PF/2 • Student Professional Portfolio
• Purpose of portfolio
o To “showcase” credentials not available on a resume’ o Collections of papers demonstrating best ideas, work and accomplishments while
a student nurse o Reflects progress through nursing program, as examples are from start of the
program until completion o Learning tool to be utilized throughout and at completion of nursing program o Contents may be helpful in reflecting best work during employment interview
• General Directions
o Portfolio is divided into sections, each of which has specific directions for the papers and documentation that should be included.
o All papers and documentations should be typed on unlined paper, using APA (5th Ed) format, when applicable.
o Recommended: • Keep second photocopied portfolio, in the event the original is lost
o Keep contents in a notebook – ½ -1 inch width. o Portfolio should be started the 1st semester of the nursing program. o Maintenance of the portfolio is the responsibility of the student. o Faculty may periodically request review of portfolio.

• Purpose o To enable the student nurse to periodically gain insight into his/her professional
and personal strengths and weaknesses, that will impact on one’s career. o To develop an action plan for improvement and to evaluate the effectiveness of
the improvement plan
• Directions o At the beginning of each semester
• Make an entry on the self-evaluation form, stating at least three (3) strengths and three (3) areas needing improvement.
o The action plan should include expanding the identified strengths and improving the areas identified as needing improvement.
o At the end of the semester, the success of action plan should be evaluated. PF/4

• ATI Assessment Evaluations
• Purpose: o To enable the student nurse to periodically gain insight into his/her academic
strengths and weaknesses o To develop an action plan, if necessary, for improvement in nursing education
knowledge base, necessary for successful completion of curriculum o Failure to demonstrate satisfactory performance on the comprehensive tests and
successful remediation, if necessary, the student may not progress in the Nash Community College ADN program
• Directions: o After each ATI test, the student should review evaluation of test, with academic
advisor o Should remediation be necessary, student will complete required remediation
plan o All test results and evaluations should be filed accordingly

• Clinical Experiences
• Purpose:
o To enable the student nurse to account for clinical focus experiences while a
student in the nursing program o To provide evidence of specific planned clinical rotation experiences o To provide evidence of specific clinical skills competency o To provide evidence of ability to apply the nursing process while caring for a
client
• Directions: o List the course, dates and clinical setting for the clinical experiences per
semester o Include a description of student role and the specific practice experience
obtained for each clinical rotation o File a rotation schedule for each semester o File selected evaluations, papers, projects appropriate for clinical
experiences Include copies of outstanding evaluated written care studies for each
clinical experience o File check-list of Clinical competencies (AHEC) & additional skills

• Group Leadership
• Purpose: o The understanding of group dynamics, group membership and leadership
roles, and having a working knowledge of the different types of groups in health care settings is basic to the practice of nursing
• Directions: o Opportunities for participation as a member of a group, as well as a group
leader, will be provided for the development of group participation and leadership
o Include a minimum of (3) summaries (1-2 pages in length) of the experience as a group member, stating the purpose of the group and discussing the member’s role in the group process
o Include a minimum of (2) summaries (1-2 pages in length) of the experience as a group leader, stating the type of group, the purpose of the group, the goals of the group, and describing the group leadership role. (Identify strengths and areas that need improvement)

• Professional Organizations
• Purpose: o To enable the student nurse to understand the importance of professional
nursing organizations by being an active member of a nursing organization of his/her choice during the nursing education process
• Directions: o List the professional nursing organizations in which the student belongs,
including membership number and date of expiration. o For each membership, briefly describe the purpose or mission statement of
the organization o Include membership cards for each organization

• Community Organizations and Associations
• Purpose: o By becoming a member of community organizations or associations, the student
nurse can relate to the larger arena of health care needs and can also bring the image of the professional nurse into the community setting
• Directions: o List all community organizations and associations to which the student belongs o Include a brief purpose or mission statement of the organization o List activities in which the student is involved

• Purpose:
o To document any honors, scholarships, and awards received while a student nurse at Nash Community College
• Directions:
o List any health related honors, scholarships, and/or awards received during nursing education at Nash Community College
o Include the date of the award and a brief summary of the criteria for the honor o File copies of any news media recognition

• Purpose:
o To provide evidence of updated immunizations and CPR certification
• Directions: o File immunization record and CPR certification record o All records should have necessary updates, as required, yearly

• Purpose:
o To enable the student nurse to provide a written job resume’ during the final semester of nursing education at Nash Community College
• Directions: o Include a completed resume’ for a selected nursing job as a graduate nurse

program at Nash Community College
• Directions: o File diplomas and transcripts relevant to completion of the ADN program at Nash

• Purpose: o Provide 3 letters of recommendation for selected job
• Directions: o Select 3 persons to provide written evidence of recommendation for selected
nursing job o Persons selected as reference should not include friends, minister, or personal
acquaintance, but should be persons that have knowledge about student’s educational and/or clinical competences such as nursing faculty; other college faculty; nursing preceptor or employer in nursing/ job
