Arabia. The company management teams have a clear focus on individual responsibility and a high level expertise. Our manpower supply and equipment rental business are gaining a sustainable growth in the field. 1.0 SAFETY & HEALTH POLICY

FORTUNE INDUSTRIAL SERVICES EST. believes that NO Job or No Task is more important than worker Health and Safety. If a job represents a potential safety or health threat, every effort will be made to plan a safe way to do the task. Every Procedure must be a safe procedure. Shortcuts in safe procedures by either foreman or workers will not be tolerated. If a worker observes any unprotected job, which may pose a potential threat to their health or safety, he or she must inform management and management must take adequate precautions. OUR FUTURES ARE ONLY BUILT THROUGH OUR PEOPLE.WE AIM TO PROTECT THEM. 2.0 SAFETY & HEALTH OBJECTIVES FORTUNE INDUSTRIAL SERVICES EST plans to achieve worker safety and health through the following: • Using a qualified safety person. • Making regular job site safety inspections. • Enforcing the use of safety equipment. • Following Safety procedures and rules. • Providing on-going safety training. • Enforcing Safety rules and using appropriate disciplines. 3.0 EDUCATION & ORIENTATION 3.1 Instructions and guidelines will be provided by the safety Engineers and/or Foreman, as is required by the changing nature of the work and personnel. 3.2 Any work area, which is considered hazardous, will be tagged. Barricaded and appropriate warning sign will be posted. 3.3 The same safety practices procedures; orientation and instructions are to be provided to subcontractors working on the site. 3.4 Should it be deemed necessary, meetings will be held on site to re-educate employed personnel to safety requirements. 4.0 SAFE WORK PRACTICES & PROCUDURES The safe practices described in this chapter are for employee's protection. All employees shall read and observe them, accidents happen without warning, and many are caused by the lack of knowledge, in-attention and carelessness. Lack of knowledge of these safe practices will not be justifying non compliances.



5.0 HOUSE KEEPING: Good Housekeeping is an important part of the safety program. It is the responsibility of all employees, supervisors and craftsman alike to practice good housekeeping. 6.0 VIOLATION CONTROL AND PUNISHEMENT To insure safety, disciplinary measures shall be imposed to violators of safety regulations, rules and/or requirements, for instances;

I. Anyone whose thoughtlessness, inattention or lack of knowledge has resulted in injury or property damage.

II. Anyone whose violation of safety or security regulation has resulted III property damage or hindrances

to the orderly working operations.

III. Anyone who fails to report any kind of accident or tries to cover up one. 7.0 ACCIDENT REPORTING PROCEDURE:

I. All Accidents that occur on the project site or reroute to and from the project site are to be reported immediately.

II. First -- Take care of the injured.

III. In case of Injury, requiring ambulance or medical attention at the scene of the accident.

Call the Hospital using any phone on the project or near-by. 7.3 Second- Report Accident.

IV. During Normal or After Work Hours. Report immediately to the Safety Supervisor who shall in turn inform the Authorized Representative in the Branch Office.

V. Contacting GOSI.

All accidents occurring on sites must be reported to GOSI. The site safety Engineer or Supervisor must report all accidents to the Main Office.

8.0 SAFETY INSPECTION FORMS: 8.1 The Standard forms developed by our Est. will used in cases of accident reporting etc.
