In order for the company to properly bill our customers in a timely fashion, and track our costs effectively, please follow the guidelines on the following pages to ensure that you are handling all procedures correctly. If you have any questions, please do not hesitate to ask. OUR PAYROLL PERIOD RUNS FROM WEDNESDAY THROUGH TUESDAY. Payroll is processed each Wednesday, except when a holiday interferes. You will always be notified ahead of time whenever the reporting day changes (This would normally only happen when Christmas and/or New Years falls on a Thursday). TIME CLOCK The time clock is located on the outside wall of the shop by the wash rack. Each employee will have a GREEN and a RED Key Tag assigned to them. GREEN means START time, RED means STOP time. The Time Clock will beep and flash green when you touch correctly. If you do not clock in, or do not clock out on any given day YOU WILL NOT BE PAID for that day, therefore it is IMPERATIVE that you clock in and out every day, however the time clock does not negate your responsibility for also turning in your timesheet and tickets (You must do both – clock in/out AND turn in tickets and timesheet). Every Wednesday you must submit your time as follows:

Call the office BEFORE 10:00 am with the hours worked. Be prepared to break out your hours by crane, yard time, or job, if required, and/or by company (Phoenix Crane, Dixie Erectors, Northeast Georgia Crane, or Phoenix Rigging).

-OR-

Turn in your time sheet and Job Tickets to our office BEFORE 10:00 am WEDNESDAY If you call in your time, you must then turn in your time sheet and Job Tickets as a backup before Friday of that week in order to be paid the following week. The only exception will be if you are out of town. It is YOUR responsibility to get the paperwork into the office, not the fuel truck driver, or your oiler, or your Mom – YOU!

PROPER PROCEDURES FOR FILLING OUT JOB TICKETS All Job Tickets must be signed by the customer, TWICE! One signature acknowledges your arrival time on the job and the second one acknowledges the job completion and that there have been no accidents on the jobsite. Mark the Job Ticket with as much information as you can regarding the customer name, job name, and job description for our billing purposes. You are also required to mark our Dispatch # where indicated in the upper right hand corner of the Job Ticket. The Dispatch Department will give you all the information they have in our system, but they may ask you to get some other information on the jobsite. Actual travel time and job time must be properly filled in on each Job Ticket, including those jobs that are a minimum charge! Be sure to list the Total Hours to be charged to the customer in the appropriate spot on the Job Ticket (see Exhibit “A” attached).

On a daily basis, the White and Yellow copies of the Job Ticket must be turned in to the dispatch office (plastic tray hanging on the wall outside the Dispatch office) for billing. The Pink copy should be left with the customer on the jobsite. You will need to hold onto the Gold copy of the Job Ticket to be turned in weekly, attached to your time sheet for payroll that week. This is the copy that is verified against your time sheet for accuracy. Job Tickets and payments for COD jobs should be turned into the office as soon as you return to the office after completing the job, following the same guidelines listed above for Job Ticket distribution by color. ONE JOB TICKET, PER JOB, PER OPERATOR, PER CRANE. Do NOT put your time on another operator’s Job Ticket, nor use the same Job Ticket for two different cranes on the same job. On monthly crane rentals, each day you must list out your hours worked, as well as a total for the week. (Since the rental period of a crane rarely corresponds with our payroll period, it is extremely important to list the hours on a daily basis so that we may break them out according to the rental billing period of the crane). Again, ONE JOB TICKET, PER JOB, PER OPERATOR, PER CRANE, NO EXCEPTIONS. See Exhibit “B” attached, for properly filling out a Job Ticket for long-term rentals. This is one Job Ticket with a full week of time, broken out by day. Weekly time sheets must reflect each job worked per day, and the total Job Ticket time for each. It must also show ACTUAL start time and ACTUAL stop time for the time you worked each day. You will be paid Job Ticket time for all jobs, whether or not you worked the billed hours (minimums), but you will never be paid more hours than what you actually worked, -OR- what we billed out on Job Tickets.

(Example: Your day started at 7 am and ended at 5 pm. You turned in four 3-hour minimum Job Tickets, which actually took you 8 hours to perform. You then worked on your crane for 2 hours after returning to the yard. You will be paid 12 hours for this day (four 3-hour minimum Job Tickets) not 14 hours to include the Job Tickets PLUS your 2 hours of yard time. Even though you only worked 10 actual hours this day (7 am to 5 pm), you did have the minimums on the Job Tickets. (See Exhibit “C” attached).

TURNING IN JOB TICKETS/TIME SHEETS: If Job Tickets and time sheets are not turned in by Friday morning each week, you will not be paid the following week. As soon as your paperwork is brought current in the office, you will then be paid. ALL EMPLOYEES, INCLUDING OPERATORS ON MONTHLY CRANE RENTALS, MUST FILL OUT, AND TURN IN, A TIME SHEET TO CORRESPOND WITH ALL JOB TICKETS AND OTHER HOURS WORKED. EVEN THOUGH YOU MAY HAVE PUNCHED IN/OUT ON THE TIME CLOCK FOR ALL OR PART OF THE WEEK, YOU MUST STILL COMPLETE A TIME SHEET AND TURN IT IN TO THE OFFICE. CRANE INSPECTIONS: Every operated crane and tractor trailer MUST be inspected daily to comply with OSHA and DOT regulations. The forms for these inspections are located on the book case outside Rick Baker’s office. There is a separate form for trucks and cranes. Please make sure you use the appropriate form. The ORIGINAL (no photocopies) of the inspection form must be turned in with your time sheet weekly. OSHA regulations require that you always have a current inspection sheet in the crane, therefore make

sure that you are inspecting the crane each morning. Not turning in a crane checklist will also be cause for your payroll to be withheld the following week. MONTHLY RENTALS: The monthly rental period to our customers begins when any crane is DELIVERED ON SITE AND ERECTION BEGINS. The monthly rental period ends when the crane is dismantled and loaded out at the end of the job. Every customer is responsible for an operator’s time during the erection/dismantle procedure. Our charge to erect and dismantle the crane does not include an operator for the crane being erected or dismantled. Each operator is responsible for getting a Job Ticket signed by the customer for this time during the erection and dismantle. If there is no operator working the crane being erected or dismantled, then our Rigging Superintendent must have the customer sign a Job Ticket for their time during the erection and dismantle. DAILY MINIMUMS: The following daily minimums apply to all long-term rentals and require a Job Ticket to be signed by the customer according to these guidelines. You will be paid according to these minimums, but must have a Job Ticket signed by the customer to back it up:

• Two (2) hours will be charged if the operator shows up for work, and is sent home. • Four (4) hours will be charged if the operator works over two (2) hours, but is sent home before

four (4) hours are worked. • Eight (8) hours will be charged is the operator works over four (4) hours, but is sent home before

eight (8) hours are worked. YARD TIME:

1) Check with dispatch at the end of each day to determine whether you are scheduled to go out the following day.

2) If there is nothing on the schedule, you are not required to report to work. Should you choose to come in, you will not be paid until you are given something to do. You will not be paid for showing up if you have not been asked to do so, and if we do not have anything for you to do.

3) If you are asked to show up for a job which cancels, you will be given 2 hours show-up time for that day.

WORKING FOR AFFILIATES: Dixie Erectors, Phoenix Rigging & Erecting and Northeast Georgia Crane are entirely separate entities from Phoenix Crane as far as billing is concerned. All work done on their jobsites, or on behalf of them, should be treated as if you are working for any outside trade customer. Their foremen, or superintendent, will sign your Job Tickets. All procedures as outlined above are to be followed for completing these Job Tickets for affiliates.
