You're Building Permit Application and development plans for the referenced property which were submittedto the Health District on December 3, 2019 have been reviewed. The comments provided herein by the District are

based on the Information provided to the Health District by you, your AOSE/PE and VDH file record.

The pool location provided on the submitted plan appears to be acceptable.

Based on the request of the homeowner and the current licensed on-site evaiuator and/or licensed septiccontractor's submission it appears the proposed pool will maintain the requisite offsets to the on-site sewagedisposal system. The Maintenance deficiencies on file were addressed and corrected by a licensed operator.

There is no guarantee given or implied that the proposed construction will not interfere with anycomponents of the sewage disposal system and/or water supply. Further, there is no guarantee given or

implied that this sewage disposal system will continue to function properly in the future.The Department Is simply performing a courtesy review for the locality to try identify any potentialconflicts based on Information available. In the event of damage to a sewage disposal system or well duringconstruction, the owner will be responsible for any repairs or other actions deemed necessary bythe Department to correct the situation.

Section I Courtesy Review: For a pool.

For pools, decks, garages, pole barns, sidewalk installations, and other structures not designedfor human occupancy, the local building official may ask VDH to consider whether proposed constructioncould impact the existing sewage system's function. For these situations, VDH lacks authority todetermine whether the sewage system is safe, adequate and proper as contemplated by the Code.However, as a courtesy to the building official, and by request, VDH may process the request.

Please understand that the Prince William Health District has not currently implemented the useof request forms 2a and 2b {Prince William County Building Officials request forms for Health Districtreview). The subject review was a courtesy to the property owner and Prince William County.

Based on the current submission dated November 16, 2019 the application for a pool is deniedfor the following:

- The attached operation and maintenance (O&M) report from 11/13/2019 appears to note thesystem currently may have some deficiencies. Please have any deficiencies addressed andplease turn in a new O&M indicating the system is functioning properly with the new poolapplication. The deficiencies appear to be noted in red with (See Comments) on pages 1 and 2of the O&M report form 11/13/2019. GMP# 2017-03 (page 7 of 29) 5
