Problem Solving

Title : Sponsor : Facilitator : Beginning date :

Participants : Expected end date : (Including implementation)

STEP 1 : DO WE HAVE A PROBLEM ? STEP 3 : HAVE WE CONFIRMED THE CAUSE-GAP RELATIONSHIP? Problem : Gap between the actual and expected results

1) Define the gap between the actual result and the expected result

What is the expected result?

What is the actual result?

What is the gap ?

2) Define the impacts and confirm them by going on the field (Gemba)

1) Identify the countermeasures to put in place to address the root cause and eliminate the gap (temporarily or permanently). If there is more than one cause, do one experiment at a time.

2) Put the countermeasures in place

3) Confirm that the countermeasures eliminate the gap. If yes : go to step 4 If no : keep going with Problem Solving

STEP 2 : DO WE KNOW THE CAUSE OF THE PROBLEM ? STEP 4 : HAVE WE ENSURED SUSTAINABILITY?

1) Contextualize the problem and identify when and where the problem was detected

2) Look for probable causes of the gap (brainstorming) and prioritize them

3) Identify the root cause for the prioritized probable cause(s).

1)Determine what actions are required to bring sustainability of the solution (e.g. Updating the standard working method, communication, training, periodic measurements, etc.)

Required actions Responsible Deadline

The problems are due to an inadequate standard, a standard that is not followed or an inadequate system.
